RESUMO -A esclerose múltipla é uma das causas mais comuns de incapacidade neurológica crônica em adultos jovens. Avaliamos, através de estudo retrospectivo, características epidemiológicas, formas de apresentação, manifestações clínicas, evolução e o grau de incapacitação física da esclerose múltipla em 302 pacientes. A média da idade dos pacientes foi 37,7 anos e a relação entre os gêneros foi 3,13F:1M. A média da idade no início da doença foi 29,6 anos. Duzentos e oitenta e três pacientes eram brancos (94%), 15 negros (5%) e 4 amarelos (1%). Duzentos e vinte pacientes (72%) apresentaram forma clínica tipo remitente-recorrente. Oitenta e dois pacientes (28%) apresentaram a forma progressiva (50% forma secundária e 50% forma primariamente progressiva). Os sintomas iniciais mais comuns foram sensitivos (31,7%) e ópticos (26,8%).Em relação aos sintomas evolutivos predominavam os sintomas piramidais (72,5%) e os medulares (64,9%). A média do escore do EDSS final foi 3,37 e a do NRS foi 85,17. O índice anual de surtos foi 0,45. Nossos achados coincidem com os da literatura mundial, e ressaltam que 60 pacientes (19,8%) apresentavam esclerose múltipla de forma benigna de valor prognóstico. Multiple sclerosis: descriptive study of the clinical forms in 302 casesABSTRACT -Multiple sclerosis (MS) is one of the most common causes of chronic neurologic disability in young adults. We evaluate, through retrospective study, the epidemiological characteristics, the manifestations ways, the clinical manifestations, the evolution and the level of physical disability of MS in 302 patients. The average patients age was 37.7 and the relation between genders was 3.13F:1M. The average age at the beginning of MS was 29.6; 283 patients were white (94%), 15 patients were black (5%) and 4 patients were yellow; 220 patients (72%) presented relapsing-remmiting clinical form; 82 patients (28%) presented the progressive form (50% secondary form and 50% primarily progressive form). The most common initial symptoms were sensitive (31.7%) and optical (26.8%). The most common evolutive symptoms were pyramidal (72.5%) and of the spinal cord (64.9%). The average of the final EDSS was 3.37 and final NRS was 85.17. The index of the annual outbreak was 0.45. Our findings coincide to the ones in the worldwide literature, however it is important to point out that 60 patients (19.8%) presented benign MS of prognostic value.KEY WORDS: multiple sclerosis, descriptive study, evolutive forms.A esclerose múltipla (EM) é a desordem desmielinizante mais comum do sistema nervoso central (SNC), caracterizada por episódios repetidos de disfunção neurológica com remissão variável 1 .
RESUMO -Foram avaliados 95 pacientes com forma remitente-recorrente da esclerose múltipla quanto à presença de fadiga. A Escala de Severidade de Fadiga foi aplicada em todos os pacientes. Em 64 pacientes (67,4%) a fadiga foi encontrada. Não observamos diferenças clínicas quanto ao gênero, idade, grau de incapacidade funcional e depressão, nos pacientes com e sem fadiga. Foi encontrada correlação entre ansiedade e tempo de doença com a presença de fadiga. Ao analisarmos estas variáveis quanto à intensidade da fadiga, observamos haver associação entre fadiga grave e maior incapacidade funcional.PALAVRAS-CHAVE: esclerose múltipla, fadiga, EDSS, depressão, ansiedade. Fatigue in multiple sclerosis relapsing-remitting formABSTRACT -In 95 patients with the remitting-relapsing form of multiple sclerosis we investigated fatigue. All of them were evaluated with the Fatigue Severity Scale and we found it in 64 patients (67.4%). Gender, age, depression and fuctional incapacity was not predictive of fatigue occurrence, while anxiety and time of disease seems to be correlated with it. Apresenta sinais e sintomas muito variáveis, e tem a fadiga como uma de suas principais características. A definição de fadiga pode ser feita de várias maneiras, permitindo interpretações diferentes. Para os fisiologistas, é considerada como uma dificuldade em manter a contração muscular, podendo ser objetivamente analisada e quantificada. Esta definição considera apenas a fadiga muscular, tornando-se muito limitada na prática clínica. Para os pacientes com EM, a fadiga é um sintoma subjetivo, definido como sensação de cansaço físico ou mental profundo, perda de energia ou mesmo sensação de exaustão, com características diferentes daquelas observadas na depressão ou fraqueza muscular 1 . Nos pacientes com doenças neurológicas, a fadiga é diferente daquela relatada pelos demais, levando a maior comprometimento da qualidade de vida. Na EM é sintoma frequente e incapacitante que acomete de 75 a 95% dos portadores, não sendo correlacionada com idade, sexo, depressão ou grau de acometimento neurológico 2-4 . Sua mensuração usualmente tem sido relatada através de escalas de auto-avaliação, por tratar-se de sintoma subjetivo 5 . Mendes, Tilbery e Felipe 5 realizaram estudo preliminar em pacientes com EM, encontrando fadiga em 70% dos pacientes, porém sem estudar as suas principais correlações clínicas. Não encontramos no nosso meio estudos sobre este sintoma.O objetivo deste estudo é analisar na nossa população as características da fadiga nos pacientes com EM forma remitente-recorrente (RR), e os principais fatores associados.
RESUMO -Fadiga é um sintoma incapacitante, frequentemente observado nos pacientes com esclerose múltipla, porém pouco compreendido. Escalas de auto-avaliação são utilizadas para melhor estudar este sintoma subjetivo. Foram aplicadas três escalas de avaliação de fadiga em 15 pacientes com diagnóstico de esclerose múltipla, em 9 dos quais havia a referência de fadiga. Nestes os escores apresentavam-se elevados, o que significa fadiga acentuada. As escalas aplicadas demonstraram ser complementares na avaliação da fadiga, a qual se mostrou um sintoma importante nos pacientes avaliados.PALAVRAS-CHAVE: esclerose múltipla, fadiga, escalas de mensuração. Selfreport measures are designed to capture the patient's subjective sense of fatigue. We applied three scales in 15 patients with MS. Nine of them reported fatigue. The scores were high in all of these patients. We conclude that these scales must be used in assemble, to evaluate this symptom. Ela é definida como sensação de cansaço físico profundo, perda de energia ou mesmo sensação de exaustão, e é importante a sua diferenciação com depressão ou fraqueza muscular 1 . Embora seja um sintoma subjetivo e inespecífico, ela acompanha diversas doenças, podendo ocorrer também em indivíduos sadios 2 . A fadiga é sintoma clínico muito estudado, sendo bem conhecida a sua associação com fenômenos autoimunes, neoplásicos, inflamatórios e infecciosos, e pode ser influenciada por sintomas como dor, distúrbios do sono, alterações do humor e alterações cognitivas. O nosso conhecimento a seu respeito torna-se limitado devido à ausência de técnicas adequadas para a sua mensuração. Na esclerose múltipla (EM), a fadiga é um sintoma frequente, geralmente crônico e incapacitante, levando a grande impacto na vida diária.O presente estudo refere-se a dados preliminares relativos à observação de 15 pacientes com EM, através de escalas subjetivas de auto-avaliação, as quais têm por objetivo quantificar e analisar as diferentes características clínicas da fadiga, verificando sua aplicabilidade no nosso meio.
RESUMO -Os autores apresentam uma série de 214 casos de esclerose múltipla provenientes de levantamento retrospectivo de prontuários na Disciplina de Neurologia da Faculdade de Ciências Médicas da Santa Casa de São Paulo e da clínica privada, no período de 1980 a 1993. Os dados encontrados foram analisados quanto a sexo, raça, média de idade de início dos sintomas, sintomas iniciais e sintomas evolutivos da doença. Multiple sclerosis: clinical and evolutive study of 214 casesSUMMARY -The authors report on a series of 214 cases of defined multiple sclerosis according to Poser et al. criteria. These cases were retrospectively selected by medical reports analysed from Santa Casa Medical School (São Paulo) and the private practive, from 1980 to 1993. The data were analysed as for sex, race, onset age of symptoms, onset symptoms, and evolutive symptoms. The results were compared to those found in other Brazilian series and in foreign series. This comparative analysis allows to verify similarities and differences among the several series, and the authors emphasize the necessity of multicentric studies in Brazil to assert with more details the multiple sclerosis profile in our country. A maioria dos autores admite uma distribuição geográfica típica da doença, com maior incidência em países de clima frio ou temperado e menor, em países de clima tropical 8,9 . No Brasil, a EM ainda é insuficientemente conhecida. Carecemos de estudos multicêntricos, que abordem os vários aspectos da doença. Há dificuldades inerentes ao diagnóstico que, até recentemente, baseava-se exclusivamente em critérios clínicos 5 . Contribui ainda para este fato, a falta de colaboração de colegas no registro de casos 11 e a precariedade da assistência médica estatal 5 .Nosso intuito, ao publicar esta casuística, é contribuir para o conhecimento da EM em nosso País.
RESUMO -Relatamos os resultados de estudo com o interferon beta 1-a em 62 pacientes ambulatoriais com a forma remitente-recorrente da esclerose múltipla durante um ano. Os critérios de inclusão para este tratamento foram de escore do EDSS entre 0 e 5,5 e de pelo menos relato de dois surtos nos dois últimos anos. Administramos 3 milhões de unidades internacionais de interferon beta 1-a três vezes por semana. Os objetivos deste estudo foram verificar o efeito da medicação no número de surtos e avaliar a eficácia da droga na progressão da doença. O índice anual de surtos nos pacientes que não tomaram a medicação foi 1,32 e naqueles medicados 0,63. O escore do EDSS em pacientes não medicados foi 4,7 e naqueles com medicamento 2,0. O interferon beta 1-a foi bem tolerado e 85% dos pacientes completaram um ano de tratamento.PALAVRAS-CHAVE: interferon beta 1-a, esclerose múltipla, tratamento. Interferon beta 1-a in multiple sclerosis: experience of one year in 62 patientsABSTRACT -We report the results of a trial of interferon beta 1-a in 62 ambulatory patients with relapsingremitting multiple sclerosis. Entry criteria included EDSS of 0 to 5.5 and at least two exacerbations in the previous 2 years. The patients received 3 million international units by subcutaneous injections three times a week. The end points were differences in exacerbation rate and treatment effect on disease progression. The annual exacerbation rate for patients that did not take the interferon beta 1-a was 1.32 and for the patients under medication 0.63. The EDSS score in patients that did not take the mediaction was 4.7 and 2.0 for the patients with interferon beta 1-a. Admite-se que células T, ativadas por autoantígeno ainda não determinado, passam da periferia para o SNC pela barreira hematoencefálica, evento considerado essencial na patogênese da desmielinização 1,2 . Até o início da década passada, dispunhamos apenas de corticóides e imunossupressores para o tratamento de pacientes com EM entretanto, estes medicamentos apresentam resultados pouco eficazes e não evitam a progressão da doença 3,4 . Após os relatos de Jacobs e Munschauer 5 , inúmeros ensaios clínicos com interferon foram realizados até que ,em 1993, o FDA americano aprovou e autorizou seu uso em pacientes portadores da forma remitenterecorrente da EM 6 .
