Fatigue is the most frequent and severe of symptoms causing morbidity and disability in multiple sclerosis (MS) patients. However, measuring fatigue is a difficult task. There is no clear definition; it is complex and multidimensional, and is a highly subjective symptom with many uncertainties regarding its pathophysiology. No biological or neuroimaging markers for fatigue are currently known 1 . The lack of standardization for fatigue measurement has resulted in development of several assessment instruments, but none of them is universally accepted. The Fatigue Severity Scale (FSS) 2 and Modified Fatigue Impact Scale (MFIS) 3 are the most widely used instruments for measuring MS fatigue, although limitations regarding their development and measurement properties have been reported. Moreover, they were not specifically developed for MS patients 4,5,6 . Studies using the Rasch measurement model have identified a need for certain items to be removed from the FSS [4] and MFIS [6] so that their measurements can be considered valid and reliable.In Brazil, FSS 7 and MFIS 8 have been widely used, despite the failings of their design and psychometric properties in Brazilian cross-cultural adaptation studies. These Brazilian versions did not fulfill the criteria for adequate sample size and design. Moreover, the psychometric properties of the Brazilian FSS adaptation were not assessed. Therefore, there are no Brazilian MS-specific instrument for assessing fatigue with reported methodological and psychometric properties ABstrACtThe Neurological Fatigue Index for Multiple Sclerosis (NFI-MS) is a new fatigue assessment instrument. The aim of this study was to cross-culturally adapt and assess the psychometric properties of the Brazilian version of the NFI-MS (NFI-MS/BR). Method: Two hundred and forty subjects with MS were recruited for this study. The adaptation of the NFI-MS was performed by translation and back translation methodology. In psychometric analysis was performed the administration of the questionnaires Epworth Sleep Scale, Fatigue Severity Scale, Modified Fatigue Impact Scale, Multiple Sclerosis Impact Scale-29, NFI-MS/BR and Pittsburgh Sleep Quality Index with retest of the NFI-MS/BR after 7 days. Results: Reliability was assessed (intraclass correlation coefficients between 0.77 and 0.86), and validity by testing 41 hypotheses about expected correlations between subscales and confirmed 36. The majority of correlations were demonstrated. Sleep disorders in patients with multiple sclerosis have various causes and interfere with daytime wakefulness. This study assessed the correlation between fatigue, excessive daytime sleepiness and level of disability. Method Retrospective review of medical records from patients with multiple sclerosis to collect data on severity of fatigue, disability, daytime sleepiness, and depression. From 912 medical records reviewed, 122 reported daytime sleepiness: 67% had relapsing remitting, 12% had primary progressive, and 21% had secondary progressive. Results In 95% of the patients with relapsing remitting who complained of daytime sleepiness and fatigue, association was found between these symptoms and neurological disability. Patients with relapsing remitting who complained of daytime sleepiness and fatigue also experienced depression (p = 0.001). No association between fatigue, excessive daytime sleepiness, depression, and disability was found in patients with progressive disease. Conclusion In relapsing remitting, there is correlation between functional disability, excessive daytime sleepiness and fatigue, a finding not confirmed in primary progressive and secondary progressive form.

Resumo: Objetivo: O estudo visa apresentar as etapas desenvolvidas para o aprimoramento de manual de orientação e sua aplicação com pessoas com esclerose múltipla (EM), ocorrida em um grupo terapêutico conduzido pela Terapia Ocupacional. Conforme as políticas de saúde, a produção de materiais educativos tem caráter multidisciplinar e é útil como ferramenta para a promoção de saúde devido à sua capacidade de aumentar o conhecimento e de solucionar problemas. O caráter crônico e progressivo da EM traz alterações em diferentes funções do corpo e demanda mudanças e estratégias para minimizar o seu impacto na vida das pessoas. Manual development for basic guidelines of the day-to-day for people with multiple sclerosisAbstract: Objective: The study aims to present the steps developed for the guidance manual enhancement and its application on people with multiple sclerosis occurred in a therapeutic group conducted by the Occupational Therapy.According to health policies, the production of educational materials is multidisciplinary and is useful as a tool to promote health due to this ability to increase the knowledge and problem solutions. The chronic and progressive disease brings alterations in different body functions and demands changes and strategies to minimize their impact on people's lives. Method: Documentary study based on written records and audio produced in the group meetings in the period from 2013 to 2014. The analysis of the documents was based on the Collective Subject Discourse (CSD) method. Results: The study included 10 people with multiple sclerosis, aged 23-55 years, the majority with over 5 years of disease. It was generated four CSD. According to the speeches the manual served as a practical tool and easy consultation, which favored demystify the disease, served as a guide for the routine activities, support identifying problems and facilitating the interaction with the social network. Conclusion: The use of the manual was useful because it facilitates the practice of acquired skills in the group and encourage co-responsibility for their own health.
