-This study has tested the validity and reliability of the Portuguese version of the Confusion Assessment Method (CAM), a diagnostic assessment instrument for delirium developed by Inouye et al. (1990). The sample was formed by 100 patients with 60 and more years of age, admitted at the emergency service of Santa Casa de São Paulo, in the time periods between July and August, 1996, November and December, 1996 and February and March, 1997. The sensibility was 94.1% and specificity 96.4%. The assessors reliability in a sample of the 24 patients resulted in a kappa = 0.70. We have concluded that CAM is an adequate instrument to assess the presence of delirium, reliable to assess elderly patients at the emergency services.KEY WORDS: delirium ,elderly, emergency service, validity, CAM.Validação e confiabilidade da versão em língua portuguesa do confusion assessment method (CAM) para a detecção de delirium no idoso. Presently, delirium is defined as an organic brain syndrome without specific etiology, characterized by the simultaneous presence of disturbances related to consciousness and attention, perception, thinking, memory, psychomotor behavior, emotions and sleep-wake cycle 1 . Prevalence studies indicate that delirium is present in 10-24% of adults admitted to hospital and that another 32% will develop the disorder as inpatients 2,3 . Typical symptoms of delirium include reduced ability to focus and maintain attention, memory deficits, disorientation, language difficulties, illusions, hallucinations, and many other behavioral abnormalities. Infection, cardiovascular diseases, metabolic disturbances, and drug use or withdrawal are the most frequent causes of delirium 4,5 . Only 33-64% of the patients with delirium are correctly identified by practicing physician [6][7][8][9][10]
July 1, 1997 was stipulated as the day for estimating the prevalence of multiple sclerosis within the city of São Paulo. The patients were identified via various sources, including associated universities and magnetic resonance services of the city of São Paulo. The area covered by the study has a population of 9,380,000, mainly white and of European origin, with a large number of immigrants from Spain and Portugal. The patients were classified in accordance with the criteria of Poser et al. (1983), and only those with defined multiple sclerosis were registered. The study gave a prevalence of 15.0/10(5) inhabitants, or three times the value obtained in a similar study in 1990. This increase reveals the larger number of cases encompassed by the study, and is attributed to the use of more detailed recording methods, improvements in diagnosis, and better conditions for treatment.
Purpose: To develop and investigate the reliability and validity of a Brazilian version of the 25-Item National Eye Institute Visual Function Questionnaire. Methods: A Brazilian version of the questionnaire was developed and applied to patients with chronic eye diseases and healthy controls. Reliability was assessed by Cronbach's alpha coefficient, intraclass correlation coefficient and interrater reliability coefficient. Participants were submitted to the Medical Outcomes Study Short Form-36 Questionnaire. Validity was estimated by the correlation between the questionnaires and among the 12 subscales of the questionnaire, subscales scores and visual acuity, patients' and controls' subscales scores, and the factor analysis. Responsiveness of the questionnaire was assessed in a group of patients before and following cataract surgery. Results: Cronbach's alpha, intraclass correlation and interrater reliability coefficients were highly significant. Validity was well established in all tests. In patients who underwent cataract surgery, a better visionrelated quality of life was observed following surgery. Conclusion: The Brazilian version of the 25-Item National Eye Institute Visual Function Questionnaire has valid and reliable psychometric properties and can be applied to vision-related quality of life research. RESUMO -Recentemente foram introduzidos novos medicamentos no tratamento da esclerose múltipla (EM) capazes de modificar a história natural da doença. Nos estudos clínicos que avaliam estas drogas utiliza-se com frequência a escala de incapacidade expandida (EDSS) como instrumento para avaliar as disfunções e incapacidades neurológicas durante estes ensaios. Há inúmeras limitações no uso desta escala, predominantemente pelo fato desta ser uma escala que privilegia em demasia a capacidade de marcha dos pacientes. Neste estudo, apresentamos nossos resultados na aplicação do teste de destreza manual da caixa e blocos em indivíduos normais e em portadores de EM. Observamos que 64,8% das mulheres e 80,7% dos homens apresentaram alterações estatisticamente significantes no escore deste teste quando comparados com indivíduos normais. Por sua facilidade de aplicação e sensibilidade em detectar alterações das habilidades motoras em membros superiores, os autores recomendam o emprego deste teste na avaliação do efeito destes medicamentos em pacientes com EM.PALAVRAS-CHAVE: teste de destreza manual da caixa e blocos, esclerose múltipla, escores de incapacidade. In clinical trials with these new agents the Expanded Disability Status Scale (EDSS) is often used as a primary outcome instrument to measure neurological impairment and disability. A number of limitations have been identified when using the EDSS, some of wich are because the EDSS is an ordinal scale that is heavily biased to locomotor function. In this study we applied the box and block test of manual dexterity in normal subjects and relapsing-remitting MS patients. The results were that 64.8% of the female and 80.7% of the male patients had significant changes on this task compared with normal subjects, and as this test is easily applied and is sensitive in detecting upper extremity functional ability, we recommend its use in clinical trials to evaluate new drugs in MS patients.KEY WORDS: box and block test of manual dexterity, multiple sclerosis, disability scales. RESUMO -A esclerose múltipla é uma das causas mais comuns de incapacidade neurológica crônica em adultos jovens. Avaliamos, através de estudo retrospectivo, características epidemiológicas, formas de apresentação, manifestações clínicas, evolução e o grau de incapacitação física da esclerose múltipla em 302 pacientes. A média da idade dos pacientes foi 37,7 anos e a relação entre os gêneros foi 3,13F:1M. A média da idade no início da doença foi 29,6 anos. Duzentos e oitenta e três pacientes eram brancos (94%), 15 negros (5%) e 4 amarelos (1%). Duzentos e vinte pacientes (72%) apresentaram forma clínica tipo remitente-recorrente. Oitenta e dois pacientes (28%) apresentaram a forma progressiva (50% forma secundária e 50% forma primariamente progressiva). Os sintomas iniciais mais comuns foram sensitivos (31,7%) e ópticos (26,8%).Em relação aos sintomas evolutivos predominavam os sintomas piramidais (72,5%) e os medulares (64,9%). A média do escore do EDSS final foi 3,37 e a do NRS foi 85,17. O índice anual de surtos foi 0,45. Nossos achados coincidem com os da literatura mundial, e ressaltam que 60 pacientes (19,8%) apresentavam esclerose múltipla de forma benigna de valor prognóstico. Multiple sclerosis: descriptive study of the clinical forms in 302 casesABSTRACT -Multiple sclerosis (MS) is one of the most common causes of chronic neurologic disability in young adults. We evaluate, through retrospective study, the epidemiological characteristics, the manifestations ways, the clinical manifestations, the evolution and the level of physical disability of MS in 302 patients. The average patients age was 37.7 and the relation between genders was 3.13F:1M. The average age at the beginning of MS was 29.6; 283 patients were white (94%), 15 patients were black (5%) and 4 patients were yellow; 220 patients (72%) presented relapsing-remmiting clinical form; 82 patients (28%) presented the progressive form (50% secondary form and 50% primarily progressive form). The most common initial symptoms were sensitive (31.7%) and optical (26.8%). The most common evolutive symptoms were pyramidal (72.5%) and of the spinal cord (64.9%). The average of the final EDSS was 3.37 and final NRS was 85.17. The index of the annual outbreak was 0.45. RESUMO -Foram avaliados 95 pacientes com forma remitente-recorrente da esclerose múltipla quanto à presença de fadiga. A Escala de Severidade de Fadiga foi aplicada em todos os pacientes. Em 64 pacientes (67,4%) a fadiga foi encontrada. Não observamos diferenças clínicas quanto ao gênero, idade, grau de incapacidade funcional e depressão, nos pacientes com e sem fadiga. Foi encontrada correlação entre ansiedade e tempo de doença com a presença de fadiga. Ao analisarmos estas variáveis quanto à intensidade da fadiga, observamos haver associação entre fadiga grave e maior incapacidade funcional.PALAVRAS-CHAVE: esclerose múltipla, fadiga, EDSS, depressão, ansiedade. Fatigue in multiple sclerosis relapsing-remitting formABSTRACT -In 95 patients with the remitting-relapsing form of multiple sclerosis we investigated fatigue. All of them were evaluated with the Fatigue Severity Scale and we found it in 64 patients (67.4%). Gender, age, depression and fuctional incapacity was not predictive of fatigue occurrence, while anxiety and time of disease seems to be correlated with it. Apresenta sinais e sintomas muito variáveis, e tem a fadiga como uma de suas principais características. A definição de fadiga pode ser feita de várias maneiras, permitindo interpretações diferentes. Para os fisiologistas, é considerada como uma dificuldade em manter a contração muscular, podendo ser objetivamente analisada e quantificada. Esta definição considera apenas a fadiga muscular, tornando-se muito limitada na prática clínica. Para os pacientes com EM, a fadiga é um sintoma subjetivo, definido como sensação de cansaço físico ou mental profundo, perda de energia ou mesmo sensação de exaustão, com características diferentes daquelas observadas na depressão ou fraqueza muscular 1 . Nos pacientes com doenças neurológicas, a fadiga é diferente daquela relatada pelos demais, levando a maior comprometimento da qualidade de vida. Na EM é sintoma frequente e incapacitante que acomete de 75 a 95% dos portadores, não sendo correlacionada com idade, sexo, depressão ou grau de acometimento neurológico 2-4 . Sua mensuração usualmente tem sido relatada através de escalas de auto-avaliação, por tratar-se de sintoma subjetivo 5 . Mendes, Tilbery e Felipe 5 realizaram estudo preliminar em pacientes com EM, encontrando fadiga em 70% dos pacientes, porém sem estudar as suas principais correlações clínicas. Não encontramos no nosso meio estudos sobre este sintoma.O objetivo deste estudo é analisar na nossa população as características da fadiga nos pacientes com EM forma remitente-recorrente (RR), e os principais fatores associados.
Treatment with music has shown effectiveness in the treatment of general behavioural and cognitive symptoms of patients with various types of dementia.Objective:To assess the effectiveness of treatment with music on the memory of patients with Alzheimer’s disease (AD).Methods:A systematic search was performed on PubMed (Medline), Cochrane Library, PsycINFO and Lilacs databases up to June 2017 and included all randomized controlled trials that assessed memory using musical interventions in patients with AD.Results:Forty-two studies were identified, and 24 studies were selected. After applying the exclusion criteria, four studies involving 179 patients were included. These studies showed the benefits of using music to treat memory deficit in patients with AD.Conclusion:To the best of our knowledge, this is the first systematic review focusing on randomized trials found in the literature that analysed the role of musical interventions specifically in the memory of patients with AD. Despite the positive outcome of this review, the available evidence remains inconsistent due to the small number of randomized controlled trials.
The clinical improvement of active MS following the treatment with methylprednisolone was associated with the modification of CSF levels of CCL2 and CXCL10, suggesting that these chemokines may be useful markers of response to treatment and relapses in MS patients.
