Пирогова (гл. врач-к.м.н. А.В. Свет), Москва, россия Одним из основных принципов лечения варикозной болезни нижних конечностей служит устранение варикозной трансформации подкожных вен. Осуществить это можно при помощи инвазивных методов, которые разделяют на две большие группы-флебосклерозирующие и оперативные. До определенного момента в арсенале хирургов были представлены только различные варианты удаления и перевязки вен-кроссэктомия, стриппинг, флебэктомия (удаление притоков), диссекция перфорантных вен. В дальнейшем были разработаны альтернативные стриппингу способы, основанные на термическом воздействии на магистральные подкожные вены-эндовенозная лазерная и радиочастотная облитерация. В течение последующих 10-15 лет эндовенозная термооблитерация стала методом выбора при планировании хирургического лечения. Эти методики в сочетании с минифлебэктомией либо склеротерапией в настоящее время являются «золотым стандартом» лечения варикозной болезни нижних конечностей. Стандарт подразумевает обязательную ликвидацию рефлюкса крови либо путем удаления вен, либо их окклюзией. В последние годы появились публикации о методах хирургического лечения варикозной болезни, основанных на ином стратегическом подходе. Он заключается в намеренном сохранении несостоятельных магистральных подкожных стволов и в некоторых случаях даже расширенных притоков при выполнении хирургического вмешательства. В основе этих операций лежит отношение к рефлюксу не как к причине заболевания, а как к гемодинамическому феномену, который может исчезать при определенных обстоятельствах и не является патологическим. Такая хирургия получила название гемодинамической либо веносохраняющей. В настоящее время она представлена двумя различными методиками ChiVA и ASVAL. В настоящем обзоре подробно рассматриваются достоинства и недостатки обоих стратегических подходов.

Пирогова Миниздрава России, Москва, Россия Цель исследования-определить факторы, повышающие вероятность сохранения рефлюкса в стволе большой подкожной вены (БПВ) и рецидива варикозной болезни после изолированной минифлебэктомии. Материал и методы. Изолированную минифлебэктомию с сохранением несостоятельного ствола БПВ выполнили на 75 нижних конечностях у 67 пациентов. Через 12 мес после операции регистрировали рефлюкс в магистральной вене и частоту рецидива варикозного расширения вен. Оценку независимых факторов риска рецидива/сохранения рефлюкса в БПВ и возврата заболевания провели методом логистической регрессии. Результаты. Рефлюкс по стволу БПВ через 12 мес после изолированного удаления варикозно-расширенных притоков зарегистрировали в 34% наблюдений. Предиктором негативного исхода стала протяженность рефлюкса-шанс сохранения рефлюкса через год в среднем оказался почти в 13 раз выше у пациентов с исходным рефлюксом до уровня ниже середины бедра. Рецидив варикозной болезни развился в 13,5% случаев. Значимым фактором риска рецидива оказалось наличие рефлюкса в БПВ через год после вмешательства. Шансы рецидива возрастают в 264 раза. При локализации поражения на левой нижней конечности шансы рецидива возрастают в 73 раза по сравнению с поражением правой нижней конечности. Заключение. Изолированная минифлебэктомия оптимальна для пациентов с несостоятельностью ствола БПВ до уровня не ниже середины бедра с локализацией поражения на правой нижней конечности. Шансы на достижение гемодинамического результата снижаются с возрастом.
