Objectives: To compare the interobserver agreement between visual and photogrammetry postural assessment and to determine whether the quantitative photogrammetry results correspond to the symmetries and asymmetries detected through qualitative visual postural assessment. Methods: Twenty-one volunteers (mean age 24±1.9 years) were visually evaluated by three experienced physical therapists, who completed a postural assessment form. The participants' face and whole body were then photographed in the anterior and posterior frontal and sagittal planes. The photographs were used to draw angles from markers fixed to the skin at various anatomical points that are frequent references in traditional postural assessment. These photographs were analyzed by three examiners (other than the ones who performed the visual assessment). The agreement in each postural assessment method was determined using Cramer's V or the Phi coefficient, with the significance level set at 5%. Results: There was agreement between the examiners who used photogrammetry, for all segments analyzed. No agreement was found for the labial commissure (p=0.00), acromioclavicular joint (p=0.01), sternoclavicular joint (p=0.00), anterior and posterior iliac spines (p=0.00 and p=0.01) or inferior angle of the scapula (p=0.00) when assessed visually. The comparison between photogrammetry and visual postural assessment showed that the agreement level between the two assessment methods was poor for some segments of the lower limb and pelvis. Conclusions: Under these experimental conditions, the photogrammetry data were not correlated with the results from the visual postural assessment. The visual postural assessment produced data that were in less agreement than the photogrammetry data, and its use as a gold standard must be questioned.Key words: photogrammetry; posture; assessment; validity; physical therapy.
ResumoObjetivos: Comparar a concordância interobservador da avaliação postural visual e por fotogrametria e verificar se os resultados quantitativos da fotogrametria correspondem à detecção de simetrias e assimetrias pela avaliação postural visual qualitativa. Métodos: Vinte e um voluntários (24±1,9 anos) foram inicialmente avaliados visualmente por três fisioterapeutas experientes que preencheram um protocolo de avaliação postural. Em seguida tiveram fotografados a face e o corpo todo nos planos frontal anterior, posterior e sagital. As fotos foram utilizadas para traçar ângulos a partir de marcadores fixados à pele, em vários pontos anatômicos, que são referências frequentes na avaliação postural tradicional. Essas fotografias foram analisadas por três examinadores diferentes da avaliação postural visual. Na literatura especializada, encontram-se variados instrumentos para avaliação da disfunção temporomandibular (DTM), sob a forma de índices, questionários, protocolos, escalas de avaliação e critérios de diagnóstico. Este estudo, dividido em duas partes, visou caracterizar os principais instrumentos de avaliação da DTM disponíveis na literatura, para auxiliar o clínico e o pesquisador na correta escolha da ferramenta apropriada para contemplar seus objetivos clínicos ou científicos. Nesta parte I são apresentados dois índices clínicos e três questionários (anamnésicos e funcionais); na parte II, um questionário funcional e dois conjuntos de critérios diagnósticos. Os índices são ferramentas que organizam a avaliação de sinais e sintomas, pela obtenção de pontuações. Os questionários são melhor aplicados para traçar perfis populacionais em estudos epidemiológicos. Para avaliação dos eventuais impactos da DTM nas atividades de vida diária, os questionários funcionais são mais adequados. Finalmente, há poucos conjuntos de critérios sistematizados para obter o diagnóstico da disfunção. A utilização de uma ou outra ferramenta depende de sua aplicabilidade e dos objetivos do profissional que a irá utilizar.
There is evidence of balance control impairments in subjects with all subtypes of migraine compared to control subjects. The presence of aura and frequent migraine attacks reflect negatively in the postural control performance and may have a significant clinical impact in patients with migraine that should be addressed with appropriate clinical interventions.
Objective To assess the presence and handicap due to vestibular symptoms in three subgroups of patients with migraine and controls. Methods Women between 18-55 years old were diagnosed by headache specialists and stratified as migraine with aura (n = 60), migraine without aura (n = 60), chronic migraine (n = 60) and controls (n = 60). Information regarding demographics, headache and vestibular symptoms were collected in this cross-sectional study. The self-perceived handicap related to vestibular symptoms was assessed through the Dizziness Handicap Inventory questionnaire. Results A total of 85% of women with migraine with aura and chronic migraine had vestibular symptoms contrasted to 70% of the migraine without aura group ( p < 0.05), and 12% of the control group reported symptoms ( p < 0.0001). Patients with migraine exhibited greater Dizziness Handicap Inventory scores than controls ( p < 0.001); and migraine with aura and chronic migraine groups reached greater scores than migraine without aura ( p < 0.01). Presence of migraine is associated with a greater risk of vestibular symptoms (migraine without aura: 5.20, migraine with aura: 6.60, chronic migraine:6.20, p < 0.0003) and with a greater risk of moderate-to-severe handicap (migraine without aura: 20.0, migraine with aura: 40.0, chronic migraine: 40.0, p < 0.0003). The presence of aura and greater migraine frequency adds to the risk of any handicap (migraine with aura: 1.9, chronic migraine: 1.7, p < 0.04) and to the risk of moderate-to-severe handicap (migraine with aura: 2.0, chronic migraine: 2.0, p < 0.0003). Migraine aura, intensity and frequency predict 36% of the dizziness handicap. Conclusion The prevalence of vestibular symptoms is increased in migraine during and between headache attacks, particularly in migraine with aura and chronic migraine along with an increased handicap due to those symptoms. Vestibular symptoms among subgroups of migraine should be considered when evaluating the functional impact of migraine.
