The aim of this study was to characterize the role of local RAS (renin–angiotensin system) in the inflammatory response of normal (N) and diabetic (D) mice with periodontal disease (PD). Diabetes Mellitus (DM) was induced by peritoneal injection of streptozotocin in Balb/c mice. PD was induced by ligature around the first molar in both N and D, irrespective of whether they were treated with aliskiren (50 mg/kg, Alisk). Mandibles were harvested for histomorphometric analyses, and gingival tissue (GT) was collected to evaluate gene expression and extracellular matrix components (ECM). Immunohistochemical (IHC) analyses were used to localize RAS in GT. The production of C-reactive protein (CRP), IL-1β, CXCL2, and CCL8 was evaluated by enzyme-linked immunosorbent assay (ELISA). Renin was found to exacerbate the inflammation and periodontal bone loss at 14 days after PD, and Alisk inhibited this process in GT of N and D. PD increased CRP, CXCL2, CCL8, and IL-1β production in both animals. Alisk could inhibit CRP, CXCL2, and CCL8 primarily in D animals. However, only CCL8 was decreased in N animals after Alisk pretreatment. PD enhanced expression and production of AGT, ACE, AT1R, and AT2R in both N and D. AT1R expression was higher in D with PD, and AT2R expression was higher in N with PD. ACE2 and receptor Mas (MasR) expression and production was elevated in the control group of both animals. PD inhibited ACE2 in N but not in D. MasR expression was unaffected in both N and D with PD. Alisk reduced expression and production of all RAS components in GT of both animals, except for ACE2 in N. RAS staining was observed in all layers of epithelium, basal cell layer, and lamina propria and was higher in N with PD. Col1a1, Col1a2, Col3a1, and fibronectin (Fn1) were increased in both animals with PD. Alisk inhibited Col1a1 and Fn in both animals, Col1a2 was decreased only in D, while levels of Col3a1 remained unchanged in all animal groups. In conclusion, these data demonstrated the presence and functional role of local RAS in GT, exacerbating the inflammatory response, periodontal bone loss, and wound healing processes in both N and D animal groups. In addition, Alisk was able to significantly reduce gingival inflammation, excessive wound healing processes, and periodontal bone loss.

O objetivo do trabalho foi analisar a associação dos distúrbios do sono e doenças crônicas não transmissíveis em pacientes com idade superior a 50 anos atendidos por Unidades Básicas ABSTRACTMany factors are associated with sleep disorders causing an increase in health spending. The objective of this study was to analyze an association of sleep disorders and chronic diseases non-transmissible in patients older than 50 years attended by Basic Health Care Units of Presidente Prudente. Participated 363 patients. The history of diseases was self-reported through the use a questionnaire, the level of physical activity was evaluated by the Baecke questionnaire, and sleep quality was assessed using the Mini-Sleep Questionnaire. Anthropometric and body composition data were collected using stadiometer and scale, patients within the 75th percentile were considered obese. The statistical analysis was made by Student t test, chisquare, and binary logistic regression. The results showed that sleep disorders is associated with osteoporosis, arthritis / osteoarthritis, low back pain, depression and obesity. Additionally, obesity and physical inactivity influences the occurrence of chronic diseases non-transmissible. Keywords: Sleep disturbance. Chronic diseases. Brazilian national health system. IntroduçãoSono é um evento de grande complexidade que interfere diretamente no estado clínico do indivíduo 1 . Benefícios do sono são decorrentes de processos neurobiológicos 2 , como manutenção da saúde física e mental, que podem modificar a temperatura corporal, trabalho cardíaco e produção hormonal, levando ao estado restaurador essencial e ao funcionamento adequado do organismo 3 . Fernandes 4 em estudo de revisão, relata que as fases do sono levam em consideração os movimentos não rápidos dos olhos (NREM) e os movimentos rápidos dos olhos (REM), observados no traçado do eletrocardiograma, determinando assim a arquitetura do sono. O ciclo do sono é formado por cinco estágios com duração de 90 minutos cada um e é repetido de quatro a cinco vezes por noite. Cada fase apresenta estágios fisiológicos específicos.
resumo Esse texto se constitui de reflexões teóricas e vivenciais das autoras acerca da articu-lação entre pesquisa, ensino e serviços de saúde para a consolidação do Sistema Úni-co de Saúde. As pesquisas de enfermagem na saúde coletiva possuem como desafio romper com as formas tradicionais em que se separa quem pesquisa, aprende, ensi-na e cuida. É da produção investigativa e reflexiva que se processa o movimento de transformação do processo de cuidar e de ensinar. Assim, as pesquisas para a consoli-dação do Sistema Único de Saúde clamam por uma intensa articulação entre o ensi-no-serviço e a produção de conhecimento. To compare the profile of adults attended at primary care level of the Brazilian National Health System according to occurrence of T2DM during 18 months of follow-up. Method: Longitudinal study carried out with 316 adults. T2DM and PA were assessed by questionnaires. Measurements of weight, height and waist circumference (WC) were also performed. Healthcare expenditures were based on the demand of services registered in medical records. Analysis of variance (ANOVA) with Tukey's post hoc test was used, as well as their similar for non-parametric variables. Spearman's correlation coefficient analyzed the relationship between variables. All analyzes were performed using the statistical software BioEstat (release 5.0) and the significance level was set at p-value < 0.05. Results: Patients with T2DM at baseline showed higher values of WC, BMI, nursing appointments, customer services, medication, total cost and lower values of PA when compared to patients T2DM-free. Patients who developed T2DM during the follow-up presented higher values of WC, screening, pharmacy frequency, medications and total cost when compared to individuals without T2DM. Being in the highest quartile of PA did not change the costs of health services among patients T2DM-free. Healthcare expenditures were positively correlated to BMI among participants T2DM-free and with T2DM at baseline. Conclusion: Participants with T2DM and participants who developed T2DM during the follow-up showed higher values of BMI, WC, healthcare expenditures and were less active when compared to participants T2DM-free. Healthcare expenditures were positively correlated with BMI among participants with T2DM at baseline and T2DM-free.
A universidade tem o papel de promotora de ciência e tecnologia (C&T) no sistema de inovação (SI), além de enfrentar pressões por maior inclusão social e maior diálogo com o setor produtivo. O estudo tem como objetivo investigar a produção de C&T dos inventores da UTFPR e verificar a presença da ecoinovação nas suas patentes de invenção, para compreender sua contribuição no SI. Esta pesquisa se caracteriza por ser qualitativa, exploratória e descritiva a partir de um estudo de caso. Os resultados mostram que a maioria dos professores inventores investigados na UTFPR é engenheiro e possui publicações com citações nas bases de dados Scopus e Web of Science. Parte significativa de suas invenções é desenvolvida com alunos da graduação e pós-graduação e menos de 20% delas são desenvolvidas inter institucionalmente, com destaque para as instituições de ensino. Por fim, cerca de 30% das invenções analisadas foram consideradas ecoinovações, predominando as inovações do tipo complementares.
A privação do sono está relacionada ao desenvolvimento de doenças cardiometabólicas. O objetivo deste estudo foi analisar a associação entre qualidade do sono e ocorrência de doenças cardiometabólicas em adultos atendidos na Atenção Primária à Saúde. Estudo longitudinal composto por 169 adultos (68,6% do sexo feminino), média de idade 61,80 (desvio padrão = 8,80), acompanhados por 30 meses. A presença das doenças hipertensão arterial, dislipidemia, diabetes mellitus e ocorrência de infarto e a qualidade do sono foram mensuradas por meio de questionários. Os participantes foram divididos em grupos de acordo com a qualidade do sono (adequada/inadequada). Para verificar a diferença de idade, classe econômica, atividade física habitual e circunferência de cintura, entre os grupos a e b, utilizou-se o teste t de Student para amostras independentes. Para comparar a proporção das doenças (hipertensão arterial, infarto, diabetes e dislipidemias) entre os grupos, utilizou-se o teste Qui-quadrado com correção de Yates. A regressão logística binária foi utilizada para testar a magnitude de tais associações, ajustadas por sexo, idade, atividade física, obesidade abdominal, etilismo e tabagismo. As análises estatísticas foram realizadas no software BioEstat (versão 5.0) e o nível de significância adotado foi de p < 0,05. Verificou-se que 83,4% dos adultos reportaram ao menos uma alteração na qualidade do sono e os que apresentaram inadequada qualidade do sono tiveram 3,4 vezes mais chances de reportar diabetes (OR = 3,47; IC95%: 1,06 - 11,30), comparados aos com o sono adequado. Inadequada qualidade do sono parece contribuir para o aumento da ocorrência de diabetes mellitus, independente do tempo de prática de atividade física e a obesidade abdominal.
The study aimed to conduct a cost-utility analysis of traditional drug therapy (TDT) provided for hypertensive patients at primary care in comparison to the protocol based on combination with an exercise program (TDT+E) in real-life conditions, adopting a health system perspective. Methods: Longitudinal study based on enrollment of 49 hypertensive adults distributed into two groups, for 12 months. Quality-adjusted life years were estimated using health-related quality of life. Direct health care costs were calculated including inputs and human resources in primary care from medical records. Sensitivity analysis was performed based on multivariate and probabilistic scenarios. Results: Incremental cost-effectiveness ratios of TDT+E in comparison to TDT were +79.69. Sensitivity analysis showed that TDT+E presented advantages considering uncertainties. Conclusion: Our findings show that exercise programs may improve quality of life and life expectancy among hypertensive patients.
