Objective: To analyze the association between cognitive impairment and physical frailty in older adults in secondary health care. Method: This is a cross-sectional study carried out with people aged ≥ 60 years, assisted at a geriatric and gerontology outpatient clinic. For cognitive screening, the Mini Mental State Examination, the semantic verbal fluency test, and frailty assessment using the physical frailty phenotype were used. The likelihood ratio test was applied to the predictive model. Results: 407 older adults participated in the study. Cognitive impairment was observed in 58.5% (n=238) of the sample, being higher in frail (n=66; 75%). A change in the semantic verbal fluency test was identified in 22% (n=90), with a higher prevalence in pre-frail patients (55.5%; n=226). It was identified 2.5 times more chance of a frail older person, when compared to a non-frail one, to have cognitive impairment (95% CI, +0.947 - 0.322). The chance for alteration in the semantic verbal fluency test was 5.4 times higher in frail compared to non-frail ones (95% CI, 1.68 - 0.38). Conclusion: A relationship was observed between cognitive impairment and physical frailty. Screening for frailty in geriatric nursing practice and the implementation of specific care is recommended.
Objetivou-se analisar a aplicação dos princípios do Prehospital Trauma Life Support pelas equipes do serviço pré-hospitalar móvel. Estudo observacional com 100 processos de atendimento às vítimas de trauma durante a admissão em um pronto socorro, em abril de 2014. Houve prevalência de vítimas de trauma entre 12 a 39 anos (64%) e predomínio do sexo masculino (69%). As equipes aplicaram os princípios do Prehospital Trauma Life Support na maioria das vítimas observadas, no entanto, identificou-se que 17% não estavam com a coluna cervical e vertebral imobilizadas, o corpo permaneceu imobilizado com cintos de fixação em apenas 31% dos casos, lesões não estavam protegidas em 15% das vítimas e 63% não foram despidas à procura de lesões. Ressalta-se que a aplicação dos princípios pelas equipes do serviço pré-hospitalar é primordial para padronizar ações, por meio de uma sequência lógica de atendimento universal. Sugere-se a capacitação continuada para essas equipes com base no Prehospital Trauma Life Support.
O objetivo do estudo foi realizar uma revisão integrativa em bases de dados para verificar a produção científica (inter) nacional sobre instabilidade postural e a condição de fragilidade física em idosos. Trata-se de uma revisão integrativa realizada nas bases de dados LILACS, MEDLINE, SciELO e PubMed, no período de 2001 a 2016.Incluíram-se artigos originais diretamente relacionados ao tema, disponíveis na íntegra, nos idiomas português e inglês. A busca inicial resultou em 483 artigos e, após critérios pré-estabelecidos de inclusão e exclusão, 24 artigos perfizeram o corpus da revisão. As publicações apresentaram crescimento recente com destaque para a literatura internacional. Emergiram três categorias temáticas: instabilidade postural associada ao envelhecimento: causas e consequências; reconhecimento da fragilidade física e instabilidade postural: relações e associações; e intervenções para a reabilitação do equilíbrio postural. A instabilidade postural associa-se ao processo de envelhecimento, como um marcador de fragilidade física e da saúde da pessoa idosa. Os resultados oferecem subsídios para fortalecer a gestão de cuidados preventivos da progressão da síndrome da fragilidade.
RESUMO Objetivo analisar a relação entre insuficiência familiar e a condição e os marcadores de fragilidade física de idosos acompanhados em ambulatório de Geriatria e Gerontologia. Método estudo quantitativo e transversal realizado com 384 idosos (≥ 60 anos) selecionados por critérios pré-estabelecidos de inclusão e exclusão. Avaliaram-se a fragilidade física segundo o fenótipo da fragilidade e a insuficiência familiar pelo APGAR de Família. Analisaram-se os dados mediante a estatística descritiva e a análise univariada mediante o teste de qui-quadrado com nível de significância estatístico de p≤0,05. Resultados não houve associação entre insuficiência familiar e fragilidade física (p=0,344), entretanto, observou-se percentual de idosos frágeis com elevada Disfunção Familiar (22,2%) e moderada Disfunção Familiar (19,4%), maior que o observado entre os idosos com boa funcionalidade familiar (12,2%). Entre os idosos frágeis para o marcador “fadiga/exaustão”, houve proporcionalidade direta ao grau de Disfunção Familiar e relação estatisticamente significativa ao escore total do APGAR de Família (p=0,001). Conclusão e implicações para a prática a insuficiência familiar no idoso está relacionada a outros fatores intrafamiliares e não exclusivamente à fragilidade física, no entanto, pode-se afirmar que o grau de fragilidade física entre os idosos é diretamente proporcional ao nível de Disfunção Familiar.
Objective to analyze the relationship between family insufficiency and the condition and the markers of physical frailty of elderly people followed up in a Geriatrics and Gerontology outpatient clinic. Method a quantitative and cross-sectional study conducted with 384 elderly (≥ 60 years) selected by pre-established inclusion and exclusion criteria. Physical frailty was assessed according to the frailty phenotype and family insufficiency by the Family APGAR. Data was analyzed using descriptive statistics and univariate analysis using the chi-square test with a statistical significance level of p≤0.05. Results there was no association between family insufficiency and physical frailty (p=0.344), however, it was observed a percentage of frail elderlies with high Family Dysfunction (22.2%) and moderate Family Dysfunction (19.4%), higher than that observed among the elderly with good family functioning (12.2%). Among the frail elderly for the marker “fatigue/exhaustion”, there was a direct proportionality to the degree of Family Dysfunction and a statistically significant relation to the total score of the Family APGAR (p=0.001). Conclusion and implications for the practice family frailty in the elderly is related to other intra-family factors and not exclusively to physical frailty, however, it can be stated that the degree of physical frailty among the elderly is directly proportional to the level of Family Dysfunction.
Objetivo: analisar a associação dos marcadores e da condição de fragilidade física à incontinênciaurinária em assistência ambulatorial de geriatria e gerontologia.Método: estudo transversal, desenvolvido na atenção secundária à saúde de ambulatório doParaná, com 384 idosos. Coletaram-se dados entre setembro de 2016 a março de 2017 mediantefenótipo de fragilidade e questionário International Consultation on Incontinence Questionnaire -Short Form.Resultados: dos idosos 118 (30,7%) foram considerados não frágeis, 212 (55,2%) pré-frágeis, 54(14,1%) frágeis, 106 (27,6%) com incontinência urinária, 50 (47,2%) com impacto muito gravena rotina diária, 18 (17,0%) grave, 16 (15,0%) moderado, 11 (10,4%) leve a nenhum impacto.Associaram-se à incontinência urinária a condição de fragilidade (p=0,011), os marcadores forçade preensão manual diminuída (p=0,027), fadiga e exaustão (p=0,002) e velocidade da marchareduzida (p=0,000).Conclusão: os resultados contribuem com o desenvolvimento crítico da enfermagem no momentode avaliar as necessidades de cuidado gerontológico.
Objective: to investigate the association between the condition of physical pre-frailty in elderly people and the final results of the driver license test for motor vehicles. Methods: cross-sectional study carried out with 347 elderly who underwent driver license tests. A questionnaire and tests were applied, and information on the result of the driver license test was collected. A descriptive statistical analysis and a non-parametric test were performed. Results: 163 (47.0%) were found to be pre-frail, 71 (43.6%) had reduced hand grip strength, 65 (39.9%) had reduced level of physical activity and 62 (38.0%), reduced gait speed. The result of the driver test indicated 115 (70.6%) pre-frail elderly to be able to drive with restriction. Pre-frailty was not associated with the results of the driver license test (p=0.744). Conclusion: although the pre-frail condition was frequent, there was no significant association with the result of the driver test.
