Objective:To investigate the socioeconomic and clinical factors that contribute to the functional independence of the oldest old of a community. Method: Cross-sectional quantitative study whose sample consisted of 214 elderly people registered in Basic Health Units. Data were collected through structured interviews and application of the Functional Independence Measure. We used descriptive statistics, and for association of the variables we used the Student t-test, ANOVA and Tukey's test for multiple comparisons. Results: The significant variables that contributed to the functional independence were remaining economically active, practicing physical and leisure activities, having a social life, eating fruits, vegetables and meat. The orientation to conduct these practices reduces the demand for care and help needed in everyday activities. Conclusion: Maintaining independence is primordial to delay disability and presents itself as an excellent field of work for nursing.
Objective: To analyze the association between cognitive impairment and physical frailty in older adults in secondary health care. Method: This is a cross-sectional study carried out with people aged ≥ 60 years, assisted at a geriatric and gerontology outpatient clinic. For cognitive screening, the Mini Mental State Examination, the semantic verbal fluency test, and frailty assessment using the physical frailty phenotype were used. The likelihood ratio test was applied to the predictive model. Results: 407 older adults participated in the study. Cognitive impairment was observed in 58.5% (n=238) of the sample, being higher in frail (n=66; 75%). A change in the semantic verbal fluency test was identified in 22% (n=90), with a higher prevalence in pre-frail patients (55.5%; n=226). It was identified 2.5 times more chance of a frail older person, when compared to a non-frail one, to have cognitive impairment (95% CI, +0.947 - 0.322). The chance for alteration in the semantic verbal fluency test was 5.4 times higher in frail compared to non-frail ones (95% CI, 1.68 - 0.38). Conclusion: A relationship was observed between cognitive impairment and physical frailty. Screening for frailty in geriatric nursing practice and the implementation of specific care is recommended.
Objetivo: analisar a associação dos marcadores e da condição de fragilidade física à incontinênciaurinária em assistência ambulatorial de geriatria e gerontologia.Método: estudo transversal, desenvolvido na atenção secundária à saúde de ambulatório doParaná, com 384 idosos. Coletaram-se dados entre setembro de 2016 a março de 2017 mediantefenótipo de fragilidade e questionário International Consultation on Incontinence Questionnaire -Short Form.Resultados: dos idosos 118 (30,7%) foram considerados não frágeis, 212 (55,2%) pré-frágeis, 54(14,1%) frágeis, 106 (27,6%) com incontinência urinária, 50 (47,2%) com impacto muito gravena rotina diária, 18 (17,0%) grave, 16 (15,0%) moderado, 11 (10,4%) leve a nenhum impacto.Associaram-se à incontinência urinária a condição de fragilidade (p=0,011), os marcadores forçade preensão manual diminuída (p=0,027), fadiga e exaustão (p=0,002) e velocidade da marchareduzida (p=0,000).Conclusão: os resultados contribuem com o desenvolvimento crítico da enfermagem no momentode avaliar as necessidades de cuidado gerontológico.
