Background and Purpose: Oropharyngeal dysphagia is a common manifestation in acute stroke. Aspiration resulting from difficulties in swallowing is a symptom that should be considered due to the frequent occurrence of aspiration pneumonia that could influence the patient’s recovery as it causes clinical complications and could even lead to the patient’s death. The early clinical evaluation of swallowing disorders can help define approaches and avoid oral feeding, which may be detrimental to the patient. This study aimed to create an algorithm to identify patients at risk of developing dysphagia following acute ischemic stroke in order to be able to decide on the safest way of feeding and minimize the complications of stroke using the National Institutes of Health Stroke Scale (NHISS). Methods: Clinical assessment of swallowing was performed in 50 patients admitted to the emergency unit of the University Hospital, Faculty of Medicine of Ribeirão Preto, São Paulo, Brazil, with a diagnosis of ischemic stroke, within 48 h after the beginning of symptoms. Patients, 25 females and 25 males with a mean age of 64.90 years (range 26–91 years), were evaluated consecutively. An anamnesis was taken before the patient’s participation in the study in order to exclude a prior history of deglutition difficulties. For the functional assessment of swallowing, three food consistencies were used, i.e. pasty, liquid and solid. After clinical evaluation, we concluded whether there was dysphagia. For statistical analysis we used the Fisher exact test, verifying the association between the variables. To assess whether the NIHSS score characterizes a risk factor for dysphagia, a receiver operational characteristics curve was constructed to obtain characteristics for sensitivity and specificity. Results: Dysphagia was present in 32% of the patients. The clinical evaluation is a reliable method of detection of swallowing difficulties. However, the predictors of risk for the swallowing function must be balanced, and the level of consciousness and the presence of preexisting comorbidities should be considered. Gender, age and cerebral hemisphere involved were not significantly associated with the presence of dysphagia. NIHSS, Glasgow Coma Scale, and speech and language changes had a statistically significant predictive value for the presence of dysphagia. Conclusions: The NIHSS is highly sensitive (88%) and specific (85%) in detecting dysphagia; a score of 12 may be considered as the cutoff value. The creation of an algorithm to detect dysphagia in acute ischemic stroke appears to be useful in selecting the optimal feeding route while awaiting a specialized evaluation.
Speech therapy in hospital bed in post-stroke hospitalized patients with dysphagia seems to bring satisfactory results in the short-term, revealing the importance of diagnosis and early intervention in these cases.
RESUMO:O presente estudo tem por objetivo comparar e correlacionar os resultados das avaliações formativa e somativa nas sessões tutoriais e habilidades práticas de um curso de graduação em Fonoaudiologia de uma Universidade pública brasileira. A amostra foi constituída por resultados de avaliações somativas e formativas de sessões tutoriais e habilidades práticas de 42 discentes do II Ciclo do curso de Fonoaudiologia de uma Universidade pública, que utiliza como método de ensino-aprendizagem a metodologia ativa, com a abordagem da aprendizagem baseada em problemas. Foram aceitos apenas os resultados de avaliações de discentes com assiduidade mínima de 75%. Os dados foram tabulados e submetidos a tratamento estatístico com p-valor ≤ 0,05. Foi observado correlação positiva entre as avaliações formativa e somativa, tanto em sessões tutoriais quanto em habilidades práticas. Foram identificados maiores escores para a avaliação formativa, com presença de diferenças significativas (p<0,01). Conclui-se que houve correlação positiva entre as avaliações formativas e somativas em sessões tutoriais e de habilidades, com melhores resultados para a formativa (tanto em sessões tutoriais quanto em habilidades). RESUMO: Objetivo: descrever o processo de elaboração de uma Situação-Problema e de um Módulo de aprendizagem e refletir a importância destes na formação do Fonoaudiólogo. Métodos: estudo descritivo do processo de elaboração de uma Situação-Problema e de um Módulo de aprendizagem de um curso de graduação em Fonoaudiologia - uma experiência precursora na área em que toda a estrutura curricular tem como base Metodologias Ativas de ensino. Resultado: o Módulo Estudo da Linguagem Infantil ocorre no Ciclo II do Curso e é constituído por nove Situações-Problema sendo, cada uma, discutida em duas sessões tutoriais com até dez discentes e conduzida por um professor tutor. As Situações-Problema, assim como o Módulo, são elaboradas por uma comissão de docentes doutores e especialistas em Linguagem de acordo com o Projeto Pedagógico do Curso. Discute-se, neste artigo, a terceira Situação-Problema que tem como título - "Mesmas características, mas diagnósticos diferentes... Por quê?"; O título tem o papel de despertar a curiosidade do discente e apresentar o enfoque principal do problema. Todas as Situações-Problema são avaliadas por docentes e discentes com objetivo de promover aperfeiçoamentos constantes. Conclusão: o contexto problematizador construído a partir das Situações-Problema permite ao discente a busca de respostas diariamente, de forma ativa e autônoma, pela vivência e discussão do trabalho em equipe, dos aspectos biopsicossociais do indivíduo e das questões éticas na atuação profissional. Possibilitando assim a formação de profissionais resolutivos, competentes para aperfeiçoar seu conhecimento constantemente e de atuar em consonância com as práticas de Saúde Pública do País.
RESUMO A pandemia da COVID-19 é associada com o novo tipo do coronavírus, o SARS-Cov-2, que causa síndrome respiratória aguda grave. Esse vírus afeta o trato respiratório superior e é transmitido, principalmente, por gotículas, espirros e aerossóis e, por isso, há um alto risco de transmissão durante os procedimentos do fonoaudiólogo que atua no ambiente hospitalar. O objetivo deste artigo foi descrever as iniciativas que podem ser adotadas pelo fonoaudiólogo que atende pacientes à beira do leito, com disfagia, visando diminuir o risco de contaminação cruzada na prática clínica durante a pandemia do SARS-CoV-2.
Introduction:Neuropsychomotor development and auditory abilities, influenced by biological and environmental factors, are directly related to school performance. In this way, a screening test in school environment allows the prior identification of change in development, reducing the losses in child life.Objective: To characterize the relationship between neuropsychomotor development and auditory processing skills in preschool children.Methods: 108 preschool children from three public institutions with ages between four years and one month to five years and eleven months, of both sexes were screened. It was performed the screening of Central Auditory Processing (CAP) through the simplified evaluation of auditory processing and the neurodevelopment using the Developmental Screening Test II Denver. The data were analyzed by chi-square tests and bivariate correlation with Pearson coefficient, adopting a significance level of 5% and alpha of 0.1. Results:The screening of the CAP, 100% of the subjects showed normal responses in the test detection, 81.5% in sound localization, 49% in the test of non-verbal sequential memory and 58.3% in the test of verbal sequential memory. In the neuropsychomotor development screening, the frequency of appropriate responses to the chronological age was significant in all areas, being 86.1% (93) in personal social area, 92.5% (100) in the adaptive fine motor, 87 % (94) in language and 92.5% (100) in gross motor. It was found a significant relationship only between the language alterations and proofs of sequential memory (p <0.05). Conclusion:It was found that the group studied was relationship between the language alterations and alterations in the temporal ordering skills of the central auditory processing.
Introdução: o aleitamento materno tem função primordial para o desenvolvimento nutricional, emocional, físico, do sistema motor oral, assim como no desenvolvimento neuropsicomotor infantil. Objetivo: investigar a associação do desenvolvimento neuropsicomotor, da introdução alimentar com o aleitamento materno de lactentes nos primeiros seis meses de vida. Método: 16 lactentes, sem fatores de risco para atraso no desenvolvimento, foram acompanhados mensalmente desde a maternidade até os seis meses para a avaliação do desenvolvimento neuropsicomotor por meio do Teste de Triagem Denver II e da rotina alimentar e hábitos orais por meio de um questionário. Resultados: no primeiro mês, 87,5% dos lactantes foram amamentados em seio materno, mas em 62,5% houve a introdução precoce de chá, água e outros leites e, 68,7% apresentavam hábitos orais. No sexto mês 18,7% realizavam aleitamento materno exclusivo, 43,75% aleitamento misto e 37,5% faziam uso de leite artificial, sendo que 84,6% já haviam introduzido sólidos. 56,25 % mantinham os hábitos orais. Quanto à avaliação do desenvolvimento, na maternidade, todos apresentaram respostas adequadas. No sexto mês na área pessoal social, 93,75% apresentavam desenvolvimento adequado, e 6,25% estavam avançadas. No motor fino adaptativo, 87,5 % adequado, 6,25% avançados e 6,25% apresentavam risco. Na linguagem 100% estavam adequadas à idade e, no motor grosseiro, 31,25% estavam adequadas e 68,75% avançadas. Conclusão: os lactentes que permaneceram em aleitamento materno apresentaram um melhor desenvolvimento neuropsicomotor e uma menor incidência de hábitos orais deletérios, não havendo diferença quanto à introdução alimentar.
It was concluded that patients with CPEO have shorter pharyngeal transit duration of paste and solid boluses than normal volunteers, which may be a consequence of a spontaneous smaller bolus volume in each swallow and/or effortful swallows.
