Objectives We examined associations between transdisciplinary collaboration, evidence-based practice, and primary care and public health services integration in Brazil’s Family Health Strategy. We aimed to identify practices that facilitate service integration and evidence-based practice. Methods We collected cross-sectional data from community health workers, nurses, and physicians (n = 262). We used structural equation modeling to assess providers’ service integration and evidence-based practice engagement operationalized as latent factors. Predictors included endorsement of team meetings, access to and consultations with colleagues, familiarity with community, and previous research experience. Results Providers’ familiarity with community and team meetings positively influenced evidence-based practice engagement and service integration. More experienced providers reported more integration and engagement. Physicians reported less integration than did community health workers. Black providers reported less evidence-based practice engagement than did Pardo (mixed races) providers. After accounting for all variables, evidence-based practice engagement and service integration were moderately correlated. Conclusions Age and race of providers, transdisciplinary collaboration, and familiarity with the community are significant variables that should inform design and implementation of provider training. Promising practices that facilitate service integration in Brazil may be used in other countries.
SUMMARYThis study advances Community-based Participatory Research (CBPR) by presenting a set of triangulated procedures (steps and actions) that can facilitate participatory research in myriad international settings. By using procedural triangulation-the combination of specific steps and actions as the basis for the International Participatory Research Framework (IPRF)-our approach can improve the abilities of researchers and practitioners worldwide to systematize the development of research partnerships. The IPRF comprises four recursive steps: (i) contextualizing the host country; (ii) identifying collaborators in the host country; (iii) seeking advice and endorsement from gatekeepers and (iv) matching partners' expertise, needs and interests. IPRF includes the following sets of recursive participatory actions: (A 1 ) becoming familiar with local languages and culture; (A 2 ) sharing power, ideas, influence and resources; (A 3 ) gathering oral and written information about partners; (A 4 ) establishing realistic expectations and (A 5 ) resolving personal and professional differences. We show how these steps and actions were used recursively to build a partnership to study the roles of community health workers (CHWs) in Brazil's Family Health Program (PSF). The research conducted using IPRF focused on HIV prevention, and it included nearly 200 CHWs. By using the IPRF, our partnership achieved several participatory outcomes: communitydefined research aims, capacity for future research and creation of new policies and programs. We engaged CHWs who requested that we study their training needs, and we engaged CHWs' supervisors who used the data collected to modify CHW training. Data collected from CHWs will form the basis for a grant to test CHW training curricula. Researchers and community partners can now use the IPRF to build partnerships in different international contexts. By triangulating steps and actions, the IPRF advances knowledge about the use of CBPR methods/procedures for international health research.
Resumo Trata-se de ensaio teórico que tem como objetivo propor o que chamamos de clínica da autonomia, no âmbito da promoção da saúde no Sistema Único de Saúde (SUS). Propusemo-nos a responder à questão: como operar a relação entre sujeitos na clínica, tomando como foco a autonomia? Parte-se da concepção das relações como tecnologias leves, potencialmente produtoras de autonomização, conforme proposto por Emerson Merhy, para apontarmos a necessidade de uma modulação da clínica que incorpore práticas de cuidado. À luz das ideias de Félix Guattari e Michel Foucault, concluímos que é necessário um fazer clínico ético-estético e político que recuse a matança dos sujeitos e sustente a invenção da vida.
Com o presente ensaio pretende-se discutir a relação entre as Equipes de Saúde Mental (EqSMs) e os Núcleos de Apoio à Saúde da Família (NASFs) para o cuidado em saúde mental na Atenção Básica (AB) de Belo Horizonte/MG. A discussão se baseia na recuperação de fragmentos históricos da implementação das ações em saúde mental no município e na experiência dos autores. São evidenciados impasses e desafios relacionados à manutenção das EqSMs, lotadas na AB mesmo depois da implantação dos NASFs, o que caracteriza exceção no cenário nacional. A existência de psicólogos em ambas as equipes colabora para a existência de conflitos quanto à divisão de clientela, o que pode resultar em encaminhamento entre elas e em dificuldade de acesso aos cuidados em saúde mental. É indicada a necessidade de diálogo entre essas diferentes estratégias matriciais, reforçando que a discussão dos processos de trabalho é prevista no próprio método de apoio. Resumo O apoio matricial (AM) se divide em duas dimensões: assistencial e técnico-pedagógica. O interesse desta revisão está no caráter técnico-pedagógico (CTP) do AM, por seu potencial para a transformação das práticas em saúde e da organização do trabalho. Este estudo pretende analisar, por meio de revisão bibliográfica exploratória, como o CTP vem sendo abordado na literatura. Parte-se da hipótese de que assim como tal dimensão tem sido pouco operada no cotidiano, ela também tem sido pouco detalhada na literatura. Foi realizada busca ampla do termo “apoio matricial” na Biblioteca Virtual em Saúde e identificados os estudos que abordavam o CTP. Os trechos de artigos que tratavam do assunto, 65 no total, compuseram o corpus submetido à análise temática. Foram identificados quatro grandes eixos na abordagem do CTP do AM: aspecto conceitual do apoio matricial; processos de ensino e aprendizagem no CTP; operacionalização do caráter técnico-pedagógico; efeitos do caráter técnico-pedagógico. Os achados comprovam a hipótese de que, apesar de o CTP ser parte fundamental do conceito e da prática do AM, há pouca produção sobre a temática. Predominam as referências ao CTP como aspecto constituinte do AM, sem maiores esclarecimentos quanto a sua natureza, especificidade ou operacionalização.
