RESUMO O objetivo deste trabalho foi realizar revisão sistemática de estudos que investigaram protocolos de desmame da ventilação não invasiva (VNI) em indivíduos adultos ou idosos hospitalizados. Por meio dessa revisão, baseada no protocolo Prisma, foram selecionados estudos de oito bases de dados publicados em português, inglês ou espanhol. Os desfechos primários desta revisão foram taxa de sucesso de desmame da VNI, duração da VNI e duração da permanência na unidade de terapia intensiva (UTI) e no hospital, e como desfecho secundário foram considerados os efeitos adversos associados aos protocolos de desmame da VNI. A qualidade metodológica dos ensaios clínicos foi avaliada por meio da escala PEDro, e a escala de Downs e Black foi utilizada para a avaliação dos estudos observacionais. Das 18.476 referências iniciais, quatro atenderam aos critérios de inclusão e foram incluídas nesta revisão. Dois dos quatro artigos eram ensaios clínicos randomizados e os outros dois eram estudos observacionais. Os estudos envolveram pacientes idosos com insuficiência respiratória, principalmente por exacerbação da doença pulmonar obstrutiva crônica (DPOC) e edema agudo de pulmão cardiogênico. Foram observadas elevadas taxas de sucesso de desmame em três dos quatro estudos, e um destes demonstrou a superioridade da implementação de um protocolo de desmame imediato na redução da duração da VNI e da permanência na UTI. No entanto, devido à heterogeneidade e à baixa qualidade metodológica dos estudos, não foi possível fazer recomendação sobre a implementação dos protocolos de desmame nessa população.
Introduction: Since 2002, with the creation of Centers of Memory Resources and Research (CMRR), considerable progress has been made in France regarding the administrative and institutional framework within which Alzheimer's disease (AD) is managed. In this study, we explore three approaches that can help optimize the inclusion of patients in clinical trials related to AD. They are as follows: to assess communication concerning clinical trials on AD in French CMRR, to analyze the internal organization of these centers concerning the dynamics of inclusion, and to evaluate screening tools used. Methods: A national, descriptive, cross-sectional survey was conducted in all CMRR in France between May 1 and July 31, 2011, using a self-administered questionnaire. All investigators, subinvestigators and the relevant CMRR personnel were involved. Results: A total of 75% of the CMRR participated, and about 30% of the physicians contacted responded positively to the survey. Only 50% reported having communicated with health care professionals at least once in the previous 3 months, and less than 50% had communicated occasionally with the general public. A total of 75% of those surveyed had a research group but only half of the groups were active, 50% of the physicians did not have a consultation time dedicated to study recruitment, and 75% of the respondents had a screening tool in their CMRR but over half of them were not satisfied with it. Conclusion: This investigation provides a basis for improving the screening of patients by both internal organizational development and adaptation of tools already available. Improvement requires promoting regular communication that is appropriate and targeted to health care professionals, smaller memory centers, associations and the public, and therefore, familiarizing the whole population with clinical research on AD.
Background Fibrinogen is an important biomarker of inflammation, but findings from longitudinal studies that correlated fibrinogen with lung function in older adults are inconsistent. Aim To investigate the relationship between fibrinogen plasma levels and lung function impairment later in life. Methods Longitudinal analysis of 2,150 participants of the English Longitudinal Study of Ageing (ELSA) aged 50 years and older. Associations between changes in plasma fibrinogen between waves 2 (2004–05) and 4 (2008–09) and lung function in wave 6 (2012–13) were performed using multiple linear regression adjusted by potential confounders. Results Regarding the fibrinogen profile, 18.5% of the participants presented higher levels in both waves. In the adjusted models, the maintenance of high fibrinogen levels was associated with a significant reduction of lung function only for men. FEV1 showed a reduction of 0.17L, FVC of 0.22L, and the percentages predicted were 5.16% for FEV1 and 6.21% for FVC compared to those that maintained normal levels of fibrinogen. Discussion To the best of our knowledge, this was the first study investigating the relationship between changes in fibrinogen levels over a long follow-up period and lung function in older adults without pre-existing chronic diseases. ELSA has information on critical demographic and clinical parameters, which allowed to adjust for potential confounding factors. Conclusion It was found that the persistence of high levels of plasma fibrinogen in older English men, but not women, is associated with lung function decline. Therefore, plasma fibrinogen showed to be an important biomarker of pulmonary dysfunction in this population.
Introdução: A Eletroestimulação Neuromuscular (EENM) é uma forma de tratamento utilizada na reabilitação de indivíduos com osteoartrite de joelhos (OJ). Contudo, ainda há na literatura científica discordância sobre a efetividade dessa modalidade de tratamento. Objetivo: Avaliar a efetividade do uso da EENM na dor, desempenho funcional e força muscular em indivíduos com OJ. Métodos: As bases de dados eletrônicas Pubmed/Medline, Pedro e Scielo foram consultadas retrospectivamente até o ano de 2010. Os artigos foram selecionados a partir dos seguintes critérios de inclusão: 1) Design: Estudo Clínico Randomizado; 2) Tipo de intervenção: a) Grupo experimental devendo utilizar a EENM como parte do programa de reabilitação, b) grupo controle, sem tratamento, placebo, outra intervenção terapêutica. Os desfechos primários incluídos na presente revisão foram dor, desempenho funcional e força muscular e como desfecho secundário a qualidade de vida. Resultados: Após a triagem, 26 estudos foram recuperados em texto completo e 7 foram inclusos no estudo. Os desfechos primários inclusos na presente revisão foram dor (7 estudos), desempenho funcional (6 estudos) e força muscular (5 estudos). O desfecho secundário qualidade de vida foi encontrado somente em 2 estudos. Conclusão: Os estudos demonstram efetividade da EENM associada ou não com exercícios físicos para diminuição da dor em indivíduos com OJ. Porém, outras variáveis avaliadas como desempenho físico, força muscular e melhora da qualidade de vida mostraram resultados significativos apenas em comparações intragrupos com o grupo controle.Palavras-chave: estimulação elétrica, modalidades de fisioterapia, doenças reumáticas.
Introduction: Due to the increasing number of postgraduate programs in rehabilitation science and of masters and doctoral researchers in Brazil, there has been significant improvement in the production and quality of these studies. However, this only addresses some research fields but not others, which highlights the need to show the current professional development process and discuss the role of these professionals in an interdisciplinary framework. Objective: To analyze the research lines and disciplines of Brazilians’ postgraduate programs in rehabilitation science. Method: A qualitative descriptive study used the Sucupira platform and data sought from websites properly registered on Capes (Coordination for the Improvement of Higher Education Personnel). Data frequency analysis and the relation between research lines and disciplines were achieved using TextStat (Simple Text Analysis Tool), version 3.0. Results: The most frequent fields of study among the postgraduate programs analyzed were neurology, cardiorespiratory, and musculoskeletal. The description of the research lines and their objectives aim the assessment or intervention for rehabilitation comprehension. Conclusion: The research lines and disciplines do not follow epistemological discussions, neither the change of concepts or paradigms necessary for the successful entry of these professionals into an interdisciplinary framework.
The infant juvenile obesity is currently one of the most serious public health problems of the 21st century, being directly related to physical inactivity as a decisive factor in the worldwide epidemic. The programs of conventional workouts have become not so attractive for this particular population. Because of that the access and permanence in those structured physical activity programs for obese adolescents are difficult. This systematic review has the general goal to verify what benefits the systematic practice of hip hop dance, can promote in obese children and adolescents. The search for relevant articles was conducted in the electronic databases Medline (Pubmed), Cochrane, LILACS, SciELO from January through June 2014, being structured as [(hip hop /) and (obesity /) and (children/ adolescent), controlled and randomized trials were included regarding the systematic practice of hip hop dance as exercise and its benefits for obese children and adolescents, subjects aged 2-19, considered obese and/ or overweight. A total of 21 articles were identified in the survey. Based on the found eligible articles for the full review, only one article met all the inclusion criteria. This preliminary study is intended to broaden the context in health for adolescents and children suffering from overweight and/or obesity, recommending an intervention through dance hip hop, it would be an important modality association for possible monitoring and reducing the risk of associated comorbidities. In this field of research, there is a lack of studies of high methodological quality, which may be able to demonstrate the benefits of this type of intervention. Introdução: Os anos de vida perdidos ajustados por incapacidade (DALY), conhecido como carga de doença, mensuram o impacto de uma doença na população. O câncer de pulmão está entre as principais causas de carga de doença no mundo. Objetivo: Analisar a tendência dos anos de vida perdidos ajustados por incapacidade em pessoas com diagnóstico de câncer de pulmão em Florianópolis, no período de 2008 a 2012. Métodos: Foi realizado um estudo ecológico com dados do Sistema de Informações sobre Mortalidade para estimar os Anos de Vida Perdidos (YLL) e do Registro de Câncer de Base Populacional de Florianópolis para os Anos Vividos com Incapacidade (YLD), e a soma destes, gerou o DALY. Foram calculadas as taxas ajustadas geral, por sexo e faixas etárias. As tendências foram estimadas pelo programa Joinpoint. Resultados: Estimou-se 8.791,17 DALYs, 98,2% relacionados ao YLL. Os indicadores apresentaram estabilidade durante o período. O sexo masculino representou 58,1% da carga. Conclusão: A tendência da carga do câncer de pulmão apresentou estabilidade. Entretanto as faixas etárias de 50 a 69 anos tiveram maiores taxas. Isto reforça a importância da redução do tabagismo na população de Florianópolis, para redução da carga estimada.
