Introduction: The COVID19 pandemic brought a new panorama for the realization of dental treatment for the general population due to the risk of cross infection in the dental office - aerosol formation and insertion of new personal protective equipment. Emergency and emergency dental criteria were defined to limit the flow of patients in the office in order to improve biosafety conditions among patients. Objective: to describe the dental procedures pertinent to outpatient special care or during hospitalization in the VOCID19 pandemic - changes in care and implement biosecurity criteria. Basic procedures: The dental care of the special patient suffers changes - mainly referring to the difficulty of access to the offices - interruption of the conditioning process and difficulty to perform outpatient sedation and sedation with nitrous oxide. Dental care during hospitalization is essential in this patient due to prolonged time of tracheal oro intubation - traumatic oral lesions - preparation of specific oral care protocols - removal of mouth infectious and installation of mouth protectors. Conclusion: Change of care - inclusion of individual protection equipment and new knowledge about the COVID19 allows us to safely assist the patient with special needs both in the dental office and in hospital environment - providing quality of life - oral comfort and reducing oral infections during and after the pandemic. A população LGBTQIA+ ainda hoje sofre com o preconceito e estigma, o que pode dificultar o acesso aos serviços de saúde, aumentando a presença de morbidades neste grupo. O objetivo deste estudo foi realizar uma revisão de literatura com dois enfoques: o acesso aos serviços de saúde e odontológicos por pacientes LGBTQIA+ e quais as medidas educacionais que estão sendo implementadas com alunos dos cursos de Odontologia para diminuir o estigma sobre essa população e universalizar o acesso ao tratamento odontológico. Foi realizada uma revisão de literatura de estudos publicados entre 1995 e 2020 nas bases de dados PubMed, SciELO e Google Scholar. Foram selecionados 19 artigos, relatando principalmente aspectos das experiências de acesso à saúde bucal por pacientes LGBTQIA+; a experiência dos alunos de graduação em Odontologia com essa população; a inserção de atividades focadas na desmistificação desta população em cursos de graduação e o suporte fornecido pelas instituições de ensino aos alunos que se identificam como LGBTQIA+. Apenas dois estudos foram conduzidos no Brasil. Existem evidências provenientes principalmente de estudos internacionais para afirmar que a população LGBTQIA+ possui menor acesso aos serviços de saúde e há uma falta de preparo formal dos alunos de graduação para o atendimento destas pessoas.
Periodontal disease (PD) is an infectious-inflammatory oral disease that is highly prevalent among adolescence and adulthood and can lead to chronic orofacial pain and be associated with anxiety, stress and depression. This study aimed to identify anxiety-like behaviors in the ligature-induced murine preclinical model of PD in different phases of the disease (i.e., acute vs. chronic). Also, we investigated orofacial mechanical allodynia thresholds and superficial cortical plasticity along the orofacial motor cortex in both disease phases. To this aim, 25 male Wistar rats were randomly allocated in acute (14 days) or chronic (28 days) ligature-induced-PD groups and further divided into active-PD or sham-PD. Anxiety-like behavior was evaluated using the elevated plus maze, mechanical allodynia assessed using the von Frey filaments test and superficial motor cortex mapping was performed with electrical transdural stimulation. We observed increased anxiety-like behavior in active-PD animals in the acute phase, characterized by decreased number of entries into the open arm extremities [t(1,7) = 2.42, p = 0.04], and reduced time spent in the open arms [t(1,7) = 3.56, p = 0.01] and in the open arm extremities [t(1,7) = 2.75, p = 0.03]. There was also a reduction in the mechanical allodynia threshold in all active-PD animals [Acute: t(1,7) = 8.81, p < 0.001; Chronic: t(1,6) = 60.0, p < 0.001], that was positively correlated with anxiety-like behaviors in the acute group. No differences were observed in motor cortex mapping. Thus, our findings show the presence of anxiety-like behaviors in the acute phase of PD making this a suitable model to study the impact of anxiety in treatment response and treatment efficacy.
Introdução: Com o aumento da incidência de doenças crônicas, a procura da população por terapias alternativas e complementares (TAC) se intensificou. Essas são normalmente usadas quando as terapias convencionais não conseguem oferecer ao paciente os resultados desejados. No entanto, há poucas evidências dessas terapias, principalmente relacionado ao seu uso na odontologia. Objetivo: realizar uma revisão de literatura verificando as evidências científicas existentes em relação às terapias alternativas e/ou complementares no tratamento odontológico. Métodos: Foram selecionados artigos científicos através de pesquisa abrangente nas bases de dados Pubmed/MEDLINE e Scielo com palavras-chave referentes ao tema “Terapias alternativas e complementares e seu uso na odontologia”. Os critérios de inclusão foram: artigos publicados entre janeiro de 2010 a julho de 2020 nos idiomas inglês e português. Resultados: Inicialmente 373 artigos foram encontrados, e destes, 37 foram selecionados. Acupuntura e musicoterapia foram as técnicas mais descritas na literatura. A aplicação destas terapias parece trazer benefícios principalmente para pacientes ansiosos com o tratamento odontológico. Conclusão: Mais estudos controlados são necessários para aumentar o entendimento sobre as TAC. Essas terapias podem ser aplicadas pelos cirurgiões dentistas, principalmente em pacientes fóbicos e ansiosos.Introduction: The high incidence of chronical diseases has increased the population search for alternative and complementary therapies (ACT). They are usually applied when conventional therapies cannot provide the desired outcome. However, there is a lack of scientific evidence of those therapies, especially regarding its use in dentistry. Purpose: review the scientific literature related to alternative/complementary therapies applied in the dental treatment. Methods: We selected scientific papers, through extensive search in Pubmed/MEDLINE and Scielo databases. Key words regarding the subject “Alternative and complementary therapies and its use in dentistry” were applied. The inclusion criteria comprised studies published between January 2010 and July 2020 in Portuguese and English languages. Results: Initially, 373 papers were found, and among them, 37 papers were selected. The literature described mostly acupuncture and musicotherapy. The appliance of these therapies seems to bring benefits, especially for patients anxious due to the dental treatment. Conclusion: More controlled studies are necessary to increase the understanding of ACT. These therapies can be applied by the dental surgeon, mostly with phobic and anxious patients.
Introdução: A síndrome do cromossomo X frágil é uma síndrome genética que acomete principalmente indivíduos do sexo masculino. O nome desta síndrome ocorre como consequência de um estreitamento da extremidade distal do braço longo do cromossomo X, local chamado de sítio frágil. O presente trabalho apresenta uma revisão de literatura, apresentando etiologia, prevalência, métodos de diagnóstico, características comportamentais, características físicas gerais e de interesse odontológico, além das considerações acerca do atendimento, realizado pelo cirurgião-dentista, em portadores da síndrome do X frágil. Revisão de literatura: As principais características comportamentais são o déficit de atenção, a dificuldade na interação social, a timidez, a ansiedade, a labilidade emocional e os movimentos estereotipados de mãos. Os achados de interesse odontológico mais prevalentes na literatura foram palato ogival, prognatismo mandibular, macroglossia, hipoplasia de esmalte e má oclusão. Discussão: Não foram encontrados muitos artigos voltados para a análise facial e odontológica destes pacientes. O atendimento deste público é um desafio para o cirurgião-dentista devido às características comportamentais e fisiológicas apresentadas. Conclusão: o conhecimento das características desta síndrome pelo profissional é importante, pois a síndrome comumente se associa à doenças sistêmicas que podem influenciar no plano de tratamento, além de alterações orofaciais importantes.
Introdução: Pacientes com Síndrome de Munchausen (SM) simulam ou reproduzem lesões ou doenças com a intenção de assumir o papel de doente, sem o objetivo de ganho secundário. A familiarização dos profissionais da saúde com a SM e suas possíveis manifestações orofaciais pode impedir a realização de procedimentos desnecessários e tratamentos com insucessos consecutivos. Objetivo: Este estudo tem como objetivo identificar as manifestações orofaciais apresentadas por pacientes diagnosticados com a SM. Método: Foi realizada uma revisão de literatura pautada nos seguintes descritores: Síndrome de Munchausen, Boca, Face, Odontologia, Manifestações Bucais, Traumatismos Faciais e Traumatismos Maxilofaciais. Foram incluídos artigos nos idiomas inglês e português que discutem a SM e suas manifestações na região orofacial. A busca foi realizada nas bases de dados PubMed, LILACS, Embase e SciELO. Resultados e Conclusão: Foram incluídas 18 publicações que resultaram no relato de 22 casos de pacientes com manifestações orofaciais e diagnosticados com a SM. As manifestações orofaciais relatadas foram: dores orofaciais difusas ou mimetizando odontalgias; tumefação em face ou intraoral devido injeção de materiais ou ar nos tecidos; lesões ulceradas, bolhosas e sangrantes em mucosa oral; ulcerações, eritemas e equimoses em face; e disfunções da articulação temporomandibular (ATM) envolvendo múltiplas cirurgias e simulação de subluxação da ATM. Incentiva-se a publicação de relatos de caso para familiarizar o profissional da saúde com possíveis manifestações orofaciais em pacientes com a SM, contribuindo para realização de um correto diagnóstico e evitando a perpetuação do ciclo de assistência médica.Palavras Chave: Síndrome de Munchausen, Manifestações bucais, Traumatismos maxilofaciais, Traumatismos faciaisABSTRACTIntroduction: Patients with Munchausen Syndrome (MS) tend to simulate or reproduce an injury or illness with the intention of assuming a “sick role”, without the objective of a secondary gain. The habituation of health care practitioners with MS and its possible oral and maxillofacial manifestations may prevent unnecessary procedures and consecutive unsuccessful treatments. Objective: This study aimed to identify the oral and maxillofacial manifestations presented by patients diagnosed with MS. Methods: A review of the literature was carried out, based on the following descriptors: Munchausen Syndrome, Mouth, Face, Dentistry, Oral Manifestations, Facial Injuries and Maxillofacial Injuries. Publications in English and Portuguese related to MS and its oral and maxillofacial manifestations were included. The search was carried out using the following databases: PubMed, LILACS, Embase e SciELO. Results and Conclusion: 22 cases were assessed in 18 articles that reported patients presenting oral and maxillofacial manifestations and diagnosed with MS. The oral and maxillofacial presentations were: diffuse orofacial pain, occasionally similar to odontalgia; facial or intraoral swelling due to materials or pressurized air injections into the tissues; ulcerative, vesiculobullous and bleeding oral lesions; facial ulceration, erythema and ecchymosis; and temporomandibular joint disorders including multiple surgeries and simulation of subluxation of temporomandibular joint. Case reports must be performed in order to acquaint health care practitioners with possible oral and maxillofacial manifestations presented by patients diagnosed with MS, thus contributing to a correct diagnosis and preventing a cycle of repeated admissions to health care services.
