Introduction The literature presents studies correlating chronic obstructive pulmonary disease to dysphagia and suggesting that the aspiration laryngeal phenomenon related to changes in the pharyngeal phase contributes significantly to the exacerbation of symptoms of lung disease. Objectives This study aimed to conduct a literature review to identify the relation between dysphagia and exacerbations of chronic obstructive pulmonary disease. Data Synthesis We found 21 studies and included 19 in this review. The few studies that related to the subject agreed that the presence of dysphagia, due to lack of coordination between swallowing and breathing, may be one of the triggering factors of chronic obstructive pulmonary disease exacerbation. Conclusions The review noted that there is a relationship between dysphagia and exacerbations of chronic obstructive pulmonary disease, identified by studies demonstrating that the difficulties associated with swallowing may lead to exacerbation of the disease. There was difficulty in comparing studies by their methodological differences. More research is needed to clarify the relationship between dysphagia and exacerbations of chronic obstructive pulmonary disease, making it possible to develop multiprofessional treatment strategies for these patients, catered to specific needs due to the systemic manifestations of the disease.
RESUMOTema: esse trabalho foi baseado na temática de que existe uma associação entre as más oclusões devido a alterações do crescimento craniofacial, e, por conseguinte, a existência de alterações miofuncionais. Objetivos: estudar a associação entre as más oclusões tipo classe II desencadeadas por alterações do crescimento craniofacial e as disfunções do sistema estomatognático (alterações miofuncionais: fala, mastigação, deglutição e fonação). Conclusão: pode-se concluir que alterações estruturais da face podem ter influência na funcionalidade das mesmas, portanto, enfatiza-se a importância do trabalho multidisciplinar entre os profissionais envolvidos em cada uma dessas aéreas para que o prognóstico desses casos seja pertinente de relevantes melhoras. DESCRITORES: Conflito de interesses: inexistenteMuitas vezes, essas alterações ocorrem devido às adaptações faciais e cranianas que são relacionadas às funções vegetativas desempenhadas pelo sistema estomatognático, à determinação genética, e à expansão do cérebro humano 1,2 . Pode-se citar, como exemplo, o caso de respiradores orais, em que o modo respiratório está alterado, podendo gerar alterações craniofaciais como as más oclusões dentárias e esqueléticas 3 . Devido à relação entre alterações esqueléticas e dentárias com os aspectos funcionais, posturais e de tônus dos órgãos fonoarticulatórios, sabe-se que as últimas podem interferir no crescimento, desenvolvimento ou funcionamento das estruturas orofaciais 4-7. Na prática clínica esse estudo possui grande relevância, visto que os aspectos aqui abordados são observados como integrantes de um todo, ou seja, para uma terapia ou tratamento ser eficaz deve-se notar a interligação de fatores, como por exemplo, a relação entre alterações oclusais e miofuncionais. Esse fato se confirma, pois, na existência da primeira alteração poderá existir INTRODUÇÃOAs alterações esqueléticas, especificamente as faciais, podem ser de diversos tipos (verticais, transversais e anteroposteriores) por existir modelos básicos e divergentes de padrões da face.
OBJETIVO: correlacionar os tempos máximos de fonação (TMF) e as relações entre os fricativos /s/ e /z/ (s/z) e entre /e/ não vozeado e a vogal /e/ (ė/e) de mulheres sem afecções laríngeas. MÉTODO: participaram 60 mulheres com média de 21,56 anos de idade. Coletaram-se os TMF/ė/, /e/, /s/ e /z/ e calcularam-se a relação s/z e a relação ė/e, com padrão de normalidade para as relações de 0,8 a 1,2; para os TMF/s/ e /z/, entre 15,57 e 34,17s; para o TMF/ė/, entre 16 a 18s; e para o TMF/e/, entre 14,04 e 26,96s. Testes de Lilliefords, Spearmann, Binomial e Mann-Whitney com nível de significância de 5%. RESULTADOS: correlação positiva entre TMF/s/ e TMF/ė/, TMF/z/ e TMF/e/, TMF/s/ e TMF/z/, e TMF/ė/ e TMF/e/. Não houve correlação entre as relações s/z e ė/e, nem diferenças entre a relação ė/e, enquanto a relação s/z foi significantemente normal. TMF/ė/ e TMF/e/ significantemente diminuídos; TMF/s/ e TMF/z/ significantemente normais. TMF/ė/ significantemente menor do que TMF/s/; TMF/e/ significantemente menor do que TMF/z/. CONCLUSÃO: os fonemas /s/ e /z/ isolados e sua relação ficaram dentro da normalidade e as relações s/z e ė/e não apresentaram correlação. Os TMF/ė/ e TMF/e/ mostraram-se diminuídos em relação à normalidade. O TMF/ė/ foi menor do que TMF/s/ e o TMF/e/ foi menor do que TMF/z/, possivelmente devido ao modo articulatório dos fricativos ter aumentado o tempo de emissão, independentemente do controle do nível glótico e respiratório.
RESUMOObjetivo: verificar e correlacionar os tempos máximos de fonação (TMF) de vogais, a capacidade vital (CV) e os tipos de afecções laríngeas (AL) de mulheres com disfonia organofuncional (DOF). Método: pesquisa retrospectiva, transversal, exploratória, não experimental, quantitativa, com banco de dados de medidas de TMF [a, i, u], de CV e de AL de mulheres com DOF; e os testes estatísti-cos Qui-quadrado e exato de Fisher, para verificar as diferenças entre as variáveis e suas relações e o teste binomial, a fim de verificar a significância de proporção ou percentual da análise descritiva, com p<0,05. Resultados: a maioria (22; 75,86%) apresentou TMF significantemente reduzidos (p=0,0053) e sete (24,14%) TMF normais. A CV normal foi estatisticamente significante (p=0,0001) (26; 89,66%), mas três mulheres (10,34%) a apresentaram reduzida. Houve predomínio significante dos nódulos vocais (p=0,0016) (22; 75,86%), seguido de edema de Reinke (6; 20,69%) e de pólipo vocal (1; 3,45%). Dentre as 22 mulheres (75,86%) que mostraram TMF reduzidos, houve predomínio com CV normal (19; 86,36%), embora sem significância estatística (p=0,558). Das mulheres com TMF normais, todas apresentaram CV normal (7; 100%). A maioria com DOF apresentou CV normal, embora sem significância estatística (p=0,199). Conclusão: nas mulheres com DOF deste estudo, os TMF reduzidos, a CV normal e a presença de nódulos vocais foram significantes e não houve relação entre as variáveis TMF, CV e AL. . As queixas referidas pelos indivíduos que apresentam alguma alteração vocal geralmente são de sensações negativas relacionadas à fonação, as chamadas disestesias vocais, tais como dores no pescoço ou na garganta após uso intenso da voz. Existem fatores que podem contribuir para o surgimento de alterações vocais, como distúr-bios alérgicos, faríngeos, orais, nasais, otológicos, pulmonares, digestivos, hormonais e/ou neurovegetativos, além de hábitos como tabagismo, etilismo, exposição a mudanças bruscas de temperatura e comportamentos de uso vocal incorreto 3,4 . Introduction: Knowledge about effective vocal techniques will contributes to the prevention/treatment of work-related voice disorder. Purpose: Verifying acoustic vocal, auditory-perceptive and videolaryngostroboscopic measures in dysphonic teachers of two groups before and after a brief and intensive therapy program with finger kazoo technique, comparing to their control groups, and comparing the study groups between themselves. Methods: Randomized study, pre-test and post-test, with control group, blind to the evaluator. Two study groups, with and without structural laryngeal affection (24 dysphonic teachers) performed15 sessions of a brief and intensive care with finger kazoo over three weeks and they were compared to two control groups (17 dysphonic teachers), who did not do therapy, in a total of 41 subjects. We carried out vocal acoustic analysis (Multi-Dimensional Voice Program Advanced), perceptual analysis (by three speech therapists) and videolaryngostroboscopy analysis (by three otolaryngologists) to compare the results obtained by each group. Results: Significant reduction in harmonic-noise ratio in the study group with alteration after therapy was observed. In the comparison between the study group without affection with the respective control, there was a significant decrease in jitter, shimmer and voiceless segments in favor of the study group. It was also observed a significant reduction in dysphonia, hoarseness, breathiness and tension levels, as well as a lower incidence of triangular slit -level II -and higher amplitude of vibration of the vocal folds in the study group with alteration. Conclusion: The brief and intensive care with finger kazoo provided voice, glottal closure and amplitude of mucosal wave vibration of the vocal folds improvements in dysphonic teachers with and without structural alterations of the vocal folds, especially in teachers without structural alterations.
OBJETIVO:verificar a diferença entre o tempo máximo de fonação /a/ e tempo máximo de fonação previsto em relação ao tipo respiratório, e correlacionar os tempos máximos em mulheres adultas com capacidade vital dentro da normalidade e sem afecções laríngeas.MÉTODOS:pesquisa transversal analítica, quantitativa e retrospectiva, em banco de dados, de 51 sujeitos do sexo feminino, com diagnóstico otorrinolaringológico de ausência de afecção laríngea, com idades entre 18 e 44 anos (média 27,64). Utilizaram-se os dados: maior valor de tempo máximo de fonação de /a/, tempo máximo de fonação previsto, capacidade vital e tipo respiratório. O valor do tempo máximo previsto para mulheres foi calculado multiplicando-se a capacidade vital por 0,0051, e o valor encontrado foi determinante para a classificação do tempo de /a/ em normal, abaixo ou acima do previsto.RESULTADOS:houve diferença significante entre tempo de fonação previsto e os tipos respiratórios superior (17,44s) e misto (15,17s). Não houve diferença significante do tempo máximo de fonação /a/ nos diferentes tipos respiratórios, nem correlação entre tempo de fonação /a/ e tempo previsto.CONCLUSÃO:não houve correlação entre tempo de /a/ e tempo de fonação previsto e este último foi significantemente maior no tipo respiratório superior do que no misto na população estudada.
The purpose of this study was to identify use of life protocols in dysphagia and verify their use in reahbilitation. Was performed a theoretical and exploratory research with the review of the literature technical, on the basis of SCOPUS data, Trip Database, LILACS, PubMed, SciELO, Google Scholar, Capes journals and MedLine. The search time comprised the years between 2004 and 2014 and the following keywords were used: swallowing; swallowing disorders; quality of life; questionnaires and their respective terms in English: deglutition; deglutition disorders; quality of Life; Questionnaires. The protocols found in the literature was the Quality of life in Swallowing Disorders-SWAL-QOL, which establishes the impairment of swallowing independent of the etiology; the MD Anderson Dysphagia Inventory, which is specific for subjects submitted treatment for head and neck cancer and the Dysphagia Handicap Index, which evaluates the effects of dysphagia on quality of life (QOL) in subjects with different pathologies and can be used at lower levels of education.The literature proposes different protocols that assess QoL in dysphagia, the most used to assess QOL in general, related to head and neck cancer and subjects with different medical diagnoses . The use of these protocols can support and complement the clinical and objective evaluation of swallowing once, that portray the self-assessment by the subject, and this view is extremely important for speech therapy.
