Extra- and intraneural ganglion cysts have been described in the literature. The tibial nerve ganglion is uncommon and its occurrence without intra-articular extension is atypical. The pathogenesis of cystic degeneration localized to connective and perineural tissue secondary to chronic mechanical irritation or idiopathic mucoid degeneration is hypothesized. Since the above pathology is extremely rare and the magnetic resonance imaging examination detects the defining characteristics of the intrinsic alterations of the tibial nerve, the authors illustrate such a case of tibial intaneural ganglion cyst with its magnetic resonance neurography and sonography appearances.
Magnetic resonance neurography is a technique for identifying anatomy and pathologic lesions of nerves, and has emerged as a helpful technique for localizing lesions and elucidating the underlying etiology. Ganglioneuromas are highly differentiated benign tumors. This lesion is rare and exhibits undetermined symptoms, the features of using the magnetic resonance neurography are a great ally to determine its diagnosis. The authors illustrate a case of retroperitoneal ganglioneuroma emphasizing its image characteristics using magnetic resonance neurography with the diagnosis confirmed by histopathological examination.
Introdução: Lesões no ápice petroso são geralmente assintomáticas ou relacionadas a sintomas inespecíficos. Seu diagnóstico depende dos resultados obtidos pela avaliação conjunta da tomografia computadorizada (TC) e ressonância magnética (RM). Objetivo: Relatar um caso de lesões incidentais em ápices petrosos (AP) e revisar na literatura sobre os diagnósticos diferenciais. Relato de Caso: Paciente com cervicalgia evidenciou lesões incidentais nos AP, em RM de coluna. Realizou-se investigação por TC e RM de crânio. As lesões apresentavam conteúdo cístico, remodelamento ósseo expansivo e íntima relação com o canal carotídeo esquerdo. Presumiu-se o diagnóstico de mucocele e programou-se abordagem cirúrgica. Entretanto, o diagnóstico intraoperatório foi cisto aracnoide. Discussão: Lesões do ápice petroso podem ser caracterizadas de acordo com sua origem, com as alterações que determinam nas estruturas ósseas adjacentes e com as características de sinal na RM. As lesões preenchidas por líquido cefalorraquidiano (LCR) representam um desafio, pois não possuem características específicas. Na RM, a mucocele segue o sinal do LCR em todas as sequências, indistinguível do cisto aracnoide pelas técnicas de imagem. Cistos aracnoides representam cerca de 1% das lesões intracranianas e podem apresentar-se em locais atípicos como o ápice petroso. Conclusão: Embora raro, esse diagnóstico deve ser considerado nas lesões preenchidas por LCR
