Amaç: Hepatit C virüsü (HCV), kronik hepatit, siroz ve hepatosellüler karsinoma yol açarak dünya çapında önemli bir sağlık sorunu olmaktadır. Dizi analizi çalışmaları ile HCV'nin 7 genotipi ve 100'den fazla alt tipi olduğu ortaya konmuştur. HCV genotip 1'in genotip 2 ve 3'e göre daha ciddi hastalık yaptığı ve tedaviye kalıcı yanıtın daha az olduğu bilinmektedir. Amaç: Bartonella türleri insanlarda kedi tırmığı hastalığı, basiller anjiomatozis, basiller peliozis, Carrion hastalığı, enfektif endokardit ve siper ateşi gibi pek çok hastalığa yol açmaktadır. Türkiye'de Bartonella'lara bağlı kedi tırmığı hastalığı ve basiller anjiomatozis olgu sunuları bulunmaktadır. Bu çalışmada, Türkiye'nin batısında, kedi/kopek sahibi riskli insan, kedi ve köpeklerdeki Bartonella henselae seropozitivitesinin araştırılması amaçlanmıştır. Gereç ve Yöntemler: Bu çalışmaya, 34'ü ev, 97'si sokak olmak üzere toplam 131 kedi/köpek sahibi ve 57'si ev kedisi, 48'i barınak kedisi olmak üzere toplam 105 kedi ile 45 ev köpeğinin serum örnekleri alınmıştır. Serum örneklerinde B. henselae IgG antikorları iki ticari kit kullanılarak immunfluoresan antikor yöntemi ile belirlenmiştir. Bulgular: B. henselae seropozitifliği, ev kedi/köpek sahiplerinde sokak kedi/köpek sahiplerine göre (sırasıyla %26,5 ve %6,8). Seropozitiflik, istatistiksel olarak anlamlı Objective: Bartonella species cause several diseases in humans such as cat scratch disease, bacillary angiomatosis, peliosis hepatis, endocarditis, Carrion disease and trench fever. There have been cat scratch disease and bacillary angiomatosis cases reports in Turkey. The aim of this study is to determine the seropositivity against Bartonella henselae in cat/dog owners who are in the risk group, cats and dogs in Western Aegean region, Turkey. MaterialsandMethods:In this study, B. henselae immunoglobulin (Ig) G positivity was investigated in a total of 281 samples including a total of 131 people, 34 of whom are pet cat/dog owners and 97 of whom are stray cat/dog owners; as well as a total of 105 cats, of which 57 pet cats, 48 shelter cats, and 45 pet dogs. Sera tested for the presence of antibodies against B. henselae IgG using immunofluorescence assay with two commercial kits. Results: B. henselae seropositivity of pet owners was significantly higher than the stray cat/dog owners (26.5% vs 6.8%). B. henselae IgG was found positive in 36.2% of total cats, 22.8% of pet cats, 52.1% of shelter cats. B. henselae seropositivity was found statistically higher in shelter cats than pet cats. No positivity was detected in the samples taken from the dogs. Conclusion: It is concluded that being pet owner at home poses a risk for B. henselae. For the differential diagnosis, especially in patients in close contact with cats, B. henselae infection should be considered.
Acinetobacter baumannii çoklu antimikrobiyal direnç gösterebilen ve hastaneden kazanılmış enfeksiyonlarda önemi olanbir patojendir. Çoklu ilaca dirençli A. baumannii suşlarının tedavisinde karbapenemler seçenektir. Çalışmamızda, karbapenem dirençli A. baumannii suşlarında direnç mekanizmalarının ve moleküler epidemiyolojisinin araştırılması amaçlanmıştır.
Objective: To evaluate the value of autoantibody and viral serologic examinations in the diagnosis of multiple sclerosis (MS) and stroke and their effect on the course of treatment. Materials and Methods: Patients who were admitted to the neurology clinic between 2012 and 2016 were retrospectively evaluated. The patients were screened for autoantibodies including anti-nuclear antibody (ANA) and anti-dsDNA, and viral serology including Epstein-Barr virus, varicella-zoster virus, cytomegalovirus, herpes simplex virus type 1 and 2. The study cohort was grouped as the MS group, stroke group, and "other" diseases group (e.g. polyneuropathy, myasthenia gravis, dementia, headache, epilepsy). The data from all the groups were further analyzed to determine whether these tests provided an increase in diagnostic performance. Results: Among the autoantibody and viral serologic tests, the most commonly used test was ANA (71 cases in the MS group, 160 cases in the stroke group, and 482 cases in the other diseases group). All test reports, based on positivity/negativity, did not lead to any change in the initial diagnosis of the disease and the treatment strategy in all groups. Conclusion: Information obtained from autoantibody and viral serologic tests does not affect the diagnosis of MS and stroke. Performing these tests for routine screening is considered worthless unless there is an important finding regarding clinical disease.
Nocardiosis is a gram positive bacterial infection caused by aerobicactinomycetes. It’s nonspesific presentation and radiographic findings usually make it undiagnosed with highly fatal outcomes. The patients with intracelluler defects are prone to nocardiosis. Multiplemyelom(MM) itself is a mainly humoral deficiency disorder because of defective antibodies but recent drugs such as proteasome inhibitors and immuno modulatory drugs also cause cellular deficiency. Here we presented two definite and oneprobable pulmonary nocardiosis patients who have a diagnosis of MM, autologou shematopoetic stem cell transplant history in their past and anti myeloma therapy presently.

Dermatofi tozların tanısında kullanılan klasik yöntemlerden direkt mikroskopik incelemenin tür ayrımını yapamaması, kültürün ise uzun süre gerektirmesi ve duyarlılığının düşük olması gibi dezavantajları bulunmaktadır. Bu çalışmada, klinik örneklerden dermatofi tlerin saptanmasında ve kültürden izole edilen suşların tanımlanmasında polimeraz zincir reaksiyonu (PCR) yönteminin performansının araştırılması amaçlanmıştır. Çalışmaya, dermatofi toz ön tanısı konulan 110 hastaya (69 kadın, 41 erkek; yaş aralığı: 4-82 yıl) ait 63'ü deri ve 60'ı tırnak kazıntısı olmak üzere toplam 123 örnek alınmıştır. Örnekler, rutin direkt mikroskopi ve mantar kül-türünün yanı sıra iki turlu (nested) PCR (nPCR) yöntemine dayalı iki ayrı protokol ile incelenmiştir. Birinci protokolde dermatofi tlerin kitin sentaz genini (CHS-1) hedefl eyen pan-dermatofi t nPCR; ikinci protokolde ise Trichophyton rubrum ve T. mentagrophytes'e özgül ITS-1 genlerini hedefl eyen primerlerin kullanıldığı nPCR uygulanmıştır. Kültürde üreyen suşlara da aynı şekilde PCR uygulanmıştır. Pan-dermatofi t nPCR'da pozitif, T.rubrum/T.mentagrophytes'e özgül nPCR ile negatif sonuç veren örnekler ayrıca dizi analizine alınmıştır. Çalışmamızda, örneklerin 62'sinde (%50) direkt mikroskopik incelemede hif ve/veya spor yapıları görülmüş, 30'unun (%24) kültüründe dermatofi t üremiştir. T.rubrum/T.mentagrophytes'e özgül nPCR ile kültürde üreyen izolatların 28'i T.rubrum, ikisi T.mentagrophytes olarak tanımlanmıştır. Direkt uygulamada klinik örneklerin 67'si (%55) pan-dermatofi t nPCR ile, 65'i (%53) ise T. rubrum/T. mentagrophytes'e özgül nPCR ile pozitif bulunmuştur. Direkt mikroskopide mantar elemanı görülmeyen örneklerin kültüründe de üreme olmamış, bu örneklerin dokuzu pan-dermatofi t nPCR, sekizi T. rubrum/T. mentagrophytes'e özgül nPCR ile pozitif sonuç vermiştir.
