We believe upper endoscopy should be performed to investigate esophageal LP and gastrointestinal pathologies especially when LP patient is symptomatic. Although we didn't detect esophageal LP, our study had the limitation of taking biopsies from pathological sites only. Since histopathological examination of normal appearing esophagus may help in diagnosing occult LP, and prevent eventual complications, it may be further evaluated in larger study groups. A new technique, magnification chromoendoscopy, may be useful in detecting esophageal involvement. We believe the possible role of H. pylori in LP is yet to be determined also.
Amaç: Bu çalışmada orta şiddetli akne olgularında, aralıklı düşük doz izotretinoin tedavisinin etkinlik, yan etki, yaşam kalitesine etkileri ve nüks görülme sıklığının araştırılması amaçlanmıştır. Gereç ve Yöntemler: Eylül 2007-Nisan 2009 tarihleri arasında akne polikliniğimizde izlenen hastalar retrospektif olarak incelendi. Haftada üç gün 20 mg/gün sabit dozda izotretinoin uygulanan hastalar değerlendirmeye alındı. Olguların demografik özellikleri ve tedavi bilgileri hasta dosyalarından edinildi. Olguların yaşı, kilosu, aknenin süresi ve önceki tedavileri, tedavi öncesi ve sonrası Global Akne Skorlama Sistemi (GASS) temel alınarak yapılan akne derecelendirmesi, ilaç uygulanmasından kaynaklanan yan etkiler ve tedavi öncesi ve sonrası bakılan Dermatoloji Yaşam Kalite İndeksi (DYKİ) skorları kaydedildi. Tedavi öncesinde ve tedavi sürecindeki aylık takiplerde bakılan tam kan sayımı, karaciğer fonksiyon testleri ve lipid profilindeki değişiklikler not edildi. Bulgular: Çalışmaya alınan toplam 35 olgunun %94.3'ünde 1 yıllık tedavi sonunda akne global derecelendirmesinde tam yanıta ulaşıldığı tespit edildi. Kısmi yanıta ulaşma oranı ise %5.7 olarak bulundu. Olgularda en erken klinik yanıtın dördüncü haftadan itibaren başladığı izlendi. DYKİ tedavi başlangıcında 7.8 iken tedavi sonunda 1.8'e gerilemişti. Hafif derecede keilit dışında yan etkiye rastlanmadı. Tedavi bitiminden ortalama 17 ay sonraki değerlendirmede, 5 olguda hafif dereceli nüks gözlendi. Sonuç: İzotretinoinin düşük doz aralıklı kullanımının orta şiddetteki akne olgularında tolere edilebilir ve etkili bir tedavi olduğunu göstermektedir. Materials and Methods: Patients followed up at our acne outpatient clinic between September 2007 and April 2009 were evaluated. Those treated with 20 mg/day isotretinoin three times a week were included in the study. Relevant data were recorded from patient files. We recorded the age and weight of the cases, duration of acne, previous treatments, acne grading based on global acne scoring system, side effects and pretreatment and posttreatment Dermatology Life Quality Index scores. Hemogram, liver function tests, lipid profiles at baseline and monthly follow-ups were noted. Results: Global acne scoring of complete response rate in 94.3% of 35 patients was achieved after one year of therapy. The partial response rate was 5.7%. The earliest clinical response was observed after four weeks. The Dermatology Life Quality Index was 7.8 at baseline and 1.8 at the end of the treatment. Cheilitis was the only side effect observed. We observed mild degree relapses in 5 cases in the evaluation 17 months after the end of the treatment.
