PurposeThe purpose of this study was to compare the visual outcomes and subjective visual quality between bilateral implantation of a diffractive trifocal intraocular lens, Alcon Acrysof IQ® PanOptix® TNFT00 (group A), and blended implantation of an extended depth of focus lens, J&J Tecnis Symfony® ZXR00 with a diffractive bifocal intraocular lens, J&J Vision Tecnis® ZMB00 (group B).MethodsThis prospective, nonrandomized, consecutive, comparative study included the assessment of 40 eyes in 20 patients implanted with multifocal intraocular lens. Exclusion criteria were existence of any corneal, retina, or optic nerve disease, previous eye surgery, illiteracy, previous refractive surgery, high axial myopia, expected postoperative corneal astigmatism of >1.00 cylindrical diopter (D), and intraoperative or postoperative complications. Binocular visual acuity was tested in all cases. Ophthalmological evaluation included the measurement of uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), uncorrected near visual acuity (UNVA), and uncorrected intermediate visual acuity (UIVA), with the analysis of contrast sensitivity (CS), and visual defocus curve.ResultsPostoperative UDVA was 0.01 and −0.096 logMAR (p<0.01) in groups A and B, respectively; postoperative CDVA was −0.07 and −0.16 logMAR (p<0.01) in groups A and B, respectively; UIVA was 0.14 and 0.20 logMAR (p<0.01) in groups A and B, respectively; UNVA was −0.03 and 0.11 logMAR (p<0.01) in groups A and B, respectively. Under photopic conditions group B had better CS at low frequencies with and without glare.ConclusionBoth groups promoted good quality of vision for long, intermediate, and short distances. Group B exhibited a better performance for very short distances and for intermediate and long distances ≥−1.50 D of vergence. Group A exhibited a better performance for UIVA at 60 cm and for UNVA at 40 cm.
OBJETIVOS: verificar a prevalência do frênulo lingual alterado e suas implicações na fala de escolares. MÉTODOS: foram avaliados por três fonoaudiólogas os frênulos de língua de 260 crianças com idades variando entre 6 e 12 anos. Os frênulos foram classificados por meio de inspeção visual, medidas empregando-se paquímetro e avaliação da tensão, mobilidade e posicionamento da língua. Foram consideradas crianças com alteração de frênulo aquelas que apresentaram alteração em todas etapas da avaliação. Nos casos de classificação de frênulo alterado, a fala foi avaliada. RESULTADOS: os dados encontrados revelaram que das 260 crianças avaliadas 47 (18%), apresentaram alteração de frênulo, sendo 28 (60%), classificados como curtos; 12 (25%) como anteriorizados e 7 (15%) como curtos e anteriorizados. Não houve diferença entre os sexos. Dos indivíduos com frênulo alterado, 34 (72 %) apresentaram alteração de fala. A prevalência de alteração na fala foi maior no frênulo curto e anteriorizado (85%), seguido pelo curto (75%) e pelo anteriorizado (58%). As implicações de fala mais encontradas foram distorção e articulação trancada. CONCLUSÃO: foi verificada uma prevalência de 18% de alteração no frênulo lingual dentre os escolares avaliados, sem diferença entre os sexos. O frênulo curto predomina sobre os demais tipos, porém o curto e anteriorizado apresenta maiores implicações na fala. As características de fala mais comuns nestes casos são distorção e articulação trancada.
Summary Background Lingual exercises are commonly used in clinical practice for swallowing rehabilitation. Associating lingual exercises with computer games increases motivation, which influences tongue motor performance. Objective To investigate the effects of tongue movement direction; resistance force level; repetition number; sustained tongue contraction duration; age and gender on tongue motor performance in healthy adults using computer games. Methods An observational pilot study was carried out at a university laboratory. Nine healthy adults, aged 22 to 38 years, used an intra‐oral joystick controlled by the tongue to play four computer games. The participants had to reach 12 targets that appeared on the computer screen using the intra‐oral joystick. Motor performance was measured by the number of attempts to score and the time during which the target force was maintained. Tongue motor performance was compared among tongue movement direction, resistance force level, game round number, and continuous force application time on the target, age and gender. Results The number of attempts depended significantly on the direction, continuous force application time on the target and age. The time during which the target force was maintained depended significantly on the direction, continuous force application time on the target and game round number. There were no significant differences in the comparisons by gender or by resistance force level. Conclusions It was seen that young adults had their best performance in the downward direction, on the third round, holding the force for a shorter time. The performance deteriorated as age increased.
OBJETIVO: descrever os achados miofuncionais orofaciais, bem como os principais problemas otorrinolaringológicos, alergológicos e ortodônticos encontrados em crianças com respiração oral. MÉTODOS: análise de prontuários de 502 crianças do Ambulatório do Respirador Oral do Hospital das Clínicas da Universidade Federal de Minas Gerais. Os participantes tinham idades entre 2 e 12 anos (mediana de 6,0 anos), sendo 289 (57,6%) do sexo masculino e 213 (42,4%) do sexo feminino. Foram coletados dados dos prontuários referentes à anamnese geral, avaliação fonoaudiológica, bem como as partes relevantes das avaliações otorrinolaringológica, alergológica e ortodôntica. Os dados foram submetidos à análise estatística. RESULTADOS: na anamnese, observou-se prevalência significante de permanência de boca aberta (98,0%), ronco (89,9%) e sialorreia noturna (68,6%). Na avaliação alergológica, verificou-se teste cutâneo positivo (59%) e rinite (57,8%) e na otorrinolaringológica, hipertrofia de adenoide (91,7%) e amígdalas (72,6%), além de mucosa nasal alterada (60,3%). A avaliação ortodôntica indicou presença de má oclusão (86,8%), perfil facial convexo (62,9%) e trespasse vertical aumentado (55,5%). Os dados da avaliação fonoaudiológica indicaram inadequação da posição habitual de lábios (70,5%), tensão de lábios (65,4%) e de língua (64,4%) alteradas, palato duro alto (57,1%), ângulo nasolabial alterado (57,0%) e assimetria facial (55,0%). CONCLUSÃO: verificaram-se alterações nas avaliações realizadas por todos os profissionais, confirmando o grande impacto da respiração oral na qualidade de vida e, portanto, a necessidade de tratamento multidisciplinar para esses pacientes.
RESUMOObjetivo: verifi car a contribuição da Fonoaudiologia no tratamento de uma paciente com a síndrome da apnéia e hipopnéia obstrutiva do sono. Métodos: relato de caso de um paciente do sexo feminino, de sessenta anos de idade, com a síndrome da apnéia e hipopnéia do sono, de grau grave, que se submeteu ao atendimento fonoaudiológico para melhora do quadro de apnéia/hipopnéia, ronco e cansaço diurno. Foram realizadas avaliação clínica de motricidade orofacial e polissonografi a antes e após a fonoterapia. Com base na avaliação clínica foi elaborado um plano terapêutico que buscou propiciar à paciente relaxamento cervical e da musculatura supra-hióidea, melhora na aeração nasal, adequação do posicionamento e força de língua, fortalecimento dos músculos do palato mole e sua mobilidade, aumento de força da musculatura mastigatória, treino da mastigação bilateral alternada e abaixamento do osso hióide. Resultados: após 12 sessões de terapia fonoaudiológica, com duração de 40 minutos cada, pôde-se observar diminuição da tensão cervical, relaxamento da musculatura supra-hióidea, adequação do posicionamento do osso hióide, língua normotensa com dorso rebaixado, palato mole com mobilidade normal e mastigação adequada. A paciente relatou importante melhora no cansaço diurno. O resultado da segunda polissonografi a indicou diminuição de 44 para 3 eventos por hora de apnéia e hipopnéia durante o sono, tendo o quadro passado do nível grave a um índice de distúrbio respiratório baixo não mais caracterizando doença apnéica do sono. Conclusão: a fonoterapia foi efi caz para o tratamento deste caso de síndrome da apnéia e hipopnéia do sono. DESCRITORES: INTRODUÇÃOA síndrome da apnéia e hipopnéia obstrutiva do sono (SAHOS) é caracterizada por pausas respiratórias durante o sono, devido a múltiplos colapsos e/ou estreitamentos da via aérea superior, ocasionando queda na saturação do oxigênio sanguíneo e fragmentação do sono. Na apnéia obstrutiva ocorre uma parada total do fl uxo aéreo por um período maior ou igual a 10 segundos, enquanto que na hipopnéia o fl uxo aéreo encontra-se somente diminuído 1 . Durante o sono os músculos da faringe e da língua tendem a se relaxar, assim como toda musculatura do corpo. Na SAHOS esse relaxamento gera uma obstrução da via aérea superior, dificultando o fl uxo aéreo e causando ruídos (ronco) e pausas respiratórias. Ocorre baixa oxigenação pulmonar e cerebral, o que leva o sistema nervoso central a agir em busca da abertura das vias aéreas superiores. Nesse momento o indivíduo tem um forte ronco e um microdespertar, que apesar de inconsciente, é sufi ciente para fragmentar o sono 2 . Na população geral, a prevalência do ronco é maior no sexo masculino; estima-se que 27% dos homens e 18% das mulheres ronquem 3 . Na faixa etária acima de 40 anos nota-se aumento da prevalência do ronco em ambos os sexos, 24% e 36%, respectivamente em homens e mulheres 4 ,
PurposeTo compare visual outcomes and performance between bilateral implantation of a diffractive trifocal intraocular lens (IOL) Acrysof®PanOptix® TFNT00 and blended implantation of two different near add power bifocal IOLs: Acrysof® Restor® SV25T0 in dominant eye and Acrysof® Restor® SN6AD1 in the nondominant eye.MethodsThis prospective, nonrandomized, consecutive and comparative study assessed 20 patients (40 eyes) who had bilateral cataract surgery performed using the IOLs described. Patients were divided into groups, bilateral trifocal implant and blended implant. Evaluation included measurement of binocular uncorrected and corrected distance visual acuity at 4 m (UDVA, CDVA) and uncorrected intermediate (60 cm) and near (at 40 cm) visual acuity; contrast sensitivity (CS) and visual defocus curve.ResultsPostoperative CDVA comparison showed no statistical significance between groups. UDVA was significantly better in the trifocal groups. Under photopic conditions, the trifocal group had better CS in higher frequencies with and without glare. The binocular defocus curve demonstrated a trifocal behavior in both groups, with the bilateral trifocal group exhibiting better performance for intermediate vision.ConclusionBoth lens combinations were able to provide good near, intermediate and distance vision, with the trifocal group showing significantly better performance at intermediate distances and better CS under photopic conditions.
Purpose To compare the anti-inflammatory efficacy of ketorolac of tromethamine 0.4% and nepafenac 0.1% eye drops for prophylaxis of cystoid macular oedema (CME) after small-incision cataract extraction.
Conflict of interest: non-existent diarrhea, malnutrition, respiratory infections, miofunctional orofacial disorders and decreases the risk of allergies, hypertension, high cholesterol rate, diabetes and obesity [1][2][3] .Breastfeeding is a perfect bound between a mother and her child, fulfilling a function of an external "umbilical cord". A breastfeeding woman sees comforted their ability to continue to generate life through the food that she offers 1 . In addition, mothers who breastfeed have a lower chance of developing breast cancer in the future. Breastfeeding may also act as an adjunct in the prevention of a new pregnancy, it has low financial cost, provides satisfaction, and, in advance, creates a motherbaby bound.Currently is recommended exclusive breastfeeding for six months, and maintaining breastfeeding supplemented by two years or more 5 . Despite abundant scientific evidence regarding ABSTRACT Purpose: to determine the influence of received guidance about breastfeeding on the knowledge and practices of mothers attending a university hospital. Methods: descriptive observational study conducted with 250 mothers, aged between 18 and 45 years, who were in the Phonoaudiology Clinic for conducting newborn hearing screening and had their babies no later than six months ago. These mothers, who were previously oriented or not, answered a questionnaire about breastfeeding. Results: mothers who received previous guidance showed greater knowledge about: the appropriate age to offer other foods (p = 0.001), the utensils used for food (p = 0.031), the disadvantage of bottlefeeding (p = 0.037) and pacifier (p = 0.019). Only mothers guided both on pre and post-natal reported using a syringe for feeding (p = 0.045). Moreover, the percentage of mothers who breastfeed on demand and new how to store milk was higher among those who had received guidance on the subject (p <0.001 and p = 0.027). The preparation of the breasts was best performed by mothers not previously guided (p = 0.002). Conclusion: this study showed that receiving guidance on breastfeeding did not determine greater knowledge on the subject, emphasizing the need of having a multidisciplinary team working in guidance of mothers as well as the importance of participation of the phonoaudiologist, who is the qualified professional to address issues of prevention of orofacial disorders.
