Resumo O racismo é um sistema estruturante, gerador de comportamentos, práticas, crenças e preconceitos que fundamentam desigualdades evitáveis e injustas, baseadas na raça ou etnia. Na saúde o racismo pode se manifestar de diversas formas, como o institucional, que frequentemente ocorre de forma implícita. A pandemia do coronavírus tem sido um desafio para países que apresentam profundas desigualdades. No Brasil, em que pese a ausência das informações desagregadas por raça ou etnia ou que quando coletadas apresentam um preenchimento precário, sabe-se que negras e negros irão sofrer mais severamente os impactos da pandemia e seus vários desfechos negativos. No texto recuperamos aspectos históricos e sua relação com as condições de vulnerabilidade da população negra e apresentamos uma agenda de ações específicas para o combate ao racismo e suas devastadoras consequências no contexto da Covid-19.
COVID-19 vaccine hesitancy (VH) has caused concerns due to the possible fluctuations that may occur directly impacting the control of the pandemic. In this study, we aimed to estimate the prevalence and factors associated with COVID-19 VH in Portuguese-speaking countries. We developed a web survey (N:6,843) using an online, structured, and validated questionnaire. We used Measurement Models, Exploratory Factor Analysis, Exploratory Structural Equation Models, and Confirmatory Factor Analysis for the data analysis. The overall prevalence of COVID-19 VH in Portuguese-speaking countries was 21.1%. showed a statistically significant direct effect for VH: vaccine-related conspiracy beliefs (VB) (β = 0.886), perceived stress (PS) (β = 0.313), COVID-19 Misinformation (MIS) (β = 0.259) and individual responses to COVID-19 (CIR) (β = −0.122). The effect of MIS and CIR for VH was greater among men and of PS and VB among women; the effect of PS was greater among the youngest and of VB and CIR among the oldest. No discrepant differences were identified in the analyzed education strata. In conclusion, we found that conspiracy beliefs related to the vaccine strongly influence the decision to hesitate (not to take or to delay the vaccine). Specific characteristics related to gender, age group, social and cognitive vulnerabilities, added to the knowledge acquired, poorly substantiated and/or misrepresented about the COVID-19 vaccine, need to be considered in the planning of vaccination campaigns. It is necessary to respond in a timely, fast, and accurate manner to the challenges posed by vaccine hesitancy.
Context Sarcopenia, besides having an impact on functional capacity, has been associated with increased hospitalization and mortality, and stands out as an essential cause of disability among older people. Objective We conducted a systematic review and meta-analysis of published studies comparing the calories and nutrients ingested by elderly people with and without sarcopenia. Data sources MEDLINE/PubMed, Scopus, LILACS, Cochrane Library, and Scielo databases were searched. Study Selection Studies comparing calories and nutrient intake among elderly people diagnosed with sarcopenia and people without sarcopenia were included. Data Analysis Mean differences and 95% confidence intervals (CIs) were calculated, and heterogeneity was assessed using I2 test. Results A total of 23 studies fulfilled the inclusion criteria. The average number of calories and nutrients ingested were significantly lower in elderly study participants with sarcopenia compared with those without sarcopenia. The meta-analyses showed that the average number of calories ingested (n = 19 studies; mean difference, −156.7 kcal; 95%CI, −194.8 to −118.7) were significantly lower in those with sarcopenia than in elderly participants without sarcopenia. Compared to those without sarcopenia, elderly people with sarcopenia consumed lower amounts of proteins; carbohydrates; saturated fatty acids; vitamins A, B12, C, and D; and minerals such as calcium, magnesium, sodium, and selenium. Conclusions The evidence so far available suggests a difference in caloric, macronutrient (ie, proteins, carbohydrates, saturated fatty acids), and micronutrient (ie calcium, magnesium, sodium, selenium, and vitamins A, B12, C, and D) intake among the elderly with and without sarcopenia. Additional studies are needed to define the best interventions to improve the consumption of calories and nutrients by the aging population.
In the wake of the COVID-19 pandemic, a complex phenomenon called the “infodemic” has emerged, compromising coping with the pandemic. This study aims to estimate the prevalence of agreement with misinformation about COVID-19 and to identify associated factors. A web survey was carried out in Portuguese-speaking countries in two stages: 1. the identification of misinformation circulating in the included countries; 2. a multicentric online survey with residents of the included countries. The outcome of the study was agreement or disagreement with misinformation about COVID-19. Multivariate analyzes were conducted using the Poisson regression model with robust variance, a logarithmic link function, and 95% confidence intervals. The prevalence of agreement with misinformation about COVID-19 was 63.9%. The following factors increased the prevalence of this outcome: having a religious affiliation (aPR: 1454, 95% CI: 1393–1517), having restrictions on leisure (aPR: 1230, 95% CI: 1127–1342), practicing social isolation (aPR: 1073, 95% CI: 1030–1118), not avoiding agglomeration (aPR: 1060, 95% CI: 1005–1117), not seeking/receiving news from scientific sources (aPR: 1153, 95% CI: 1068–1245), seeking/receiving news from three or more non-scientific sources (aPR: 1114, 95% CI: 1049–1182), and giving credibility to news carried by people from social networks (aPR: 1175, 95% CI: 1104–1251). There was a high prevalence of agreement with misinformation about COVID-19. The quality, similarity, uniformity, and acceptance of the contents indicate a concentration of themes that reflect “homemade”, simple, and easy methods to avoid infection by SARS-CoV-2, compromising decision-making and ability to cope with the disease.
Resumo Acumulam-se evidências de que o uso de máscaras é uma medida indispensável de proteção à COVID-19, devido ao alto poder de transmissão do novo coronavírus por via respiratória, inclusive por indivíduos assintomáticos. Por sua vez, o uso das máscaras de tecido em locais públicos tem se consolidado como medida adicional de proteção às medidas de distanciamento social e higienização das mãos. Objetivou-se sistematizar as evidências científicas que justificam o amplo uso de máscaras de tecido como prevenção à COVID-19 e descrever a evolução dos posicionamentos contrários ou favoráveis ao seu uso em ambientes extradomiciliares, diante do avanço da pandemia do novo coronavírus pelo mundo. A triagem de artigos e documentos oficiais do Brasil e de outros países indica que o uso de máscaras em locais públicos tornou-se uma intervenção essencial graças ao potencial de reduzir a velocidade de propagação do novo coronavírus. Recomenda-se que o poder público adote estratégias para aumentar a oferta deste produto e fomente estudos para avaliação do impacto da medida no controle da pandemia no Brasil. É imperativo assegurar disponibilidade de máscaras a grupos socioeconomicamente desfavorecidos e garantir que determinados grupos raciais não sejam estigmatizados diante do uso de máscaras em ambientes extradomiciliares.
Resumo O nascimento prematuro (NP) é uma síndrome resultante de uma complexa relação entre múltiplos fatores que não possuem relações e causalidade totalmente compreendidas. Esse artigo traz uma discussão de um modelo teórico hierarquizado dos determinantes de NP, considerando características maternas como aspectos sociodemográficos, psicossociais, nutricionais, comportamentais e biológicos, tradicionalmente associados ao risco aumentado de NP. As variáveis foram distribuídas em seis dimensões, alocadas em três níveis hierárquicos (distal, intermediário e proximal). Nesse modelo, os determinantes socioeconômicos da mãe, da família, do domicílio e do bairro exercem efeitos indiretos sobre o NP por meio de variáveis no nível intermediário, que por sua vez afetam fatores biológicos de risco no nível proximal, os quais apresentam um efeito direto sobre o NP. O estudo adota um modelo teórico hierarquizado dos fatores envolvidos na cadeia de determinação do NP e suas inter-relações. O entendimento dessas inter-relações é um passo importante na tentativa de interromper a cadeia causal que torna algumas mulheres vulneráveis ao parto prematuro.
