Introduction:The aim of this study was to investigate the knowledge of toxoplasmosis among professionals and pregnant women in the public health services in Paraná, Brazil. Methods: A cross-sectional observational and transversal study of 80 health professionals (44 nurses and 36 physicians) and 330 pregnant women [111 immunoglobulin M (IgM)-and IgG-non-reactive and 219 IgG-reactive] was conducted in 2010. An epidemiological data questionnaire was administered to the professionals and to the pregnant women, and a questionnaire about the clinical aspects and laboratory diagnosis of toxoplasmosis was administered to the professionals. Results: The participants frequently provided correct responses about prophylactic measures. Regarding the clinical and laboratory aspects, the physicians provided more correct responses and discussed toxoplasmosis with the pregnant women. The professionals had diffi culty interpreting the avidity test results, and the physicians stated that they referred pregnant women with high-risk pregnancies to a county reference center. Of the professionals, 53 (91.4%) reported that they instructed women during prenatal care, but only 54 (48.6%) at-risk pregnant women and 99 (45.2%) women who were not at risk reported receiving information about preventive measures. The physicians provided verbal instructions to 120 (78.4%) women, although instructional materials were available in the county. The pregnant women generally lacked knowledge about preventive measures for congenital toxoplasmosis, but the at-risk pregnant women tended to respond correctly. Conclusions: This study provides data to direct public health policies regarding the importance of updating the knowledge of primary care professionals. Mechanisms should be developed to increase public knowledge because prophylactic strategies are important for preventing congenital toxoplasmosis.
Objective: To analyze situations in which premature children are vulnerable in home care, in the first six months after hospital discharge. Method: Qualitative study, from the perspective of philosophical hermeneutics, carried out in a Brazilian city on the border. In-depth interviews were conducted, with a data analysis considering the method of interpretation of meanings. 18 mothers of premature children discharged from a hospital unit participated. 25 home visits and 56 calls were made. Results: The reports from the mothers express situations of vulnerability, concerns, needs for care, singularities of the development of the premature baby, and repercussions of institutional routines in home care. Final considerations: There are vulnerable circumstances in prematurity that reaffirm interconnected individual, social, and institutional dimensions. It is important to highlight that the institutional dimension involves the responsibility of health professionals not to increase individual and social vulnerabilities, but to promote care and seek to reduce situations that generate risks, uncertainties, concerns, and damages.
This study's objective was to analyze Brazilian and international scientific evidence on the reasons people store medication at home and the disposal methods used. This systematic literature review includes studies published from 2001 to 2016 and indexed in PubMed, Lilacs and Elsevier's Scopus Database, using the following key words: Disposal of medicines in household, Disposal of drugs expired, Residential expired drugs, Management of expired medications in household. Thirty-three studies were selected after applying inclusion and exclusion criteria. The studies reveal that the main reasons people keep a stock of medicines at home include the possibility of future use, leftovers from previous treatments, and self-medication. Inappropriate disposal methods prevail, such as throwing medicines in the household trash or flushing them into the sewer. Some countries have adopted programs to educate on proper storage and disposal of medicines, though only one study describes a national program guiding these practices that is free of charge. Evidence shows there is a need to implement public policies intended to provide guidance on the proper storage and disposal of medicines at home.
Objetivo: analisar fatores associados ao preenchimento da caderneta de saúde da criança parafomentar a coordenação do cuidado e acesso à saúde.Método: estudo quantitativo, realizado entre janeiro e junho de 2016, em município paulista.Entrevistadas 284 mães e observadas as cadernetas de seus filhos; considerados significativosresultados com p< 0,05.Resultados: identificou-se baixo preenchimento para pré-natal (5%), dados do recém-nascido(40%), exames de triagem neonatal (10%) e alta hospitalar (6%). Escolaridade materna (p=0,006)foi fator associado ao correto preenchimento de dados do recém-nascido.Conclusão: a caderneta não tem sido fonte de dados para seguimento da atenção ao recém-nascidono município investigado. Favorecer continuidade da atenção após o nascimento e a execução deregistros adequados potencializa a coordenação do cuidado. O preenchimento desse instrumento,incorporado às práticas dos serviços e dos profissionais de saúde em todos os pontos da rede deatenção à criança, traz contribuições ao acesso à saúde.
Objetivo: Apreender o conhecimento e práticas de médicos e enfermeiros para promover o aleitamento materno, após a implementação da Rede Mãe Paranaense. Método: Pesquisa qualitativa, com análise baseada na Fenomenologia Social. Realizaram entrevistas com 35 médicos e 27 enfermeiros atuantes no serviço de atenção primária. Resultados: Os profissionais possuem pouco conhecimento sobre a adesão do aleitamento materno em seus municípios. Suas ações foram pontuais, ocorrendo em algum momento específico do pré-natal, puerpério ou puericultura, sendo realizadas orientações individuais ou palestras. Entretanto, os profissionais esperam que a promoção do aleitamento materno possa reduzir a mortalidade infantil. Considerações finais: O desconhecimento da baixa adesão e a fragmentação das práticas, para promover o aleitamento materno, fragilizam o planejamento de ações integrais para transformar os indicadores de saúde infantil.
Objetivo: avaliar a prevalência do aleitamento materno em municípios da Rede Mãe Paranaense e identificar fatores relacionados a sua prática até o sexto mês de vida da criança. Métodos: estudo exploratório, prospectivo de abordagem quantitativa, com 280 lactantes, realizada entre julho de 2017 e junho de 2018, com aplicação de questionários na maternidade, 24 horas pós-parto e durante visita domiciliar aos 6 meses de vida da criança. Resultados: o aleitamento materno no sexto mês foi 7,9% exclusivo, 38,2% predominante e 30,7% misto. As consultas de puericultura estiveram associadas com o aleitamento materno, e o retorno ao trabalho foi apontado como a principal dificuldade para continuidade da amamentação. Conclusão: a prevalência do aleitamento materno exclusivo está aquém do preconizado, sendo fundamental o planejamento de ações de promoção e proteção à amamentação por meio de uma rede de apoio social, familiar e da equipe multiprofissional.
Objetivo: Identificar evidências científicas que investiguem os cuidados e limitações no manejo do Cateter Central de Inserção Periférica em neonatologia. Método: Revisão integrativa da literatura, realizada na Biblioteca Virtual, em Saúde, considerando as publicações disponíveis no período 2007 a 2016, que retratassem a experiência brasileira. Foram selecionadas 24 publicações. Resultados: Dentre os cuidados para inserção do cateter, o conhecimento teórico-prático foi o de maior relevância, seguido por contenção da dor; orientação e consentimento familiar; escolha do sítio de inserção e mensuração do cateter; uso de barreira máxima e preparo do membro. Os cuidados para manutenção incluíram: educação permanente da equipe; técnicas assépticas para manuseio e realização de curativo; higienização das mãos e lavagem do cateter com solução salina 0,9%. As limitações que se destacaram: infecção, flebite bacteriana; obstrução; ruptura; perda acidental; extravasamento; não progressão; dificuldade de acesso vascular; posicionamento da ponta do cateter; tipo de cateter, flebite química ou mecânica. Conclusão: Aprimorar as ações de enfermagem por meio da educação permanente contribui para que a assistência torne-se efetiva, contínua e proativa. Ressalta-se também a necessidade de estudos com melhores níveis de evidência científica em relação ao manejo do Cateter Central de Inserção Periférica em neonatologia.
Objective: to investigate factors associated with the use and quality of Primary Health Care, as well as the occurrence of pneumonia and diarrhea in children under one year. Method: cross-sectional study conducted with 317 caregivers of 56 Family Health teams from Juazeiro do Norte, Ceará, from August 2014 to October 2015, using the child version of the Primary Care Assessment Tool, child version. Results: in the logistic adjustment, children received assistance (OR: 0.54; 0.32-0.91) and accessibility to the Primary Health Care Unit (OR: 0.49; 0.31-0.76). identified as possible protective factors for episodes of diarrhea. The variables associated with pneumonia were unsatisfactory access (ORaj: 2.13; 95% CI 1.18-3.82), a likely risk factor, and coordination (ORaj: 0.33, 95% CI 0.12-0,94), as a probable protection factor. Conclusion: primary care attendance resulted in a higher chance of avoiding hospitalization for pneumonia, a condition considered sensitive to care in this care model. Complementing family income and children's accessibility to the health unit were protective factors for episodes of diarrhea. The search to ensure the presence and extension of the Primary Health Care attributes has the potential to positively modify the child's health and, therefore, both should be ensured as rights of this population.
