PURPOSE: This review aimed at presenting the benefits regarding the speech perception in noise shown by children who wear hearing aid devices and/or cochlear implants with the Frequency Modulation (FM) System at school. RESEARCH STRATEGY: A bibliographic survey was conducted in an electronic database with standardized search until the year 2012, and a manual search was performed by using specific keywords. SELECTION CRITERIA: For the selection and evaluation of the scientific studies chosen in the search, criteria were established covering the following aspects: type of study, participants, adopted intervention, and evaluation of results. DATA ANALYSIS: The FM system was verified to improve speech perception and speech threshold in noise in all studies. RESULTS: Regarding the performance as to type, the best results were obtained when children used the personal FM system, followed by the table and the sound field systems. CONCLUSION: After extensive review of national and international literature, it was concluded that the studies indicate the need for further research concerning mainly the impact of the FM system on the school performance of children who have sensory devices coupled to the FM system. Findings in the literature with relation to the publications focused on speech perception in noise did not relate educational and auditory aspects.
Purpose: use the International Classification of Functioning, Disability and Health-Children and Youth (ICF-CY) at National Health System services to record the hearing and language development of children in their first year of life. Methods: the community health agents (CHAs) of the Basic Health Unit in the city of Sorocaba, São Paulo, Brazil, were trained to apply a questionnaire to monitor the hearing and language in children in their first year of life assisted by the Family Health Strategy (FHS). Audiologists made the correlation of each of the questions with the codes of the ICF-CY. Results: thirteen CHAs participated in the training program and had higher percentage hits on post-training evaluation, regarding pre-training. Twenty-two children were followed up, of whom 17 evolved as expected, 2 showed improvement in the results, 2 required continuity in their monitoring and for 1, the suspected development risk was maintained. Conclusions: the use of the ICF-CY allowed the expansion of the attention of health professionals and it can be used to monitor child development. The creation / adaptation of standardized assessment tools based on ICF favors its use in the clinical practice.
Purpose: To analyze the phonological and vocabulary performance of children with unilateral hearing loss. Methods: Participants were twelve subjects with unilateral hearing loss of any degree and configuration, with deficit in any ear and without other associated impairments -six with conductive hearing loss due to congenital malformation of outer and/or middle ear, and six with congenital sensorineural hearing loss or acquired in the first year of life. Subjects' ages ranged from 3 years and 7 months to 7 years and 8 months. The phonology and vocabulary tasks of the "ABFW -Child Language Test" were applied. Results: Thirty-three percent of the children presented performances different than expected for their age groups regarding phonological (2 children) and lexical developments (2 children). On the phonology test, one child with sensorineural hearing loss presented phonological processes that were not expected for the age group (liquid simplification and fricative devoicing) and low indices of correct production in the phonetic inventory. One child with conductive hearing loss due to malformation presented stopping and liquid simplification, and borderline results in the analysis of the phonetic inventory (75%). On the vocabulary test, two children with sensorineural unilateral hearing loss presented mean of Usual Verbal Designations below the expected for their ages in different semantic fields. There were no differences between children with sensorineural and conductive hearing loss in the phonology and vocabulary tests. Conclusion: Children with unilateral hearing loss are considered at risk for language development. We emphasize the importance of language and auditory monitoring of these children, providing early and efficient interventions, thus preventing possible language disorders and learning difficulties.
We hereby report on the audiological and genetic findings in individuals from a Brazilian family, with the following mitochondrial mutation A1555G in the 12SrRNA gene (MT-RNR-1). Nine individuals underwent speech, audiologic (tonal audiometry and logoaudiometry) and genetic evaluations. Eight individuals among the A1555G carriers were affected by hearing impairment and one person had normal hearing thresholds till the end of the present study. The audiologic evaluation results indicated normal hearing thresholds all the way to bilateral profound hearing loss with post-lingual onset and variable configuration. Two affected siblings presented progressive hearing loss. It was impossible to precise the time of hearing loss onset; however, the impairment was present in both children and adults. The genetic study revealed the A1555G mitochondrial mutation in the 12SrRNA gene. Given the prevalence of mitochondrial mutations as a cause of hearing loss, it is fundamental to perform the etiopathologic diagnosis in order to postpone the onset or avoid hearing impairment progression. This kind of hearing impairment represents a challenge to the professionals since there are no physical traits that indicate genetic transmission.
