Objective. To describe the prevalence of burnout syndrome among intensive care physicians in Salvador, Brazil, and analyze it for associations with demographic data and aspects of their working conditions (psychological demands and control over tasks).MethOds. This was a cross-sectional study investigating associations between psychosocial aspects of work and professional burnout in a population of 297 intensive care physicians in Salvador. A selfadministered individual questionnaire was used to collect data on psychosocial features of work using the demand-control model (Job Content Questionnaire) and on the mental health of the physicians using the Maslach Burnout Inventory (MBI).Results. An elevated level of working hours overload and on-call work overload was observed. The prevalence of professional burnout was 7.4% and was more strongly associated with the psychological demands of work than with intensive care physician's control over that work. cOnclusiOns. Physicians doing highly demanding work (many demands and little control) suffered 10.2 times more burnout than those with undemanding work (few demands and a high degree of control).

ResumoContexto: Estudos indicam que estudantes de Medicina, apesar do seu dedutível conhecimento sobre os efeitos das substâncias psicoativas, consomem-nas em proporção semelhante à dos jovens de mesma idade na população geral. Objetivo: Analisar o padrão local do consumo dessas substâncias entre graduandos de Medicina e contribuir na formulação de atividades preventivas. Métodos: Aplicação de questionário semi-estruturado no qual foi avaliada a freqüência de uso das substâncias psicoativas entre os estudantes, assim como as principais razões apontadas para o consumo. Foram analisados 404 questionários obtidos entre alunos dos seis anos de curso das duas maiores escolas médicas de Salvador. Resultados: As drogas mais utilizadas no critério uso na vida foram álcool (92,8%) e lança-perfume (46,2%). O uso de álcool apresentou-se constante nos seis anos de curso. Entretanto, o uso de tabaco, lança-perfume e tranqüilizantes aumentou significativamente para os alunos dos últimos anos (p < 0,05). Há um maior consumo de drogas pelo gênero masculino. Diversão foi apontada como razão mais importante para o uso dessas substâncias (58,7% das respostas válidas). Conclusão: O consumo de substâncias psicoativas entre estudantes de Medicina de Salvador é considerado alto, em concordância com o que se constatou em outros trabalhos científicos.Lemos, K.M. et al. / Rev. AbstractBackground: Studies have shown that medical students consume psychoactive substances at the same rates as the same age youth on general population, despite their predictable knowledge about drugs effects. Objective: To analyze the local pattern of psychoactive substances use among medical students and contribute for the formulation of preventive activities. Methods: The frequency of psychoactive substances use was evaluated by a self-administered anonymous questionnaire that also asked the main reasons for that use. Four hundred and four students in the first to sixth year from the two biggest medical schools in Salvador answered the questionnaire. Results: The lifetime use was bigger for alcohol (92.8%) and inhalant (46.2%). Alcohol use was constant for the students from first to sixth year of course, but it was not statistically significant (p > 0.05). However, tobacco, inhalants and tranquilizers use had a significant increase at the last two years of course students (p < 0.05). It was verified a bigger drugs use by the male gender. "Fun" was shown as the first main reason for psychoactive substances (58.7% of the valid answers). Conclusion: Psychoactive substances consume rates by medical students from Salvador are as high as the ones shown on other scientific researches about this subject.

O objetivo deste trabalho é avaliar as características dos Processos Ético-Profissionais (PEP) com denúncia de infração por erro médico e discutir a importância da educação médica na sua prevenção. O principal artigo do Código de Ética Médica que caracteriza o erro médico é o artigo 29. Trata-se de um estudo descritivo em que foi feita uma revisão de todos os PEP julgados no Conselho Regional de Medicina do Estado da Bahia (Cremeb) de 2000 a 2004. Dos 372 médicos processados no Cremeb no período, 42,7% (n = 159) foram denunciados no artigo 29. Destes, a maioria (78,6%) era do gênero masculino, e a idade média era de 44 anos. As especialidades mais freqüentes foram: Ginecologia-Obstetrícia (24,8%), Cirurgia Geral (9,4%) e Anestesia (7,4%). A maioria das denúncias de erro médico se deu em atendimento público (80,1%, n = 109) e relacionada a atos cirúrgicos (66%, n = 97). Foi identificada negligência em 67,3% (n = 107) das denúncias, imprudência em 23,3% (n = 37) e imperícia em 8,8% (n = 14). Apenas 23,9% (n = 38) foram considerados culpados, enquanto 31,4% (n = 50) foram absolvidos por falta de provas e 44% (n = 70) por comprovada inocência. Conclui-se que o erro médico é uma freqüente causa de denúncias contra médicos no Cremeb, sendo a maioria por negligência, o que justifica a necessidade de discutir e valorizar este tema cada vez mais na graduação médica.

RESUMOObjetivo: Transtornos mentais comuns (TMC) possuem alto impacto nos relacionamentos interpessoais e na qualidade de vida, sendo potenciais substratos para o desenvolvimento de transtornos mais graves. Estudantes de medicina vêm sendo apresentados como população de risco para o desenvolvimento de TMC. O objetivo deste estudo é estimar a freqüência de TMC em acadêmicos de medicina da Universidade Federal da Bahia e identificar fatores relacionados. Métodos: Realizou-se estudo transversal entre uma amostra de estudantes de medicina. A identificação dos TMC foi feita por meio do 20-item Self-Report Questionnaire. Resultados: Foram avaliados 223 estudantes. A prevalência de TMC foi de 29,6%, sendo independentemente associada a alterações do padrão do sono, não possuir transporte próprio, não trabalhar e não realizar exercício físico. Conclusões: Estes dados demonstram uma elevada prevalência de TMC na amostra pesquisada e são importantes para subsidiar ações de prevenção de transtornos mentais entre futuros médicos e reflexões sobre o modelo curricular vigente nas escolas médicas. Breast metastases from extramammary cancers are rare and usually related to poor prognosis. The extramammary tumours most frequently exhibiting breast metastases are melanoma, lymphomas, ovarian cancer, lung and neuroendocrine tumours, and sarcomas. Owing to the lack of reliable and specific clinical or radiological signs for the diagnosis of breast metastases, a combination of techniques is needed to differentiate these lesions from primary breast carcinoma or even benign breast lesions. Multiple imaging methods may be used to evaluate these patients, including mammography, ultrasound, MRI, CT and positron emission tomography CT. Clinical and imaging manifestations are varied, depend on the form of dissemination of the disease and may mimic primary benign and malignant breast lesions. Haematologically disseminated metastases often develop as a circumscribed mass, whereas lymphatic dissemination often presents as diffuse breast oedema and skin thickening. Unlike primary carcinomas, breast metastases generally do not have spiculated margins, skin or nipple retraction. Microlobulated or indistinct margins may be present in some cases. Although calcifications are not frequently present in metastatic lesions, they occur more commonly in patients with ovarian cancer. Although rare, secondary malignant neoplasms should be considered in the differential diagnosis of breast lesions, in the appropriate clinical setting. Knowledge of the most common imaging features can help to provide the correct diagnosis and adequate therapeutic planning.

Foram utilizados dois questionários auto-aplicáveis, um para docen ABSTRACT Objectives: To evaluate the interest and knowledge about Medical Ethics and Bioethics in medical graduation. Methods: Transversal and descriptive study. Two different questionnaires were applied with questions about the interest in Medical Ethics and Bioethics and the knowledge about the Brazilian Code of MedicalEthics (CME), one to professors and the other to students. Results: One hundred and one professors and 331 students answered the questionnaires. The Brazilian CME had been read by 86.2% of the teachers and by 100% of the students. The importance given to the discipline Medical Ethics, on a scale from 1 to 5, was similar among teachers and students (4.7 ± 0.7 vs. 4.5 ± 0.8; p = 0.086); however the self-evaluation on knowledge about this subject was higher in the first group (3.4 ± 0.9 vs. 3.2 ± 0.7; p = 0.017). In a block with 9 questions, the right answer was given by 5,0 ± 1.9 of teachers and 5.9 ± 1.5 of students (p < 0,001); the mean of correct answers were related to the reading of the CME. Conclusions: The present study presents unpublished data about the perception of medical teachers and students about medical ethics and bioethics and can be useful for improving the teaching of these disciplines in our medical schools.
