ÂâåäåíèåПереломы проксимального отдела бедренной ко-сти (ПОБК) напрямую связаны с возрастом, часто они происходят в пожилом и старческом возрасте. Со-гласно прогнозам ООН, в 2025 году количество пожи-лых, возраст которых превысит 60 лет, увеличится до 600 млн, что составит более 15 % населения планеты. К 2050 году количество людей старше 65 лет во мно-гих развитых странах удвоится и составит 2 млрд [5, 10]. Поэтому лечение переломов данной локализации продолжает оставаться актуальной проблемой клини-ческой медицины.Переломы ПОБК внезапно приковывают пожилого пациента, имеющего различные хронические заболе-вания, к постели, что приводит к синдрому декомпен-сации всех органов и систем, обусловливая высокую летальность. При этом остается важной проблема вос-становления двигательной активности, возможности самообслуживания, возвращения больных к привыч-ному образу жизни [2, 8].В лечении переломов ПОБК большинство современ-ных травматологов отдают предпочтение оперативному методу [3]. На сегодняшний день существует множество способов хирургического лечения, одним из которых является остеосинтез с использованием экстра-и ин-трамедуллярных конструкций [1, 4].Цель: оценить отдаленные результаты оператив-ного лечения методом накостного остеосинтеза пла-стиной пациентов пожилого и старческого возраста с переломами проксимального отдела бедренной кости. Оперативные методы лечения при всех переломах проксимального отдела бедренной кости (медиальные и латеральные) применены в 36,42 % случаев (257 па-циентов). Ìàòåðèàëû è ìåòîäûПациентам с вертельными переломами в 58 случаях выполнена фиксация перелома при помощи накостно-го остеосинтеза пластиной.Для дальнейшего исследования по возрастным кри-териям и типу перелома была сформирована группа из 24 пациентов, куда вошли больные с переломами типа А (вертельные). Средний возраст пациентов составил у мужчин 71 год, у женщин -74 года.Большинство пациентов (22) доставлены в при-емное отделение бригадами скорой и неотложной помощи. Время с момента травмы до госпитализа-ции составило от 1 часа до 4 суток. Все пациенты имели низкоэнергетическую травму (падение с вы-соты собственного роста). До травмы 13 пациентов передвигались без внешних опор, 11 -использова-ли трость. У всех пациентов выявлена сопутствующая патоло-гия: доминировали заболевания сердечно-сосудистой системы (ишемическая болезнь сердца, гипертониче-ская болезнь), в 16,6 % случаев диагностирован сахар-ный диабет. 66,66 % пациентов имели две сопутствую-щие патологии, 29,16 % -три и более сопутствующих заболевания, что, в свою очередь, способствовало уве-личению периода предоперационной подготовки.Сроки оперативного вмешательства с момента по-ступления в стационар составили от 4 до 19 суток (в среднем -8 суток). ВступЧерез постаріння населення планети відмічає-ться збільшення кількості переломів кісток у людей похилого та старечого віку. За прогнозами фон-ду ООН по народонаселенню UNFPA, чисельність старіших за 60 років людей у 2025 році становити-ме 15 % населення планети (більше 600 млн.), а у 2050 році -2 млрд. осіб [1]. Стрімкими темпами збільшується вікова група людей старіших 80 років, щорічно зростаючи на 4,2 % [2].Особливе місце серед переломів кісток нижніх кінцівок займають переломи проксимального відді-лу стегнової кістки (ПВСК), що становить 17 % від усіх травм опорно-рухового апарату. У 1990 р., за даними ВООЗ, у світі зареєстровано більше 1,3 млн. переломів ПВСК, кількість яких з часом буде збіль-шуватися: в 2025 р. може зрости вдвічі, а в 2050 р. -втричі [3]. У структурі переломів ПВСК вертлюгові та через вертлюгові переломи складають 35-40 % та 5-10 %, відповідно [4,5].Основний контингент хворих з переломами ПВСК -це особи похилого та літнього віку. У 95 % випадків переломи ПВСК є травмами «низької енер-гії», спричиненої, у більшості випадків падінням з висоти власного тіла [6,7]. Відомо, що низькоенерге-тичні переломи нерідко пов'язані з супутніми захво-рюваннями, порушенням ходи, дефіцитом статевих гормонів, недоїданням [8].Важливим аспектом при лікуванні переломів ПВСК є наявність у більшості пацієнтів супутніх захворювань різного ступеня тяжкості, частота яких сягає 60-100 %, включаючи серцево-судинні (90 %) та бронхолегеневі (20-26 %) захворювання. При цьо-му більшість хворих мають два та більше супутні захворювання [9,10], які можуть загостритися у зв'язку з травматичними пошкодженнями і стресом, підвищуючи ризик при оперативному лікуванні та збільшуючи летальність [11].Тяжкість стану хворих, включаючи вибір так-тики лікування, перебіг післяопераційного періоду, тривалість перебування хворого в стаціонарі, багато в чому залежить від супутніх захворювань. Обґрунтування дослідженняВзаємозв'язок віку, супутніх захворювань об-межують та ускладнюють лікувально-діагностичний процес, впливаючи на прогноз лікування, підвищую-чи ймовірність летального результату. Доведено, що з віком погіршується перебіг супутніх захворювань, зростає ризик ускладнень, що суттєво впливає на ви-бір методу оперативного лікування переломів ПВСК [12]. Встановлено наявність супутньої патології у пацієнтів із переломом шийки стегнової кістки на тлі остеопорозу, яким проводили ендопротезування кульшових суглобів [9]. Внаслідок травматичного пошкодження та стресу супутні захворювання мо-жуть загостритися, що істотно підвищує ризики ускладнень, особливо при оперативному лікуванні [13, 14]. Через високу летальність переломи ПВСК вважають найбільш прогностично несприятливими [15]. Аналіз структури супутньої патології у осіб старечого віку з переломами ПВСК дозволить ви-робити алгоритм з прийняття рішення з визначен-ня оптимального методу лікування, що сприятиме підвищенню ефективності лікування та мінімізації несприятливих наслідків.
