Запорожский государственный медицинский университет, Украина Цель работы-установить возможность ассоциации однонуклеотидных полиморфизмов (SNP) генов NFATC1 и NFATC4 с развитием концентрической гипертрофии левого желудочка (ГЛЖ) у подростков 16-18 лет с впервые выявленной первичной артериальной гипертензией (АГ). Материалы и методы. В исследование включены 74 подростка в возрасте 16-18 лет (50 юношей и 24 девушки), у которых впервые в ходе профилактического медицинского осмотра выявлена первичная АГ. У 32 (43 %) подростков при эхокардиоскопии (ЭхоКС) установлены признаки концентрической ГЛЖ. У 42 (57 %) молодых людей с АГ группы контроля показатели ЭхоКС левого желудочка сердца были в пределах возрастной нормы. На следующем этапе исследования проводилось генотипирование с использованием образцов тотальной ДНК, выделенной из цельной венозной крови стандартным методом. Молекулярно-генетическое исследование проводились методом полимеразной цепной реакции (ПЦР) в режиме реального времени. Результаты. Как показали результаты генотипирования и статистический анализ, ассоциация SNP с ГЛЖ выявлена в отношении rs2229309 гена NFATC4 и не подтверждена для SNP (rs7240256, rs11665469, rs754505) гена NFATC1. Таким образом, статистически значимые отличия (р < 0,05) частот встречаемости выявлены для гетерозиготного генотипа rs2229309 гена NFATC4, который почти в 2 раза чаще встречается у подростков с первичной АГ с предрасположенностью к раннему формированию ГЛЖ. Прогностическая чувствительность признака «гетерозиготный генотип CG rs2229309 гена NFATC4» в отношении развития концентрической ГЛЖ сердца у подростков в возрасте 16-18 лет с первичной АГ составила 46 %, специфичность-74 %, точность-62 %. Отношение шансов развития концентрической ГЛЖ сердца у подростков с выявленным признаком составило 2,43 с 95 % доверительным интервалом от 0,78 до 7,58. В этой группе больных также установлено преобладание носителей аллели G.
Актуальність. Дослідження, спрямовані на фенотипування варіацій генів NFATC, можуть бути перспективними для вивчення патогенезу і клінічних проявів артеріальної гіпертензії (АГ) у молодих осіб. Мета дослідження: визначення впливу варіабельності генів NFATC1 і NFATC4 на оцінки якості життя (ЯЖ) відповідно до анкети SF-36 у підлітків з первинною АГ. Матеріали та методи. Об’єкт дослідження — 84 підлітки 16–18 років, студенти медичного університету. На початку дослідження і через 2 роки, по досягненню молодими людьми віку 18 років, проводилося анкетування з використанням опитувальника SF-36. У плані клінічного обстеження використовували офісні вимірювання артеріального тиску (АТ), добовий моніторинг АТ (апарат CardioTens), інструментально-лабораторне тестування з метою виключення симптоматичної АГ, молекулярно-генетичне дослідження експресії генів NFATC1 і NFATC4 і частоти виникнення їх однонуклеотидних поліморфізмів rs7240256, rs11665469, rs754505, rs2229309. Результати. Дослідження виявило підвищену експресію названих генів, яка в 3 і 17 разів відповідно перевищувала референтні рівні. Генотип ТТ rs11665469 гена NFATC1 був асоційований з найнижчими оцінками ЯЖ життя у хворих АГ, які становили лише 75 % від ідеального рівня, за рахунок незадоволеності хворих загальним станом свого здоров’я; рівнем рольового функціонування, обумовленого фізичним станом; рівнем соціального функціонування; наявністю болю; рівнями життєвої активності та психічного здоров’я. Висновки. Встановлено асоціацію генотипу ТТ rs11665469 NFATC1 з низькою самооцінкою ЯЖ його носіїв. Вперше отримані дані свідчать про те, що генотипи rs11665469 NFATC1 впливають на самопочуття хворих молодого віку з АГ і, можливо, беруть участь у формуванні патогенезу АГ.
Своєчасна діагностика та лікування первинної артеріальної гіпертензії (АГ) у молодих осіб має велике значення для запобігання ускладненням захво рювання і передчасній смерті у дорослому віці. Мета: обговорення ефективності програми діагностики, профілактики та лікування первинної АГ у студентів, створеної на основі щорічних профілак тичних медичних оглядів, та подальшого спостереження за участю медичного персоналу кафедр педіатричного та терапевтичного профілю. Матеріали і методи. Щорічно в медичному університеті у студентів першокурсників під час профілактичного огляду вимірюється артеріальний тиск (АТ). Підставою для подальшого медичного обстеження стають значення АТ 120/80 мм рт. ст. і вище. Діагноз первинної АГ підтверджується добовим моніторуванням АТ і поглибленим клініко лабораторним та інструментальним обстеженням. З огляду на те, що суб'єктивні скарги у підлітків з пер винною АГ малоінформативні для діагнозу і прогнозу, для визначення загального стану здоров'я використовували запитальник SF 36. Гіпертрофія лівого шлуночка (ГЛШ) серця є однією з найбільш несприятливих кардіоваскулярних подій у молодих людей з АГ, тому для уточнення предикторів розвитку ГЛШ проводили ехокардіоскопію (Ехо КС), електрокардіографію (ЕКГ) і дослідження поліморфізму (SNP) генів сімейства NFATC. Результати. Діагноз первинної АГ встановлюється щорічно у близько 19% першокурсників. Серед них з АГ виявлено 16,7% осіб з «високими нор мальними цифрами АТ», лабільну АГ (ЛАГ) -у 48,8% підлітків, стабільну АГ (САГ) -у 23,8% осіб, «гіпертензію білого халату» -у 8,3% осіб і маско вану гіпертензію (виявлялася тільки методом ДМАТ) -у 4,8% осіб. Встановлено, що 29,8% підлітків з уперше виявленою АГ мали на ЕКГ позитив ний щодо ГЛШ індекс Соколова-Лайона, і подальше обстеження цих хворих протягом 6-12 місяців виявило її формування. Виявлено асоціацію гено типу СG SNP rs2229309 гена NFATC4 з розвитком ГЛШ. Перераховані ознаки, як прогностично несприятливі, можуть бути використані для своєчасного призначення медикаментозного лікування. Висновки. Для організації медичної допомоги студентам з вперше виявленою первинною АГ необхідно впроваджувати медичний скринінг підвище них цифр АТ і клінічне обстеження з вивченням прогностичних предикторів несприятливого перебігу захворювання. Background. The purpose of the work is to study the blood content of biologically active substances — peptides and hormones that can affect blood pressure and the development of hypertension, depending on the gender and disease course in adolescents aged 17 years by comparing them with healthy individuals. Materials and methods. There were examined 87 adolescents (37 boys and 50 girls) who were divided into 4 main groups with stable and labile hypertension and 2 control groups with normotension. All patients underwent daily blood pressure monitoring and a comprehensive clinical examination, which ruled out symptomatic hypertension. The serum concentrations of biologically active substances were determined, namely endothelin, renin, copeptin, testosterone, estradiol, osteocalcin and protein induced by vitamin K absence (PIVKA-II), by enzyme immunoassay. Results. It was found that the most important feature that distinguishes hypertensive adolescents from normotensive peers was renin, which serum concentrations were almost 1.5 times higher in boys with labile hypertension and twofold — with stable hypertension. Girls with stable hypertension had the same patterns, with a twofold increase in renin level. Regardless of hypertension course, female adolescents had higher endothelin levels. In addition, girls with stable hypertension had high serum content of PIVKA-II. Conclusions. The elevated serum renin level in boys and girls at late stages of puberty as an indicator of renal dysfunction can be considered an essential cause of hypertension development. Determining the importance of renal dysfunction with increased levels of vasopressor hormones directs therapeutic efforts in the treatment of adolescents with hypertension precisely to the correction of this link in the pathogenesis of juvenile hypertension.
Introduction.The priority area in the study of the etiological factors of arterial hypertension (AH) is genetic research. Single nucleotide polymorphisms (SNPs) of genes of the NFATC (nuclear factor of activated Tcells) family do not directly affect blood pressure (BP) in humans but can participate in the formation of an unfavorable course of AH. Among the most important SNPs of the NFATC4 gene, rs2229309 (Gly160Ala), which affects the development of cardiac hypertrophy, can be noted [1, P. 435]. Previously, we have established the prevalence of the occurrence of the G rs2229309 allele of the NFATC4 gene in the genotypes of adolescents with primary AH, with early formation of left ventricular hypertrophy. Further research aimed at phenotyping variations in the NFATC genes could be promising for the future study of the pathogenesis and clinical manifestations of AH in young people.
