Bicuspid aortic valve is one of the most common congenital heart diseases with low manifestation in childhood and severe consequences in adults that determines the importance in early diagnostics of myocardial changes in this anomaly. According to the literature the polymorphisms in the genes of NFATC family could result both in impaired embriogenetic valves formation and development of postnatal myocardial hypertrophy.The aim of the study was to detect the early changes of intracardial hemodynamics at aortic valve in children with bicuspid aortic valve (BAV) and establish their interrelations to the signs of myocardial hypertrophy in these children.Materials and methods: Dopplerograhphic study of basic intracardiac hemodynamics parameters in 38 children with BAV and in 28 children of control group was conducted. The results were processed statistically by Student's t-test, correlation analysis and multiple regression. Results:In the result of study the moderate concentric left ventricle myocardial hypertrophy development was detected in 62 % of children with BAV which is accompanying to significant increasing of blood flow velocity and pressure gradient at aortic valve. There were not established significant correlations between the parameters of hemodynamics at valve and left ventricle's posterior wall depth and septum depth whereas the highest inputs of these values were obtained in the left ventricle systolic dimension and volume and less in the hypertrophic signs. Conclusions:In children with BAV the moderate concentric myocardial hypertrophy with significant changes of intracardial hemodynamics at aortic valve takes place with the highest inputs in left ventricle volumetric values The obtained data serves as a substantiation for the treatment and prevention of it further development.Гіпертрофія міокарда та внутрішньосерцева гемодинаміка в дітей із двостулковим аортальним клапаном серцяДвостулковий аортальний клапан є однією з найпоширеніших вроджених вад серця, котра має малосимптомний перебіг у дитинстві та важкі прояви в дорослому віці, що визначає важливість ранньої діагностики змін у міокарді при цій аномалії. Згідно з науковими літературними даними, поліморфізми генів сімейства NFATC можуть впливати як на ембріогенетичний формування клапанів серця, так і на розвиток постнатальної гіпертрофії міокарда.Мета роботи -визначення ранніх змін внутрішньосерцевої гемодинаміки на аортальному клапані в дітей із двостулковим клапаном серця (ДАК) і встановлення взаємозв'язків цих параметрів із показниками гіпертрофії міокарда.Матеріали та методи. Актуальність. Дослідження, спрямовані на фенотипування варіацій генів NFATC, можуть бути перспективними для вивчення патогенезу і клінічних проявів артеріальної гіпертензії (АГ) у молодих осіб. Мета дослідження: визначення впливу варіабельності генів NFATC1 і NFATC4 на оцінки якості життя (ЯЖ) відповідно до анкети SF-36 у підлітків з первинною АГ. Матеріали та методи. Об’єкт дослідження — 84 підлітки 16–18 років, студенти медичного університету. На початку дослідження і через 2 роки, по досягненню молодими людьми віку 18 років, проводилося анкетування з використанням опитувальника SF-36. У плані клінічного обстеження використовували офісні вимірювання артеріального тиску (АТ), добовий моніторинг АТ (апарат CardioTens), інструментально-лабораторне тестування з метою виключення симптоматичної АГ, молекулярно-генетичне дослідження експресії генів NFATC1 і NFATC4 і частоти виникнення їх однонуклеотидних поліморфізмів rs7240256, rs11665469, rs754505, rs2229309. Результати. Дослідження виявило підвищену експресію названих генів, яка в 3 і 17 разів відповідно перевищувала референтні рівні. Генотип ТТ rs11665469 гена NFATC1 був асоційований з найнижчими оцінками ЯЖ життя у хворих АГ, які становили лише 75 % від ідеального рівня, за рахунок незадоволеності хворих загальним станом свого здоров’я; рівнем рольового функціонування, обумовленого фізичним станом; рівнем соціального функціонування; наявністю болю; рівнями життєвої активності та психічного здоров’я. Висновки. Встановлено асоціацію генотипу ТТ rs11665469 NFATC1 з низькою самооцінкою ЯЖ його носіїв. Вперше отримані дані свідчать про те, що генотипи rs11665469 NFATC1 впливають на самопочуття хворих молодого віку з АГ і, можливо, беруть участь у формуванні патогенезу АГ.
Aim. The nuclear factor of activated T-cells (NFATc 1-4) family gene expression against the background of cardiovascular disorders, accompanying both congenital heart diseases (mainly ones involving valvular defects) and arterial hypertension, is adaptive and stressinduced by the respective hemodynamic stress.Results. In this regard, given the fact that these proteins play a critical role in both formation of the cardiac structures and, at the same time, development of pathological myocardial and vascular wall hypertrophy, they, along with the signaling activating factors, can be regarded as quite promising biological markers for early clinical prediction of development of the cardiovascular disorders in patients with certain congenital heart diseases (especially with valve anomalies) or hypertension.
Запорожский государственный медицинский университет, Украина Цель работы-установить возможность ассоциации однонуклеотидных полиморфизмов (SNP) генов NFATC1 и NFATC4 с развитием концентрической гипертрофии левого желудочка (ГЛЖ) у подростков 16-18 лет с впервые выявленной первичной артериальной гипертензией (АГ). Материалы и методы. В исследование включены 74 подростка в возрасте 16-18 лет (50 юношей и 24 девушки), у которых впервые в ходе профилактического медицинского осмотра выявлена первичная АГ. У 32 (43 %) подростков при эхокардиоскопии (ЭхоКС) установлены признаки концентрической ГЛЖ. У 42 (57 %) молодых людей с АГ группы контроля показатели ЭхоКС левого желудочка сердца были в пределах возрастной нормы. На следующем этапе исследования проводилось генотипирование с использованием образцов тотальной ДНК, выделенной из цельной венозной крови стандартным методом. Молекулярно-генетическое исследование проводились методом полимеразной цепной реакции (ПЦР) в режиме реального времени. Результаты. Как показали результаты генотипирования и статистический анализ, ассоциация SNP с ГЛЖ выявлена в отношении rs2229309 гена NFATC4 и не подтверждена для SNP (rs7240256, rs11665469, rs754505) гена NFATC1. Таким образом, статистически значимые отличия (р < 0,05) частот встречаемости выявлены для гетерозиготного генотипа rs2229309 гена NFATC4, который почти в 2 раза чаще встречается у подростков с первичной АГ с предрасположенностью к раннему формированию ГЛЖ. Прогностическая чувствительность признака «гетерозиготный генотип CG rs2229309 гена NFATC4» в отношении развития концентрической ГЛЖ сердца у подростков в возрасте 16-18 лет с первичной АГ составила 46 %, специфичность-74 %, точность-62 %. Отношение шансов развития концентрической ГЛЖ сердца у подростков с выявленным признаком составило 2,43 с 95 % доверительным интервалом от 0,78 до 7,58. В этой группе больных также установлено преобладание носителей аллели G.
