Шупика (Украина) ³ГУ «Институт педиатрии, акушерства и гинекологии НАМН Украины» (Украина) 4 Межрегиональная академия управления персоналом (Украина) Резюме: Данный обзор посвящен анализу доказательств участия иммунной системы в патогенезе расстройств спектра аутизма у детей. На сегодняшний день продемонстрирована связь аутизма с локусами гистосовместимости, первичными иммунодефицитами, признаками иммунной дизрегуляции, оппортунистическими и условнопатогенными инфекциями, аутоантителами к мозговым антигенам, гиперчувствительностью к пищевым аллергенам, канцерогенезом и плохим профилем переносимости вакцин. Установлена эффективность некоторых иммунотерапевтических и иммуномодулирующих подходов, включая элиминационную диету, в/в иммуноглобулин, препараты глюкортикостероидов и адренокортикотропного гормона, леналидомид, пиоглитазон, трансфер-фактор, пентоксифиллин, трансплантацию стволовых гемопоэтических клеток. Ключевые слова: расстройства спектра аутизма, иммунодефицит, аутоиммунитет, иммунотерапия.Резюме: Даний огляд присвячений аналізу доказів участі імунної системи в патогенезі розладів спектру аутизму у дітей. На сьогоднішній день продемонстрований зв'язок аутизму з локусами гістосумісності, первинними імунодефіцитами, ознаками імунної дизрегуляції, опортуністичними та умовно патогенними інфекціями, автоантитілами до мозкових антигенів, гіперчутливістю до харчових алергенів, канцерогенезом і поганим профілем переносимості вакцин. Встановлено ефективність деяких імунотерапевтичних та імуномодулюючих підходів, включаючи елімінаційну дієту, в/в імуноглобулін, препарати глюкортикостероідів і адренокортикотропного гормону, леналідомід, піоглітазон, трансфер-фактор, пентоксифілін, трансплантацію стовбурових гемопоетичних клітин. Summary: This review analyzes the evidence of immune system involvement in the pathogenesis of autism spectrum disorders in children. To date, demonstrated link autism with MHC loci, primary immune deficiencies, signs of immune dysregulation, opportunistic and streptococcal infections, autoantibodies to brain antigens, hypersensitivity to food allergens, carcinogenesis and poor tolerability profile of the vaccines. Recovery of health in families planning a child and pregnancy support in families at risk of a child with autism are the necessary measures, which, along with the results of researching certain patterns in epigenetic inheritances of autism, lead scientists to conclusions about the possibility to reduce the future birth rate of such children. The results of studying of some of the patterns of epigenetic inheritance of autistic disorders are also lead scientists to those conclusions. A short review of the array of works related to the topic of preventing the birth of children with autism and the results of a study of an Ukrainian medical team that confirms the hypothesis about seasonality in births of autistic children are presented. Seasonality of the birth of children with autism and psychoorganic syndrome was studied, fluctuations in the birth of sick children throughout the year, highly reliable for those with autism (р<0,001). Some measures are offered to reduce the risk of ASD, primarily accompanying the family by a medical psychologist.
