Application of MMC for prevention of excessive scarring after EEDCR is impractical as it is not possible to achieve antifibrotic concentration of the drug at dacryocystorhinostomy ostium site using this method.
Основным источником рецидивов после эндоскопической эндоназальной дакриоцисториностомии (ДЦр) является заращение сформированного искусственного соустья. Однако процессы репарации, протекающие в области дакриостомы после эндоскопической эндоназальной ДЦр, остаются недостаточно изученными. Цель -проведение морфологического анализа процессов репарации в области дакриостомы после эндоскопической эндоназальной ДЦр. материал и методы. в исследование вошли 18 пациенток (18 женщин в возрасте 62,59±10,07 года с облитерацией слезоотводящих путей. всем пациенткам была проведена эндоскопическая эндоназальная ДЦр по модифицированной методике P. Wormald. На 2, 5, 7, 10, 14, 21, 28, 60-е сутки после операции брали биопсию из области дакриостомы. Полученный материал окрашивали гематоксилином и эозином. С целью более детального исследования клеточного состава воспалительного инфильтрата (54 препарата) изучали полутонкие срезы. Полученные гистологические препараты исследовали с помощью фотомикроскопа Leica DM-2500 («Leica», Германия). Фоторегистрацию изображений осуществляли цифровой фотокамерой Leica DFC320 с последующим анализом изображений с помощью программного обеспечения ImageScope Color. Результаты. При гистологическом исследовании выявляли, что в 1-е сутки после операции в исследуемом материале преобладает воспалительный инфильтрат. К 14-м суткам после операции его количество уменьшалось, что свидетельствовало о завершении экссудативной стадии воспаления. Преобладающими клеточными элементами в это время становились активированные фибробласты. К 28-м суткам после операции в биоптатах обнаруживали пролиферирующие фибробласты и коллагеновые волокна. На 60-е сутки после операции отмечали преобладание фибриллярных компонентов над клеточными и их компактизацию. Заключение. результаты исследования показали, что завершение процессов репарации наступает к 60-м суткам после операции. Динамическое морфологическое исследование биоптатов из области, окружающей дакриостому, позволило изучить индивидуальные особенности заживления слизистой оболочки носа и слезного мешка после эндоскопической эндоназальной ДЦр. Lower meniscus morphometry with 'depth' measurement is a comprehensive method to assess the state of lacrimal drainage system. However, it should not be used alone due to significant variations in morphometric parameters, particularly in patients with intensive epiphora.
Introduction. Mitomycin-C is an alkylating antibiotic used to prevent excessive scar formation in the dacryostoma area after dacryocystorhinostomy. In vitro studies proved its inhibition effect on fibroblast growth in 0.2 mg/ml concentration. Up to date, clinical data on its efficacy remain contradictory. Aim. To evaluate the concentration of Mitomycin-C in nasal cavity and lacrimal sac mucosa after topical application and to determine the clinical efficacy of this procedure. Materials and methods. 30 patients with nasolacrimal duct obliteration underwent an endonasal endoscopic dacryocystorhinostomy. At the end of the surgery, at the osteotomy site, a sponge soaked in Mitomycin-C 0,2 mg/ml was applied for 3 minutes. Nasal mucosa biopsy was performed immediately after application, in 30 minutes, and at the 1st day after surgery. Biopsy material analysis of was performed by liquid chromatography - mass spectrometry. The surgical treatment efficacy was established according to proposed efficacy criteria. Results. The analysis of Mitomicyn-C concentrations established them to be: 626 ± 176 ng/g tissue immediately after application, 230 ± 61 ng/g tissue in 30 minutes after application. In 24 hours after surgery, there was no Mitomycin-C in the tissue. Surgical efficacy was 86.7%, recurrences were found in 13.3% of cases. Conclusion. Surgery clinical results coincide with those obtained by other researchers. But chemical investigation showed that Mitomycin-C tissue concentration was lower than that in previous in vitro studies.
The method of injecting MMC during the final stage of EEDCR has proved clinically effective and safe and can be recommended for use in clinical practice.
Цель исследования -разработка метода изучения особенностей строения устья носослезного протока по данным многосрезовой компьютерной томографии (МСКТ).Материал и методы. Изучено строение устья носослезного протока у 65 человек (96 случаев), в том числе у здоровых добровольцев (29 случаев), пациентов с дакриостенозом (45 случаев) и с односторонним дакриоциститом (22 случая). Определение типа устья носослезного протока проводили по разработанной методике на основе данных МСКТ слезоотводящих путей с контрастированием. Для верификации во всех случаях применяли ретроградное зондирование носослезного протока. Результаты. Определено четыре возможных типа строения устья носослезного протока. Несоответствие типа устья носослезного протока по данным МСКТ типу, установленному при ретроградном зондировании, обнаружено в 5 случаях. Точность разработанной методики определения типа строения устья носослезного протока составила 94,8%. Заключение. Разработанная методика, основанная на МСКТ, неинвазивна, позволяет достоверно определить тип строения устья носослезного протока и может быть включена в алгоритм обследования пациентов с нарушениями слезоотведения. To develop a method for investigating the perculiarities of the structure of the nasolacrimal duct orifice according to the data of multislice computed tomography (MSCT).Material and methods. The structure of the nasolacrimal duct orifice was investigated in 96 cases in 65 people, including healthy volunteers (n = 29), patients with dacryostenosis (n = 45), and those with unilateral dacryocystitis (n = 22). The type of the nasolacrimal duct orifice was identified using the developed procedure on the basis of the data of contrast-enhanced MSCT of the lacrimal passages. For verification, the investigators used retrograde probing of the nasolacrimal duct in all cases.Results. There were four possible types of the structure of the nasolacrimal duct orifice. Five cases showed discordance between the type of nasolacrimal duct office and the MSCT type identified by retrograde probing. The accuracy of the developed procedure to identify the type of the nasolacrimal duct orifice was 94.8%. Conclusion.The developed methodology based on CT is noninvasive, can reliably identify the type of the structure of the nasolacrimal duct orifice, and may be included in the algorithm for examination of patients with lacrimation disorders.Index terms: lacrimal passages; opening of the nasolacrimal duct; multislice computed tomography.
