Nowadays, the handling of a chronic disease is seen as a process, influenced by a variety of factors. Case studies, taken from health behavior, help to illustrate the conduct of patients suffering from chronic diseases and develop strategies of how to increase a patient's skills in coping with his illness. The concept of self-efficacy beliefs is of great importance and will be discussed in detail. In addition, possible effects of patient education in the comprehensive treatment of rheumatic diseases will be presented. In teaching methods, equal emphasis is placed on the transmission of information as on the development of practical know-how. They are on a par with emotional support, motivation, assistance in accepting the disease and coping with fear. The special methods and media required will be discussed, along with the framework for training methods and qualification required of the staff in charge of training.

Die Entwicklung und Einf ü hrung von Patientenschulungsprogrammen ist f ü r viele chronische Erkrankungen weit fortgeschritten, jedoch besteht noch Handlungsbedarf hinsichtlich der fl ä chendeckenden Implementierung. Die Gr ü nde f ü r die unbefriedigende Umsetzung wurden bislang nicht untersucht. Im Rahmen einer explorativen Studie wurde der Frage nachgegangen, welche Hindernisse und Probleme auftreten, die eine Umsetzung erschweren oder sogar verhindern. Die Prozessevaluation erfolgte durch leitfadengest ü tzte Befragungen der Mitarbeiter und Dokumentationen in sechs ausgew ä hlten Rehabilitationskliniken. Der Zeitaufwand f ü r die Vorbereitung und Umsetzung der Schulung in einer dreimonatigen Probephase umfasste einen durchschnittlichen Zeitaufwand pro Patient in H ö he von 2,7 Personenstunden. Als Ergebnis der Routinephase wurde ein zus ä tzlicher regelm ä ß iger Zeitaufwand zur Vor-und Nachbereitung bzw. Durchf ü hrung der Schulungsdurchg ä nge von durchschnittlich 1,5 Personenstunden pro Patient ermittelt. Die in den Kliniken aufgetretenen Probleme und Hindernisse unterschieden sich erheblich. Sie waren ü berwiegend in ung ü nstigen Rahmenbedingungen begr ü ndet, doch gab es auch Kliniken, die trotz vergleichbar schlechter Voraussetzungen eine dauerhafte Implementierung erreichten. Insgesamt gelang die Implementierung in den pneumologischen Einrichtungen besser, was sich auch in geringeren Ressourcenverbr ä uchen zeigte. Eine hohe Motivation der Mitarbeiter wie auch eine gewisse Routine im Umgang mit Schulungen k ö nnen dazu beitragen, dass auch bei ung ü nstigen Voraussetzungen eine erfolgreiche Implementierung und Verstetigung gelingt. Overall, the great number of people participating in the program and their acceptance of the booster week are positive. Patients appreciated the group setting and the possibilities of exchanging experience on a high level. But it was difficult to change patients' traditional expectations concerning medical rehabilitation to a behaviour-orientated course like ours. It is worth thinking about whether this concept should play a greater role in traditional medical inpatient rehabilitation programming.
ZusammenfassungDie Rehabilitation von entzündlich rheumatischen Erkrankungen ist eine an den individuellen Bedürfnissen und Krankheitsstadien der Betroffenen angepasste interdisziplinäre Aufgabe. Eine wichtige Rolle spielen dabei die Krankheitsbewältigung und die den Betroffenen zur Verfügung stehenden Kompetenzen im Umgang mit der Erkrankung. Patientenschulung kann Wissen über die Erkrankung transportieren, sie kann Kompetenzen vermitteln und Einstellungen zur Erkrankung beeinflussen und auch verändern. Patientenschulung ist deshalb ein wichtiger Baustein in der medizinischen Rehabilitation. Patientenschulungsprogramme sind nach erwachsenenpädagogischen Gesichtspunkten aufgebaut und können nach standardisierten Vorgaben während der medizinischen Rehabilitation umgesetzt werden. Für Patienten mit chronischen Polyarthritiden und Spondylarthropathien wurden kontrollierte Studien zur Evaluation durchgeführt. Die Ergebnisse zeigen ein günstiges Ergebnis sowohl auf der Wissensebene als auch auf der Einstellungs-und Verhaltensebene. Gesundheitsökonomische Vorteile liegen ebenfalls vor. Trotz der Ergebnisse ist die Umsetzung aus vielfältigen Gründen problematisch. Bisher ist erst ein Drittel aller Patienten, die an einer medizinischen Rehabilitation teilgenommen haben, geschult. It is important how patients cope with the disease and handle acquired techniques and abilities. Patient education imparts knowledge of the disease, improves skills, and influences and changes attitudes. Therefore, patient education is an important building block in medical rehabilitation. Patient education programs are based on educational theories for adults. Under standardized conditions, they can be put into practice during medical rehabilitation. Controlled studies were carried out on patients with arthritis and spondyloarthritis. The results show an increase in knowledge as well as a positive change in attitude and behavior. Economical advantages in health care also occur. Despite results, the clinical implementation is difficult due to diverse causes. So far only a third of all patients took part in an educational program during their medical rehabilitation.
The economic evaluation of a patient education program "ankylosing spondylitis"-based on a randomised controlled prospective multi-center study design-compares program costs and savings in direct and indirect follow-up costs. The collection of health services and cost data was conducted by questioning staff (education programme costs) and patients (follow-up costs). The present study results are: program costs per patient range between 117 euro (10 participants per course) and 186 euro (6 participants per course). Savings in indirect costs (unfitness to work) amount to 2500 euro and thus overcompensate program costs. Currently patient data are verified by sickness fund data and direct follow-up costs and qualitiy of life data are analysed.
