Vaginal wall cysts occur rarely and can cause a variety of symptoms. A 35 year old woman presented with history of mass bulging from vagina for a year. Pelvic examination revealed a reducible non-tender mass protruding from the vagina and diagnosed as rectocele. Physical examination is important for the detection and characterization of vaginal cysts but may be inadequate for appropriate diagnosis.

Müllerian cysts typically are <2 cm in size and asymptomatic. Our patient presented with big mass coming out per vaginum and intermittent vaginal discharge. A provisional diagnosis of first degree uterovaginal descent with cystorectocele with small right lateral vaginal wall cyst was made. The patient was posted for vaginal hysterectomy with anteroposterior colporrhaphy with excision of the cyst. What we thought was a routine vaginal hysterectomy for prolapse allotting another few minutes for a small cyst removal, turned out to be a complicated procedure, requiring additional time and expertize. Not all patients presenting with mass coming out per vaginum, necessarily, are a case of prolapse. One should not minimize the importance of vaginal wall cysts, irrespective of their size. RESUMENSe reportan dos casos de quistes vaginales: se analiza el cuadro clínico y su dificultad diagnóstica. PALABRAS CLAVES: Quistes vaginales SUMMARYWe report two cases of vaginal cysts: clinical presentation and diagnostic difficulties are analysed. KEY WORDS: Vaginal cysts Casos Clínicos INTRODUCCIÓNLos quistes vaginales corresponden a formaciones saculares cubiertas por mucosa vaginal. La presencia de un quiste en la vagina está estimada en un 1%. La mayoría son asintomáticos, benignos y no requieren la escisión (1). El cáncer vaginal da cuenta del 1% de todos los cánceres ginecológicos, por lo que es poco frecuente (2). Existen en la literatura distintas clasificaciones, pero en general se considera en estas su histología, localización y origen. Este último punto merece importancia ya que no todas las lesiones visualizadas en la vagina son provenientes de esta, también existen lesiones que emergen de la uretra o parauretral y de otros tejidos circundantes que se presentan de manera similar. Por esta razón no es fácil realizar un diagnóstico etiológico de certeza.Se exponen dos casos que ilustran la dificultad en el diagnóstico diferencial, utilizando sólo la anamnesis y el examen físico. Caso 1Paciente de 25 años, primigesta, consulta aproximadamente a las 35 semanas de embarazo, por sensación de peso y plenitud vaginal. Al examen físico se evidencia masa de aproximadamente 5 cm que protruye con maniobras de Valsalva. Impresiona clínicamente como quiste parauretral, de paredes lisas, con contenido líquido, no sensible a la palpación y ovalado. Durante el embarazo el tamaño de masa fue fluctuante hasta desaparecer por completo aproximadamente a la semana 37 de embarazo y luego reaparece una semana más tarde. Se realiza cesárea electiva por presentarse como obstrucción del canal del parto. La resección del quiste se realiza en forma electiva al 4º mes de puerperio. En el intraoperatorio se visualiza un quiste de pared vaginal anterior sin compromiso aparente uretral de aproximadamente 10 cm de diámetro, se punciona dando salida a líquido citrino sin signos de infección (Figura 1). Abstract: The timing of onset and general characteristics of prolapse as well as defecography and magnetic resonance imaging can be used to aid the gynecologist in diagnosing vaginal cysts instead of the more common condition of vaginal prolapse.
