KEY WORDS: Vaginal cysts Casos Clínicos INTRODUCCIÓNLos quistes vaginales corresponden a formaciones saculares cubiertas por mucosa vaginal. La presencia de un quiste en la vagina está estimada en un 1%. La mayoría son asintomáticos, benignos y no requieren la escisión (1). El cáncer vaginal da cuenta del 1% de todos los cánceres ginecológicos, por lo que es poco frecuente (2). Existen en la literatura distintas clasificaciones, pero en general se considera en estas su histología, localización y origen. Este último punto merece importancia ya que no todas las lesiones visualizadas en la vagina son provenientes de esta, también existen lesiones que emergen de la uretra o parauretral y de otros tejidos circundantes que se presentan de manera similar. Por esta razón no es fácil realizar un diagnóstico etiológico de certeza.Se exponen dos casos que ilustran la dificultad en el diagnóstico diferencial, utilizando sólo la anamnesis y el examen físico. Caso 1Paciente de 25 años, primigesta, consulta aproximadamente a las 35 semanas de embarazo, por sensación de peso y plenitud vaginal. Al examen físico se evidencia masa de aproximadamente 5 cm que protruye con maniobras de Valsalva. Impresiona clínicamente como quiste parauretral, de paredes lisas, con contenido líquido, no sensible a la palpación y ovalado. Durante el embarazo el tamaño de masa fue fluctuante hasta desaparecer por completo aproximadamente a la semana 37 de embarazo y luego reaparece una semana más tarde. Se realiza cesárea electiva por presentarse como obstrucción del canal del parto. La resección del quiste se realiza en forma electiva al 4º mes de puerperio. En el intraoperatorio se visualiza un quiste de pared vaginal anterior sin compromiso aparente uretral de aproximadamente 10 cm de diámetro, se punciona dando salida a líquido citrino sin signos de infección (Figura 1). KEY WORDS: Stems cells, human embryo, ethic Documentos INTRODUCCIÓNLa investigación sobre células troncales o cé-lulas madres obtenidas de embriones nos tiene inmerso en un debate científico, con implicancias jurídicas y éticas de enormes proporciones, especialmente por la férrea defensa que hacen los que piensan que para alcanzar los beneficios de la medicina reparativa -a la cual todos tenemos derecho-las células troncales adultas no bastan y que sólo las obtenidas de embriones pueden cubrir una amplia gama de actividades curativas sobre enfermedades tan diversas como la diabetes mellitus, el cáncer, ciertas formas de ceguera, los síndromes de inmunodeficiencias, la esclerosis múltiple, el mal de Alzheimer, la enfermedad de Parkinson, la osteoporosis, las enfermedades metabólicas congénitas y muchas otras.La obtención de células madres pluripotenciales humanas, a través de la utilización de embriones, plantea la legitimidad de producir, emplear o modificar formas iniciales de vida humana con fines no reproductivos y enfrenta en forma permanente a aquellos que condenan la producción y utilización de embriones humanos con fines experimentales no reproductivos, porque consideran inmoral la instrumentalización y posterior destrucción de un organismo capacitado para convertirse en un ser humano completo, versus aquellos que defienden la creación y experimentación con embriones en la primera fase de su evolución sobre la base de los beneficios individuales, clínicos, sociales y científicos que pudieran ser obtenidos.Esta polémica, cuando se da en el marco de sociedades democráticas contemporáneas que acogen diversas concepciones de la vida humana en estadios iniciales, está lejos de alcanzar una decisión consensuada, pero no debiera impedir un mayor conocimiento de los detalles que rodean estos experimentos o la búsqueda de nuevas formas de regulación que entorpezcan éstas y otras investigaciones afines.A priori, no debieran existir razones intrínsecas a la creación de embriones humanos que justifiquen su prohibición, aunque muchos comités asesores de ética para la investigación científica y
RESUMENLa endometriosis es una causa importante de dolor pélvico e infertilidad en las mujeres premenopáusicas. Aunque poco se conoce sobre su etiopatogenia, se considera como un trastorno multifactorial donde se conjugan elementos endocrinológicos, inmunológicos, ambientales y genéticos. El estudio de genes candidatos no ha sido exitoso en la ubicación de genes de susceptibilidad. Se reporta una familia con tres hermanas afectadas de endometriosis, se comenta su evolución y posibles implicancias genéticas. KEY WORDS: Endometriosis, genetic Casos Clínicos INTRODUCCIÓNLa endometriosis consiste en la presencia y crecimiento de tejido endometrial en una ubicación aberrante, fuera de la cavidad uterina. Es un trastorno ginecológico relativamente común; se describe una prevalencia de 33% en mujeres con dolor pélvico crónico y una incidencia entre 30-45% en mujeres con infertilidad (1). El dolor pélvico crónico femenino se presenta con una prevalencia de 3,8%; generalmente la etiología no es clara, y se asocia a múltiples enfermedades, jugando la endometriosis un factor protagónico, con una asociación de más de 60% (2).Afecta a mujeres en etapa de reproducción, limita la fertilidad y puede producir serios daños a la salud, aunque raras veces es causa de muerte. En parejas normales, la fecundidad mensual está en rangos de 0,15 a 0,20 y disminuye progresivamente con la edad. En mujeres con endometriosis e infertilidad, la fecundidad mensual está en rangos de 0,02 a 0,10. Estudios previos demostraban que el 25-50% de mujeres infértiles tenían endometriosis y que 30 a 50% de mujeres con endometriosis eran infértiles. Existe una mayor prevalencia de endometriosis en mujeres infértiles (48%) comparadas con mujeres fértiles que se someten a esterilización tubaria (5%) (3). Otros estudios han demostrado que la mujer infértil tiene 6 a 8 veces más probabilidad de tener endometriosis que la mujer fértil. El embarazo es un estado de hipercoagulabilidad en el que se puede incrementar hasta en cinco veces el riesgo de eventos tromboembólicos (2). Los cambios en el sistema de coagulación comienzan en el momento de la concepción y sólo terminan 8 semanas postparto (3).En mujeres con trombofilia, el riesgo durante el embarazo se incrementa hasta el doble de un embarazo normal (4). También poseen riesgo aumentado de desprendimiento placentario, preeclampsia, restricción del crecimiento intrauterino, muerte fetal in útero y probablemente aborto (5).La terapia anticoagulante se indica durante el embarazo para la prevención y tratamiento del embolismo sistémico en pacientes portadoras de válvulas cardiacas mecánicas y valvulopatías con fibrilación auricular (6.) Habitualmente también, para la prevención de complicaciones durante el embarazo en mujeres con anticuerpos antifosfolípidos y otras trombofilias que han presentado complicaciones con anterioridad (7).El objetivo de esta comunicación es presentar los aspectos básicos del tratamiento anticoagulante durante el embarazo. Indicaciones de anticoagulaciónA pesar del aumento del riesgo de trombosis durante el embarazo y posparto, la mayoría de las mujeres no requieren anticoagulación. Aunque algunos expertos recomendarían tromboprofilaxis en todas las embarazadas con trombofilia hereditaria, la anticoagulación no es necesaria si no existe historia personal de tromboembolismo o una mala historia obstétrica (7). Las excepciones son las mujeres con deficiencia de antitrombina, mutación homocigota para el factor V de Leiden, mutación homocigota para el gen de protrombina G20210A o heterocigota para ambas mutaciones (heterocigota compuesta) (8). We discuss this uncommon complication, and an actualized review is maked.KEY WORDS: Preeclampsia, eclampsia, cortical blindness Casos Clínicos INTRODUCCIÓNLa preeclampsia (PE) es una patología partíci-pe del síndrome hipertensivo del embarazo y afecta al 5 a 8% de las gestantes. Conjuntamente con el aborto séptico, la PE es la primera causa de mortalidad materna en nuestro país (1), por lo que tiene gran impacto en la morbimortalidad del binomio madre-hijo (2, 3, 4).El desorden comienza con un gatillo placentario, seguido por una respuesta sistémica materna. La eclampsia es una forma severa de PE donde la magnitud del alza tensional, el daño endotelial o el vaso espasmo provocan un cuadro convulsivo y/o coma en ausencia de patología neurológica previa. Afecta principalmente al sistema vascular, hepáti-co, renal, cerebral, y a la coagulación. La alteración más común en el cerebro es el edema (5, 6). En mujeres eclámpticas se han encontrado como hallazgos post mortem: hemorragias cerebrales, petequias, necrosis fibrinoide, daño vascular y micro infartos (7).El objetivo de esta comunicación es presentar una complicación excepcional de la PE como es la amaurosis bilateral cortical. CASO CLÍNICOPrimigesta de 24, que cursa un embarazo de 36 +4 semanas de amenorrea. Sin antecedentes mórbidos de importancia. El control prenatal registra cifras tensionales normales previo a los últimos tres controles obstétricos donde se observa cifras de 130/80. El día previo a su ingreso, acusa visión borrosa de 10 minutos de duración, cediendo espontáneamente. El día de su ingreso debuta con amaurosis bilateral, síndrome vertiginoso y cefalea, por lo que consulta en el Servicio de Urgencia del Hospital Clínico de la Universidad de Chile.
