A pproximately 0.5-2% of all pregnant women undergo non-obstetric surgery during gestation.1 Operations during pregnancy may be related to pregnancy such as cerclage or unrelated to pregnancy such as cholecystectomy or trauma. The type of anesthetic technique is important in all trimesters of pregnancy due to the maternal and neonatal side effects. Especially, the organogenesis period (from 18 to 58 days after conception) has maximum sensitivity to teratogens and anesthetic agents. A AB BS ST TR RA AC CT T Both anesthetic techniques and surgery in the first trimester can adversely affect fetal development during pregnancy and may cause premature birth, fetal abnormalities, spontaneous abortion and lower birth weight. Especially, the organogenesis period (from 18 to 58 days after conception) has maximum sensitivity to teratogens and anesthetic agents. A 26-year-old, previously healthy female was admitted to the Emergency Department with shaft fracture of the left tibia and fibula. Eight weeks of gestation was defined via obstetrics ultrasound. Using combined spinal-epidural anesthesia with 2% lidocaine in a graded fashion provided safe anaesthesia for both the mother and the developing fetus. This case presents the anesthetic approach in a patient with first trimester pregnancy that underwent a surgery due to left tibia and fibula fractures. If non-obstetric surgery is inevitable in the first trimester, the safest anesthesia for both the mother and the baby must be preferred.K Ke ey y W Wo or rd ds s: : Pregnancy; anesthesia; tibial fractures; surgery Ö ÖZ ZE ET T İlk trimesterde yapılan ameliyat ve kullanılan anestezi teknikleri, gebelik süresince fetüsün gelişimini olumsuz etkileyebilir ve erken doğum, fetal anormallikler, spontan abortus ve düşük doğum ağırlığına sebep olabilir. Özellikle, organegenezis dönemi (konsepsiyondan sonraki 18'den 58. güne kadar olan dönem) teratojenlere ve anestezik ajanlara en fazla duyarlılığa sahiptir. Daha önceden sağlıklı olan 26 yaşındaki kadın hasta sol tibia ve fibula kırığı ile Acil Servis'e müracaat etti. Obstetrik ultrason ile 8 haftalık gebelik tespit edildi. Kombine spinal-epidural anestezi kullanılarak %2 lidokain solusyonunun kademeli şekilde uygulanması ile anne ve gelişmekte olan fetüs için gü-venli anestezi sağlandı. Bu vaka raporunda sol tibia ve fibula kırığı sebebi ile ameliyat geçiren ilk trimester gebeliği olan bir hastadaki anestezi yaklaşımı sunulmuştur. IntroduçãoO termo doença falciforme engloba um grupo de anemias hemolíticas hereditárias que têm em comum a presença de hemoglobina S dentro da hemácia. Representa a enfermidade hereditária mais prevalente no mundo.1,2 Está presente, notadamente, na África tropical e em todos os países em que existe a contribuição do africano na formação étnica da população. Na Europa, está limitada à comunidade africana das antigas colônias, sendo encontrada ainda em algumas regiões da Itália onde mapeia as correntes migratórias da região de Benin, na África. Sua prevalência em vários paí-ses é estimada prospectivamente pelos resultados de programas de triagem neonatal. 3,4 Por causa da grande miscigenação, no Brasil a prevalência é fortemente relacionada ao percentual de afro-descendentes em cada região e varia de um caso novo para cada quinhentos nascidos vivos (Bahia) até um caso novo para cada 8 mil nascidos vivos (Rio Grande do Sul). Os estados do Rio de Janeiro e Minas Gerais apresentam números intermediários com um caso novo para cada 1.200 nascimentos. 5A hemoglobina S, quando desoxigenada, sofre interações hidrofóbicas, o que compromete a solubilidade e acarreta formação de um polímero de moléculas de hemoglobina, o que resulta em dano celular irreversível, traduzido clinicamente pelo surgimento dos sinais e sintomas da doença.A dor é o resultado da obstrução da microcirculação causada pelo afoiçamento das hemácias. Este é o mais dramático quadro da doença, pois as crises álgicas ocorrem inesperadamente, muitas vezes sem pródromos e impactam diretamente a qualidade de vida do paciente. A crise dolorosa ocorre, às vezes, após episódio infeccioso, sugerindo que febre, desidratação e acidose podem desencadear a vasoclusão. A dor também pode se instalar após o resfriamento súbito da pele ou exposição à estresse físico ou emocional. 6A subjetividade da dor representa um fator complicante para o seu tratamento, no entanto a já comprovada correlação entre a dor persistente e o maior risco de morte súbita justifica o destaque do tema para uma análise mais pormenorizada, focalizando os fatores envolvidos na sua fisiopatologia e, sobretudo, no tratamento a ser adotado.

Medications Used by Pregnant Women: Any Safety Concerns?

Objective: The use of medications during pregnancy may impose a potential risk to the fetus. Therefore, this study was conducted to determine the prevalence and safety of medications used by pregnant women. Methods:A cross-sectional study was conducted on pregnant women who attended the antenatal clinic of a major teaching hospital in Malaysia, from January to April 2013. Data was collected via face-to-face interviews using a structured questionnaire. Results:Of the 500 respondents in this study, 62% (95% confidence interval, CI: 57.7%, 66.3%) used at least one medication during their pregnancy and 30.8% (95% CI: 26.8%, 34.8%) took the medications during the first trimester. The classes of medications commonly used were analgesics (26.8%), followed by cough and cold medications (18.6%) and medications for gastrointestinal disorders (11.8%). Among the 697 medications used by the respondents during pregnancy, most of them were relatively safe, with approximately 50% being classified under the Food and Drug Administration of the United States (FDA, US)-assigned pregnancy category B. However, seven potentially teratogenic medications were identified, including four antiepileptic drugs and an antimigraine medication which contained ergotamine. Conclusion:This study found that a high proportion of pregnant woman consumed at least one medication during their pregnancy. The use of antimigraine medications containing ergotamine in pregnant women is of major concern as this medication could be obtained without a prescription in some countries, including in Malaysia. Therefore, it is essential for health care professionals to educate and counsel pregnant women on the safety of medications used.

Treatment of Pain in Pregnancy

Abstract: Experience in women seeking recovery from opioids and their newborns illustrates that opioids are an effective and safe pharmacological option for the treatment of pain during pregnancy. Controlled studies are needed to expand knowledge in this clinical area.
