Children with sickle cell anemia (SCA) and conditional transcranial Doppler (TCD) ultrasound velocities (170-199 cm/sec) may develop stroke. However, with limited available clinical data, the current standard of care for conditional TCD velocities is observation. The efficacy of hydroxyurea in preventing conversion from conditional to abnormal TCD (≥200 cm/sec), which confers a higher stroke risk, has not been studied prospectively in a randomized trial. Sparing Conversion to Abnormal TCD Elevation (SCATE #NCT01531387) was an NHLBI-funded Phase III multicenter international clinical trial comparing alternative therapy (hydroxyurea) to standard care (observation) to prevent conversion from conditional to abnormal TCD velocity in children with SCA. SCATE enrolled 38 children from the United States, Jamaica, and Brazil [HbSS (36), HbSβ0-thalassemia (1), and HbSD (1), median age 5.4 years (range, 2.7-9.8)]. Due to slow patient accrual and administrative delays, SCATE was terminated early. In an intention-to-treat analysis, the cumulative incidence of abnormal conversion was 9% (95% CI 0 to 35%) in the hydroxyurea arm and 47% (95% CI 6 to 81%) in observation arm at 15 months (p=0.16). In post-hoc analysis according to treatment received, significantly fewer children on hydroxyurea converted to abnormal TCD velocities, compared to observation (0% versus 50%, p=0.02). After a mean of 10.1 months, a significant change in mean TCD velocity was observed with hydroxyurea treatment (−15.5 versus +10.2 cm/sec, p=0.02). No stroke events occurred in either arm. Hydroxyurea reduces TCD velocities in children with SCA and conditional velocities.
IntroduçãoO termo doença falciforme engloba um grupo de anemias hemolíticas hereditárias que têm em comum a presença de hemoglobina S dentro da hemácia. Representa a enfermidade hereditária mais prevalente no mundo.1,2 Está presente, notadamente, na África tropical e em todos os países em que existe a contribuição do africano na formação étnica da população. Na Europa, está limitada à comunidade africana das antigas colônias, sendo encontrada ainda em algumas regiões da Itália onde mapeia as correntes migratórias da região de Benin, na África. Sua prevalência em vários paí-ses é estimada prospectivamente pelos resultados de programas de triagem neonatal. 3,4 Por causa da grande miscigenação, no Brasil a prevalência é fortemente relacionada ao percentual de afro-descendentes em cada região e varia de um caso novo para cada quinhentos nascidos vivos (Bahia) até um caso novo para cada 8 mil nascidos vivos (Rio Grande do Sul). Os estados do Rio de Janeiro e Minas Gerais apresentam números intermediários com um caso novo para cada 1.200 nascimentos.
5A hemoglobina S, quando desoxigenada, sofre interações hidrofóbicas, o que compromete a solubilidade e acarreta formação de um polímero de moléculas de hemoglobina, o que resulta em dano celular irreversível, traduzido clinicamente pelo surgimento dos sinais e sintomas da doença.A dor é o resultado da obstrução da microcirculação causada pelo afoiçamento das hemácias. Este é o mais dramático quadro da doença, pois as crises álgicas ocorrem inesperadamente, muitas vezes sem pródromos e impactam diretamente a qualidade de vida do paciente. A crise dolorosa ocorre, às vezes, após episódio infeccioso, sugerindo que febre, desidratação e acidose podem desencadear a vasoclusão. A dor também pode se instalar após o resfriamento súbito da pele ou exposição à estresse físico ou emocional.
6A subjetividade da dor representa um fator complicante para o seu tratamento, no entanto a já comprovada correlação entre a dor persistente e o maior risco de morte súbita justifica o destaque do tema para uma análise mais pormenorizada, focalizando os fatores envolvidos na sua fisiopatologia e, sobretudo, no tratamento a ser adotado.
Situação atual da doença falciforme Anemia falciforme (AF) é a doença hereditária monogênica mais comum do Brasil, ocorrendo predominantemente entre afrodescendentes. Distribui-se heterogeneamente, sendo mais frequente nos estados Norte e Nordeste. Estimase que cerca de 4% da população brasileira e 6% a 10% dos afrodescendentes são portadores do traço falciforme (Hb AS) e que, anualmente, nascem aproximadamente 3 mil crianças portadoras de doença falciforme (DF), número este que corresponde ao nascimento de uma criança doente para cada mil recém-nascidos vivos.
