Introduction: We sought to prospectively assess anxiety, pain, and embarrassment associated with diagnostic cystoscopy and multichannel urodynamic study (UDS). Methods: All consecutive patients undergoing diagnostic cystoscopy or UDS in our department over a period of nine months were asked to participate. Two anonymous auto-administered questionnaires were specifically designed to collect basic epidemiological data, document medical history, and assess the quality of information provided, along with prevalence and level (0-10 numerical visual analog rating scale) of anxiety, pain, and embarrassment experienced before and/or during the procedures. Statistical analysis was carried out to identify underlying factors that could have influenced patients' experience and ascertain potential correlations between anxiety, pain, and embarrassment. Results: 101 and 185 patients were respectively evaluated immediately after cystoscopy and UDS. Multivariate analysis repeatedly showed statistical correlations between anxiety, pain, and embarrassment, with regard to prevalence and level of intensity in both cystoscopy and UDS populations. Males and young patients were more likely to present anxiety, pain, or embarrassment during cystoscopy and UDS. Interestingly, patients who reported having received complete information before cystoscopy were significantly more likely to experience anxiety (62.6% vs. 20.0%; p=0.009). Conclusions:The present study demonstrated the major impact of gender and age on patients' experience. Interestingly, information provided before cystoscopy was reported to have a negative impact on patients' perception of anxiety; this could be partly prevented by optimizing the way information is provided to patients. Cystoscopy is part of the evaluation process in patients complaining of lower urinary tract symptoms (LUTS) and is undertaken repeatedly in patients being monitored for non-invasive bladder tumours. This procedure allows direct visual examination of the bladder cavity and wall to detect abnormalities, such as diverticula, stones, inflammation, and tumours. INTRODUÇÃOOs sintomas do trato urinário inferior são freqüentes nos consultórios de Ginecologia e, algumas vezes, conduzidos de forma simples e mal investigados 1 . Uma anamnese bem efetuada associada a um exame físico minucioso muitas vezes é capaz de fornecer o diagnóstico preciso e orientar o tratamento correto 2 . Entretanto, existem alguns sintomas urinários similares, que apresentam etiologia diferente e, nestes casos, uma propedêutica subsidiária específica torna-se fundamental 3-5 . A avaliação urodinâmica é capaz de identificar as causas especí-ficas dos sintomas urinários e de fornecer dados para orientar o tratamento correto 1,5 . Em se tratando do tratamento cirúrgico para a incontinência urinária, este diagnóstico preciso é fundamental, tendo em vista que a principal e melhor cirurgia deve ser a primeira, e que a taxa de cura declina mais ou menos proporcionalmente ao número de cirurgias subseqüentes . Entretanto, ainda existe uma minoria de pacientes que considera o teste embaraçoso, doloroso e estressante 10 . Um melhor conhecimento da expectativa com relação ao EUD, bem como do desconforto físico e psíquico causado pelo exame, pode contribuir para uma orientação mais adequada dos pacientes com sintomas urinários. Aims: Urodynamic studies (UDS) are complex, invasive and may be emotionally and physically distressing. Inadequate information may heighten distress. Prior to UDS, our patients receive written and verbal information, yet concerns remains that some women may not be adequately informed. This study examined how informed women believe they are to undergo UDS in our clinic. Methods: This was a before-after questionnaire-based study. Data were collected from a convenience sample of 200 women prior to UD testing, and within 3 days of completion. Participants rated their level of agreement/disagreement with the following statements: I understand why I am having the test; I have been given enough information about the test; I understand what the test will involve. Women also reported their perceived levels of distress (anxiety, embarrassment, and discomfort). Results: One hundred fifty-one women returned both pre-and post-UD questionnaires. Most(88.9%)had read the information sheet andagreed thattheyunderstood theindications forthetesting (78.1%). Fewer agreed that they understood what the tests would involve (68.2%), or that they had been given enough information about the test (64.9%). The overall distress score (anxiety, embarrassment, and discomfort) was 6 (IQR 5-7; possible range 3-9). Conclusions: Urodynamic testing remains an invasive yet prevalent form of urogynecological assessment. One-third of our patients may not understand what UD testing involves and may not feel adequately informed about the tests. Additional research to explore informational needs is important.
