INTRODUÇÃOOs sintomas do trato urinário inferior são freqüentes nos consultórios de Ginecologia e, algumas vezes, conduzidos de forma simples e mal investigados 1 . Uma anamnese bem efetuada associada a um exame físico minucioso muitas vezes é capaz de fornecer o diagnóstico preciso e orientar o tratamento correto 2 . Entretanto, existem alguns sintomas urinários similares, que apresentam etiologia diferente e, nestes casos, uma propedêutica subsidiária específica torna-se fundamental 3-5 . A avaliação urodinâmica é capaz de identificar as causas especí-ficas dos sintomas urinários e de fornecer dados para orientar o tratamento correto 1,5 . Em se tratando do tratamento cirúrgico para a incontinência urinária, este diagnóstico preciso é fundamental, tendo em vista que a principal e melhor cirurgia deve ser a primeira, e que a taxa de cura declina mais ou menos proporcionalmente ao número de cirurgias subseqüentes . Entretanto, ainda existe uma minoria de pacientes que considera o teste embaraçoso, doloroso e estressante 10 . Um melhor conhecimento da expectativa com relação ao EUD, bem como do desconforto físico e psíquico causado pelo exame, pode contribuir para uma orientação mais adequada dos pacientes com sintomas urinários. MÉTODOSAs participantes do estudo foram atendidas no setor de Uroginecologia e Cirurgia Vaginal da Unifesp/EPM, durante o período de julho a dezembro de 2003. Todas apresentavam sintomas do trato urinário inferior e necessitavam de avaliação urodinâmica.Adotamos como critérios de inclusão idade superior a 18 anos, nenhuma experiência prévia com o exame urodinâmico e capacidade de entender e completar um questionário.Os critérios de não inclusão foram pacientes com amostra de urina não estéril e incapacidade para cooperar durante o exame.Na primeira consulta, após a anamnese e exame físico, o estudo urodinâmico era explicado verbalmente à paciente e quaisquer dúvidas eram sanadas. Caso a paciente aceitasse participar da
adequado controle de qualidade. Pacientes com menos de 35 anos corresponderam a 72,6% dos casos. Um grande número não tinha queixas clínicas (36,5%). Nos casos em que foi realizada colposcopia (n = 58), a zona de transformação atípica foi observada em 60%. A subclassificação das ASCUS em provavelmente displásico (D), provavelmente reativo (R) e não-determinado (U) indicou predominância do primeiro (65%). O acompanhamento de 86 pacientes mostrou que, após 3 a 6 meses (média de 4,5 meses), em 12,5% foi possível detectar citologicamente uma lesão intra-epitelial escamosa. Conclusão: com base nos resultados deste trabalho foi possível concluir que as ASCUS incidem em mulheres jovens, com queixas clínicas corriqueiras e apresentam correlação colposcópica positiva. Nestes casos o acompanhamento citológico se faz imprescindível para esclarecimento de lesão intra-epitelial escamosa subjacente ou subseqüente.
Os autores apresentam um caso de uma paciente de 44 anos , tabagista, com dermatose disseminada diagnosticada como acantose nigricante. As lesões da pele levaram ao diagnostico de metátase de carcinoma provavelmente de origem pulmonar, localizado na região laringofaringea, até então assintomatico.
