The outcome of retained instrument removal in a specialist practice

Abstract: The position of the instrument within the root canal, the angle of the curvature of the root canal and the location of the fractured instrument in relation to the root canal curvature were the decisive factors that had a negative influence on the treatment outcome. The removal method tested represents a highly effective technique for the retrieval of fractured instruments.

“…1,3,6,8 Orthograde retrieval is a time consuming and often a difficult procedure. Many orthograde techniques such as grasping with hemostats, a tube and Hedstrom file, wire loops, and devices such as the Masserann Kit, Instrument Removal System (IRS), Endo Extractor, Endo Safety System, and ultrasonics have been described for fragment retrieval.…”

“…1,6,7 The removal of an instrument fragment is influenced by the anatomy of the root canal, the length of the separated fragment, and the remaining tooth structure. 1,5,[7][8][9] A variety of devices and techniques to remove fractured instruments have been described, including drills, extractors, ultrasonic tips, dental operating microscopes, and electrochemical processes. 5,8,10,11 These techniques have shown encouraging results.…”

“…The use of the operating microscope as adjunct to these procedures increases the chances of intracanal instrument removal (10,11). However, unsuccessful attempts at removal of a fragment can affect the root dentin integrity, especially in the apical third of the canal (3,9,10).…”

“…Vertical root fracture and crack formation can be seen in root dentin during and after endodontic procedures, and is thus a complication of root canal treatment, often leading to tooth extraction [2][3][4][5].…”
