Im Rahmen der Wurzelkanalaufbereitung kann es zu Frakturen der eingesetzten Instrumente kommen. Obwohl der Instrumentenbruch bei der Behandlung eines engen, gekrümmten Kanals immer in Betracht gezogen werden muss, wird er vom Behandler selbst als großes Ärgernis wahrgenommen. Grossman bemerkte hierzu Folgendes: "Ein Zahnarzt, welchem noch nie die Spitze eines Reamers, einer Feile oder einer Exstirpationsnadel gebrochen ist, hat noch nicht viele Wurzelkanäle behandelt. Wer hat noch nicht den plötzlichen Schmerz, die Pein und Demütigung durch die Fraktur eines Instruments gespürt? Das Gefühl der Reue hält tagelang an, nur abgeschwächt durch den Lauf der Zeit." [1]. In erster Linie jedoch stellt ein Fragment in dem behandelten Wurzelkanalsystem ein Hindernis für dessen vollständige Erschließbarkeit dar und stellt somit den Erfolg einer endodontischen Therapie infrage. Die generelle Überlegung in Bezug auf die Entfernung eines solchen Hindernisses basiert auf den grundlegenden Prinzipien und Zielen einer Wurzelkanalbehandlung: Infektionskontrolle innerhalb des komplexen Endodonts durch vollständiges Erschließen der von Wurzel zu Wurzel variierenden Kanalstrukturen und Hohlräume, mechanische Formgebung und Vergröße-rung der zu instrumentierenden Kanalabschnitte ohne Verlegung des ursprünglichen Kanalverlaufs. Ziel hierbei ist die Schaffung eines optimalen Zugangs zur Spü-lung mittels antimikrobiell wirksamer und gewebeauflösender Spüllösungen zur maximalen Desinfektion des Endodonts. Die Verhinderung einer Reinfektion erfolgt im Anschluss daran mittels eines dichten Verschlusses des Wurzelkanalsystems nach apikal und koronal. Die mangelnde oder unmögliche Instrumentierbarkeit und die somit der Desinfektionslösung und dem Verschlussmaterial nur begrenzt oder mitunter überhaupt nicht zugänglichen infizierten Kanalabschnitte stellen hierbei offensichtlich die eigentliche Ursache für den Misserfolg der endodontischen Therapie dar. Dies gilt insbesondere für die von einer Instrumentenfraktur betroffenen Zähne, bei denen das Instrument nicht oder nur teilweise entfernt werden konnte [2]. Der Entfernung des Instrumentenfragments scheint also eine entscheidende Rolle bei der Behandlung der o. g. Problematik zuzukommen, weshalb dieser hohe Priorität eingeräumt werden sollte [3].
