The Modified Sequential Treatment Regimen Containing Levofloxacin for Helicobacter pylori Eradication in Turkey

Abstract: Sequential therapy using "rabeprazole and amoxicillin 7 days followed by rabeprazole, metronidazole and levofloxacin for 7 days" is a new regimen with acceptable eradication rates in naïve patients in Turkey. Further modifications in the dose or duration of this new sequential therapy might increase its effectiveness as both first and second line treatment.

“…In treatment-naive patients, ITT and PP eradication rates were 82.5% and 86.7%, respectively. As a second-line treatment, eradication was successful, with ITT 75.7% and PP 80% (19). In the case of clarithromycin resistance, it was suggested that consecutive therapies comprising three antibiotics can increase the eradication rate (43)(44)(45)(46).…”

“…Therefore, an increasing number of patients actually require a second or further therapeutic attempt to eradicate the infection. The sequential regimen is a novel, promising therapeutic approach for Hp eradication (17)(18)(19). We aimed to report herein the outcomes of different Hp eradication regimens used in our institution over the last 10 years.…”

“…This may be related to the antibiotic resistance as a result of intense use for infections. Aydın et al [34] attained an eradication rate of 82.5% by ITT analysis and 84.2% by PP analysis with the sequential treatment with levofloxacin in patients naive to that treatment. The chance for cure is higher in populations where local resistance against levofloxacin is below 10% [29,30].…”

“…The first one was a national report that detected 90% eradication rate with 14 days rabeprazole, amoxicillin, and clarithromycin therapy in 10 patients (31). The second one was a modified sequential therapy (rabeprazole and amoxicillin 7 days followed by rabeprazole, metronidazole, and levofloxacin for 7 days) that reported 82.5% ITT and 86.7% PP eradication rates in the treatment of naive patients (32). So together with these results we can suggest that when using PPI-based triple therapy, it is more logical to prefer rabeprazole.…”
