RESUMO: Objetivou compreender os significados do processo parturitivo a partir da vivência do risco gestacional da mulher portadora de cardiopatia. Estudo qualitativo de abordagem fenomenológica, com coleta de dados entre julho e dezembro de 2014, em hospital para alto risco materno na Região Sudeste do Brasil. Dezessete participantes foram entrevistadas e os significados expressos analisados na perspectiva teórico-metodológica de Martin Heidegger. As mulheres significaram o medo de morrer no parto; e a preocupação com a sobrevivência do bebê, pedindo ao médico para dar prioridade em salvá-lo. Evidenciou-se o desvelamento do temor frente ao medo expresso, implicando em vulnerabilidade que cardiopatas vivenciam ao engravidarem. Isto porque, para além das possíveis complicações advindas da doença cardíaca, residem subjetividades que são apartadas do processo de cuidado em saúde, especialmente no que diz respeito à falta de diálogo e tomada de decisão unilateral acerca do tipo, momento e via de parto. MATERNAL HEALTH PROMOTION BASED ON THE BIRTH EXPERIENCE OF WOMEN WITH HEART DISEASEABSTRACT: The aim of this study was to understand the meanings of the birth process based on the experiences of high-risk pregnancies of women with heart disease. A qualitative study with a phenomenological approach, with data collection happening between July and December 2014, was developed at a high-risk pregnancy hospital in the Southeastern region of Brazil. Seventeen participants were interviewed and the expressed meanings were analyzed with the theoretical-methodological approach of Martin Heidegger. Women mentioned fearing death during birth; and worries about their infants' survival, asking physicians to prioritize their lives. The display of concerns facing the expressed fears stood out, involving vulnerabilities which patients with heart disease experience when pregnant. That is because, in addition to possible complications stemming from heart disease, there are subjective issues that are ignored during the healthcare process, especially related to lack of communication and one-sided decision making regarding the type, time and route of delivery. Objective: To analyze the existential movement of women with heart diseases in the context of Reproductive Planning.Method: Qualitative with Heideggerian phenomenological approach. Seventeen participants with heart disease and with recent experience of pregnancy at a reference institution for maternal risk participated in the study, through the phenomenological interview.Results: Two units of meanings emerged in the hermeneutical analysis: To be more attentive and concerned with the treatment because the problem remains the same; Expressing that they don´t want to get pregnant anymore because they´ve reached the limit and to seek planning in doing the tubal ligation. Conclusion and implications for practice: The unveiling of the existential movement made it possible to understand that, although the participants felt secure with the institutional follow-up of high-risk pregnancies, this proved to be insufficient with regard to Reproductive Planning. It stands out the necessary consideration by nurses and other health professionals that this planning is an important set of actions to identify and modify risks in women with heart diseases in the perspective of better perinatal outcomes. It is necessary to advance in the comprehension of the subjective aspects of the woman and the couple through the dialogue, besides the permanent education work of the whole team. Objetivo: Analisar o movimento existencial da mulher com cardiopatia no contexto do Planejamento Reprodutivo. Método: Qualitativo com abordagem fenomenológica Heideggeriana. Foram participantes 17 mulheres cardiopatas com experiência recente de gravidez em instituição referência para risco materno, por meio da entrevista fenomenológica. Resultados: Emergiram na análise hermenêutica duas unidades de significados: Ficarem mais atentas e preocupadas com o expressarem tratamento porque o problema continua o mesmo; Expressarem não querer mais engravidar porque chegaram no limite e buscarem o planejamento para fazer laqueadura. Conclusão e implicações para a prática: O desvelamento do movimento existencial permitiu compreender que, embora as participantes se sentissem seguras com o acompanhamento institucional da gravidez de alto risco, este se mostrou insuficiente no tocante ao Planejamento Reprodutivo. Aponta-se a necessária consideração por parte de enfermeiras e demais profissionais de saúde de que esse planejamento é um importante conjunto de ações para identificar e modificar riscos em mulheres com cardiopatia na perspectiva de melhores resultados perinatais. Deve-se avançar na abrangência dos aspectos subjetivos da mulher e do casal por meio do diálogo, além do trabalho de educação permanente de toda a equipe. Objective: to describe the perception of pregnant women and their relatives about the vulnerability of a highrisk pregnancy. Methods: qualitative research with eight pregnant women and ten relatives. Data were collected through semi-structured, audio-taped interviews, which, after transcription, were submitted to content analysis in the thematic modality proposed by Bardin. Results: three categories emerged: Experiences of relatives and pregnant women in relation to the diagnosis of a risky pregnancy; The family as the structuring axis of care in a high-risk pregnancy; and Invisibility of relatives in prenatal care. Conclusion: both the relatives and the pregnant women experienced worry, anxiety, fear and stress in the face of vulnerability, but the pregnant women perceived themselves to be more protected and safer with the support of the family, which although did not perceived to be valued by health professionals during prenatal consultations, plays an important role in encouraging and supervising care during pregnancy. Métodos: pesquisa qualitativa realizada com oito gestantes e dez familiares. Os dados foram coletados por meio de entrevistas semiestruturadas, audiogravadas, que após transcritas, foram submetidas à análise de conteúdo, modalidade temática proposta por Bardin. Resultados: emergiram três categorias: Vivências do familiar e da gestante frente ao diagnóstico de uma gravidez de risco; A família como eixo estruturante do cuidado em uma gestação de alto risco; e a Invisibilidade do familiar no acompanhamento prénatal. Conclusão: tanto familiares como gestantes vivenciam preocupação, ansiedade, medo e estresse frente à condição de vulnerabilidade, porém, as gestantes se percebem mais amparadas e seguras com o apoio da família, que embora não se perceba valorizada/considerada pelos profissionais de saúde durante as consultas de pré-natal, tem papel importante no incentivo e fiscalização dos cuidados durante a gestação.
