The dominance of a multiresistant strain of Neisseria gonorrhoeae among prostitutes and STD patients in The Gambia.

Abstract: Objective-To study the epidemiology of antibiotic resistant strains of Neisseria gonorrhoeae from sexually transmitted disease clinics in The Gambia. Materials and methods-One hundred and sixty five strains of N gonorrhoeae were tested for their antibiotic susceptibility, auxotype, serotype, and plasmid content.

“…In this study the gonococcal population was heterogeneous, although there were fewer serovars and A/S classes among the PPNG than the non-PPNG. We have encountered this phenomenon in other studies both in London, UK [25] and in The Gambia [26]. Serovar IA-6 strains were the most frequently detected in this study and have been reported as one of the predominant serovars in Kenya [24].…”

“…The isolation of penicillinase-producing Neisseria gonorrhoeae strains (PPNG) was described for the first time by both Philips (1976) and Ashford et al (1976). Thereafter, PPNG strains presenting an higher resistance to penicillin have been isolated and its number increased rapidly allover the world: Nigeria 3% in 1979 to 48% in 1984 (Lind, 1991); Senegal 21% in 1984 (Lind, 1991); Gabon 48% in 1984 (Ison, 1992); Zaire 59% in 1988(Van Dyck, 1992and Portugal 19.2% in 1985to 50% in 1995(Ferreira, 1997. These strains harboured the 4.5 MDa or 3.2 MDa plasmid in conjunction with or without the 24.5 MDa conjugative plasmid, defining the rapid diffusion of the resistance.…”

“…4 Appropriate and effective treatment requires updated information about susceptibility patterns. Because of the emergence of drug resistant strains like penicillinase producing N. gonorrhoeae (PPNG), plasmid-mediated tetracycline resistance (TRNG), and chromosomally mediated resistance to penicillin and/or tetracycline (CMRNGPT), the Centers for Disease Control and Prevention, Atlanta, has advocated the use of expandedgeneration cephalosporin or fluoroquinolones as the first line of therapy for uncomplicated gonorrhea.…”
