In all units where health care is provided, especially in intensive care units, infection-related complications are conditions that increase the cost and complicate the treatment process. In our study, in 2014-2015, the Ministry of Health Kartal Dr. Data of 1204 patients followed up in intensive care units of Lutfi Kirdar Training and Research Hospital were analyzed. We excluded the patients with antibiotic therapy at admission to the intensive care unit. Cases followed up in the intensive care unit for less than 24 h, and cases with culture detected in the first 48 h were also excluded from the study. Data from 287 patients with culture-positive infections of 743 patients who met the study admission criteria were analyzed. In diagnosing infectious diseases, laboratoryproven blood circulation infections were grouped as catheter-related urinary tract, central venous catheter-related bloodstream infection, ventilator-associated pneumonia, and burn infection. The hospital infections detected in our intensive care units and the determination of the causative pathogens were evaluated according to the differences in these infections according to years, units, diagnoses, demographic data, and clinical presentations. It was determined that hospitalization diagnoses, presence of comorbid diseases, and the percentage of burns were firmly related to the duration of ICU stay and mortality. Acinetobacter baumannii infections were significantly higher in our intensive care units compared with other microorganisms. We aimed to evaluate the data of the patients who were followed up in intensive care units of Dr. Lutfi Kirdar Kartal Training and Research Hospital between 2014 and 2015 with culture-positive infection retrospectively.

ABSTRACT This study aimed to determine the antibacterial and antioxidant activities of the newly synthesized Schiff base and to support the laboratory results with molecular modeling studies. Antibacterial activity of schiff bases was demonstrated using Gram (-) Pseudomonas aeruginosa and Acinetobacter baumannii bacterial strains. Minimum Inhibition Concentration (MIC) values were determined to evaluate their antimicrobial activity against Gram-negative P. aeruginosa and A.baumanni bacterial strains. In antioxidant experiments, the responses to DPPH and ABTS radicals were calculated at certain concentration ranges and graphs were drawn. For the molecular modeling study, Autodock Vina and Discovery Studio 2020 package programs were used. The observed bacterial inhibition activity was variable depending on the clinical isolate and the concentration of the tested samples. The highest inhibition activity was achieved at concentration of 75 μl -100 μl. N,N'-(ethane-1,2-diyl)bis(1-(9H-fluoren-2-yl)methanimine) samples. Molecular docking results show that N,N'-(ethane-1,2-diyl)bis(1(9H-fluoren-2-yl) methanimine) binds strongly to the 4ZIY and 4ZHU structures. It has been proven by molecular docking study that the synthesized Schiff base ligand has antibiotic resistance properties. N, N'-(ethane-1,2-diyl)bis(1-(9Hfluoren-2-yl) methanimine) synthesis compound showed moderate activity against A. baumannii and P. aeruginosa strains. It is known that schiff bases have strong biological activities and show antibacterial activity. In this study, it can be said that the synthesis schiff base showed antibacterial and antioxidant activity. In addition, our results were supported by molecular modeling. Our findings can be taken to a higher level with in vivo and in vitro studies.

Kan kültürü örnekleri BACT/ALERT 3D otomatize sistemi ile inkübe edilmiştir. Üreyen mikroorganizmaların identifikasyonu ve antibiyotik duyarlılıkları Phoenix otomatize sistem ile yapılmıştır Bulgular Çalışmaya dahil edilen 3719 kan kültürü örneğinin 2822'sinde (%75.8) Gram pozitif bakterilerin, 584'ünde (%16.7) Gram negatif bakterilerin, 180'inde (%4.8) mayaların ürediği görülmüştür. En sık izole edilen Gram negatif bakteriler Escherichia coli 227 (%36.37), Klebsiella pneumoniae 126 (%20.19) olarak bulunmuştur. Gram pozitif bakterilerin 2245'inin koagülaz negatif Staphylococcus türleri, 251'inin (%8.8) Enterococcus spp., 194'ünün (%6.87) Staphylococcus aureus olduğu saptanmıştır. Gram negatif bakterilere en etkili antibiyotikler karbapenemler, aminoglikozidler ve tigesiklin olarak bulunmuştur. Tüm Acinetobacter izolatları karbapenemlere dirençli bulunmuştur. Staphylococcus aureus izolatlarında vankomisin direnci saptanmazken metisilin direnci %55, koagülaz negatif Staphylococcus izolatlarında metisilin direnci %84 olarak bulunmuştur. Enterococcus spp. izolatlarında vankomisin direnci %9.7 olarak bulunmuştur Sonuç Sonuç olarak bakterilerin direnç oranlarının bu denli yüksek olması enfeksiyon kontrol önlemlerinin artırılması gerektiğini düşündürmektedir Anahtar Kelimeler AbstractObjective Blood stream infections remain a major cause of morbidity and mortality.
