Ten-year trends in CD4 cell counts at HIV and AIDS diagnosis in a London HIV clinic

Abstract: The observed decline in CD4AIDS value was related to the introduction of antiretroviral therapy in 1988, and PCP prophylaxis in 1989. Temporal changes in the CD4 cell count at HIV and AIDS diagnosis among different demographic groups can provide insights into the changing natural history of the HIV epidemic and access to medical care. We recommend monitoring of the CD4 cell count at new HIV and AIDS diagnosis and at initiation of antiretroviral therapy as additional measures in national HIV/AIDS surveillance.

“…Association of higher age groups of 35-44 years ages 45 years and above seen in our study with late and very late presentation to care has been emphasized in literature [19,22,23]. Homosexual transmission route has been reported to be associated with a lesser risk of late presentation in previous studies [23,24].…”

“…25 Literature on the topic emphasizes several factors that may be associated with delay in initiating therapy, among them, risk category (especially heterosexuals), age (especially older adults), sex (males, since women are often tested during pregnancy), as well as other cultural, political, and economic factors. 17,19,26 In Brazil, women of reproductive age are routinely screened for HIV during pregnancy, as recommended by the Ministry of Health. Detection of HIV infection during prenatal care also promotes the early detection of infected partners, generally in the under-50 age group.…”

“…In particular, increased CD4 counts over time may result from an increase in the number of newly diagnosed individuals with high CD4 counts attending the centres in the collaboration. However, results from the CD4 surveillance scheme in the United Kingdom12 and from individual cohorts in the collaboration13 14 indicate that this is not the case. A second concern is that as this is a dynamic cohort, patients may leave the cohort at any time as they move to centres that are not part of the collaboration.…”
