The utility of acoustic radiation force impulse (ARFI) imaging for real-time visualization of abdominal malignancies was investigated. Nine patients presenting with suspicious masses in the liver (n = 7) or kidney (n = 2) underwent combined sonography/ARFI imaging. Images were acquired of a total of 12 tumors in the nine patients. In all cases, boundary definition in ARFI images was improved or equivalent to boundary definition in B-mode images. Displacement contrast in ARFI images was superior to echo contrast in B-mode images for each tumor. The mean contrast for suspected hepatocellular carcinomas (HCCs) in B-mode images was 2.9 dB (range: 1.5-4.2) versus 7.5 dB (range: 3.1-11.9) in ARFI images, with all HCCs appearing more compliant than regional cirrhotic liver parenchyma. The mean contrast for metastases in B-mode images was 3.1 dB (range: 1.2-5.2) versus 9.3 dB (range: 5.7-13.9) in ARFI images, with all masses appearing less compliant than regional non-cirrhotic liver parenchyma. ARFI image contrast (10.4 dB) was superior to Bmode contrast (0.9 dB) for a renal mass. To our knowledge, we present the first in vivo images of abdominal malignancies in humans acquired with the ARFI method or any other technique of imaging tissue elasticity.

Background: FNAC has been widely applied in the diagnosis of tumors and tumor like lesion of the abdomen including the kidney. The aim of this study was to evaluate the outcome of FNAC to a sample of Iraqi patients with kidney tumor.Methods: Patients with clinical, radiological suspicion of renal masses underwent FNAC of the kidney under ultrasound guidance were invited to this study. Those who fulfilled inclusion criteria were recruited. FNAC was done in accordance with stipulated clinical practice.Results: 53 patients with clinical and radiological suspicion of renal mass underwent FNAC of the kidney under ultrasound guidance. The age ranged from 1-2 year to 70 years (mean 44.7 years) with male to female ratio (1.9:1). Around 94.3% had primary renal mass, 1.9% metastatic tumor to the kidney and the rest 3.8% was undiagnosed. Sensitivity was 90% while specificity was 100%.Conclusions: We conclude that FNAC of the renal masses is safe and reliable, and fast diagnostic procedure in the evaluation of patient with renal masses.

Purpose:To evaluate the effectiveness, cost, and cost-effectiveness of using renal mass biopsy to guide treatment decisions for small incidentally detected renal tumors. Materials and Methods:A decision-analytic Markov model was developed to estimate life expectancy and lifetime costs for patients with small ( Յ 4-cm) renal tumors. Two strategies were compared: renal mass biopsy to triage patients to surgery or imaging surveillance and empiric nephron-sparing surgery. The model incorporated biopsy performance, the probability of track seeding with malignant cells, the prevalence and growth of benign and malignant tumors, treatment effectiveness and costs, and patient outcomes. An incremental cost-effectiveness analysis was performed to identify strategy preference under a willingness-to-pay threshold of $75 000 per quality-adjusted life-year (QALY). Effects of changes in key parameters on strategy preference were evaluated in sensitivity analysis. Results:Under base-case assumptions, the biopsy strategy yielded a minimally greater quality-adjusted life expectancy (4 days) than did empiric surgery at a lower lifetime cost ($3466), dominating surgery from a cost-effectiveness perspective. Over the majority of parameter ranges tested in one-way sensitivity analysis, the biopsy strategy dominated surgery or was cost-effective relative to surgery based on a $75 000-per-QALY willingness-to-pay threshold. In twoway sensitivity analysis, surgery yielded greater life expectancy when the prevalence of malignancy and propensity for biopsy-negative cancers to metastasize were both higher than expected or when the sensitivity and specifi city of biopsy were both lower than expected. Conclusion:The use of biopsy to guide treatment decisions for small incidentally detected renal tumors is cost-effective and can prevent unnecessary surgery in many cases.q RSNA, 2010Supplemental material: http://radiology.rsna.org/lookup /suppl
