Significance of indeterminate third-generation hepatitis C virus recombinant immunoblot assay

Abstract: Indeterminate hepatitis C virus (HCV) third-generation recombinant immunoblot assay (RIBA3.0; Ortho Diagnostic Systems) patterns were arbitrarily defined by the manufacturer as the detection of only one antibody out of the four that were sought, namely, c100 (NS4 encoded), c22 (core encoded), c33c (NS3 encoded), and NS5 (NS5 encoded). The aims of the present study were (i) to determine the prevalence of indeterminate RIBA3.0 patterns in patients consecutively tested for anti-HCV antibodies in a university hosp… Show more

“…There have been many studies trying to clarify the significance of anti-HCV-indeterminate results, including the study of ALT levels, immunoassay S/CO ratios, RIBA band strengths, and risk factors. In this study, we aimed to compare the histological findings in this group with those who are RIBA-3 positive [15,16,23,24]. Those subjects with an indeterminate RIBA 3.0 result and a positive result for HCV-RNA showed minimal histological changes on liver biopsy.…”

“…All patients were subjected to a percutaneous liver biopsy under ultrasound guidance. The biopsies were all evaluated by the same pathologist and were classified according to the Knodell index, with determination of the grade of fibrosis [15].…”

“…Indeterminate reactivity against the c22 antigen was the rarest finding, present in approximately one-fifth of the donors. The finding of a c22 band in RIBA may be highly indicative of previous HCV exposure, [12][13][14][15] and donors with this serologic profile should probably be excluded even in spite of a subsequent seroreversion to RIBA negative.…”

“…39 RIBA-3 appears to be more specific than RIBA-2, with fewer indeterminate results. [41][42][43][44] Other supplemental assays are available, e.g., the Matrix test hepatitis C virus dot blot immunoassay 39 (Abbott Laboratories, North Chicago, IL)…”
