Rapid Assessment of Blood Pressure in the Obstetric Day Unit Using Microlife MaM Technology

Abstract: White-coat hypertension exists for systolic BP in the obstetric day unit. The MaM technology allows rapid and accurate characterization of blood pressure equivalent to repeated measures over 90 minutes.

“…every 15 min and then every 30 min in the initial phase of assessment, consistently with the Canadian guidelines [6]), and make decisions on the trend and preponderance of blood pressures. We did not address which device to use to measure BP but suggest mercury sphygmomanometry when available or sphygmomanometry using a liquid crystal device [7]. If an automated device is to be used then it should have been validated for use in pregnancy.…”

“…BP should be measured three times; the first value is disregarded, and the average of the second and third taken as the BP value for the visit [7]. Up to 70% of women with an office BP of P140/90 mmHg have normal BP on subsequent measurements on the same visit, or by ABPM or HBPM [14][15][16][17][18]. The timing of reassessment should consider that elevated office BP may reflect a situational BP rise, 'white coat' effect, or early preeclampsia [19,20].…”

“…Main characteristics of the device are reported in the appendix. The device features a 'MAM', which uses a 'weighted' average of a minimum of three consecutive BP readings taken 15 s apart [7]. A specific algorithm takes into account the change in BP between sequential readings, which influences the contribution made to the average of all readings.…”

“…The poor performance of wrist devices and the problems related to wrist BP measurement in general were highlighted by the results of a multicentre Japanese study, which showed that measurement of BP at the wrist is fraught with problems [6]. Recently, new algorithms that automatically provide multiple BP measurements have been produced which could minimize the errors of automatic monitors and identify and exclude white-coat hypertension [7]. The Microlife Average Mode (MAM) technology allows rapid and accurate characterization of BP which has been shown to be equivalent to repeated measures over 90 min [7].…”
