Nurses' perspective on health education in Diabetes Mellitus Care

Objective: to grasp the perspective of nurses on health education in the process of caring for people with Diabetes Mellitus in Primary Care. Method: a descriptive study of a qualitative nature carried out with 13 nurses from the Family Health Strategy in Southern Brazil. The data were collected in November and December of 2017, through recorded interviews, which were submitted to content analysis, thematic modality. Results: three categories emerged, which show from the perspective of nurses, the influence of structural and assistance characteristics in the development of educational actions; outcomes of these actions, and possibilities to increase their quality as central to the nursing action. Final considerations: there are still gaps in the structuring of public health policies, especially in the process of managing and caring for people with diabetes, which limits quantitatively and qualitatively the development of educational actions in Primary Care.

BackgroundThe success of primary health care relies on the integration of empowered practitioners with cooperative patients regardless of socioeconomic status. Using resources efficiently would help to improve healthcare promotion and reduce complications of chronic non-communicable diseases (NCDs). The importance of network support programmes relies on the fact that they allow to accurately deliver medical care by shaping a sense of community and purpose among the patients.AimTo evaluate the effectiveness of a network support programme for patients with type 2 diabetes mellitus (T2DM).Design & settingA centre-based observational prospective study took place in a primary care setting in Ecuador.MethodThe impact of the diabetes care programme was assessed by comparing initial and final metabolic characteristics and outcomes of 593 patients with T2DM, followed-up from April 2007 to December 2017, using paired sample t-test. Electrocardiograms (ECGs), ankle-brachial indexes (ABIs), ocular fundus, and monofilament neuropathy tests were assessed with the McNemar test to evaluate complications at the beginning and end of the study.ResultsGlycated haemoglobin (HbA1c), lipid profile, and systolic blood pressure (SBP) showed statistically significant decreases between the initial measurement (IMs) and final measurements (FMs). In the FM, significantly lower HbA1c, diastolic blood pressure (DBP), and atherogenic index were found. Despite the length of time since diagnosis, during the follow-up time, long-term micro- and macro-vascular complications, such as ocular fundus, serum creatinine, and ABI, remained unchanged throughout the period of active participation in this healthcare programme.ConclusionThis study demonstrates the feasibility of reducing plasma glucose, plasma lipids, and long-term complications in patients with T2DM by implementing a network support programme, which involves the medical team and patients themselves in an environment with limited resources.

O objetivo deste estudo foi conhecer as ações em saúde realizadas pelos Agentes Comunitários de Saúde às famílias de pessoas dependentes de cuidados. O estudo é de natureza qualitativa, realizado com 21 profissionais que integram equipes da Estratégia Saúde da Família de um município de pequeno porte, localizado na região noroeste do estado do Paraná. Para a coleta de dados utilizou-se entrevista semi-estruturada que foram submetidas à análise de conteúdo na modalidade temática. Evidenciou-se facilidades relacionadas ao vínculo criado com as famílias assistidas, a à equidade da assistência e a à receptividade dos usuários ao trabalho dos Agentes Comunitários de Saúde. Apontou-se Foram apontadas dificuldades na assistência, como baixa escolaridade, medo causado pela presença de drogadição nas áreas de abrangência e sobrecarga de trabalho. Na questão organizacional, a ineficiência dos serviços especializados, falta de transporte para as visitas e de materiais necessários para a rotina de trabalho foram considerados como fatores que impedem a realização de uma assistência mais adequada. Conclui-se que os Agentes Comunitários de Saúde, que acompanham as famílias de pessoas dependentes de cuidado, têm seu trabalho interferido negativamente pelas dificuldades encontradas no dia a dia, as quais necessitam ser improvisadas cotidianamente para que a assistência seja prestada.
