Pretransplant C3d-Fixing Donor-Specific Anti-HLA Antibodies Are Not Associated with Increased Risk for Kidney Graft Failure

Abstract: Complement-fixing antibodies against donor HLA are considered a contraindication for kidney transplant. A modification of the IgG single-antigen bead (SAB) assay allows detection of anti-HLA antibodies that bind C3d. “…One of the key differences between HLA molecules present on beads vs cells is that HLA‐molecules are immovably fixed to beads whereas on the cell‐surface, HLA molecules float in the phospholipid bilayer and can be clustered rapidly by HLA‐antibodies leading to a clustered formation of IgG Fc‐tails. On beads, the detection of complement fixation is therefore a parameter of the amount of antibodies bound, irrespective of affinity or precise specificity, which explains the direct relation between MFI values found when detecting IgG vs deposition of complement components .
