Results and reflections from the PROfiling Consortium on Antibody Repertoire and Effector functions in kidney transplantation: A mini‐review

Abstract: Kidney transplantation is the best treatment option for patients with end‐stage renal disease (ESRD). The waiting time for a deceased donor kidney in the Netherlands is approximately 3 years. Mortality among patients on the waiting list is high. The aim of the PROCARE consortium (PROfiling Consortium on Antibody Repertoire and Effector functions) was to decrease the waiting time by providing a matching algorithm yielding a prolonged graft survival and less HLA‐immunization compared with the currently used Euro… Show more

“…In particular, monitoring of pre-transplant as well as de novo DSA has become more widespread with an increasing number of studies focusing on the relationship between the presence of DSA and incidence of ABMR 42 45 . Although ABMR is often accompanied by the presence of DSA, this is not always the case, nor is the presence of DSA associated with adverse outcomes in all kidney transplant recipients 46 48 . While much remains unknown about the individual contributions of these risk factors, it is clear that donor-reactive immune memory can be difficult to detect and difficult to inhibit.…”

“…Preliminary data indicate that patients having two or more pre‐transplant DESAs, defined by taking all eplets listed in the HLA Eplet Registry into account, showed inferior graft survival compared with patients with zero to two DESAs 41 . However, 10‐year graft survival rate is ~50% in patients with ≥2 DESA.…”

“…An additional algorithm, Predicted Indirectly ReCognizable HLA Epitopes (PIRCHE-II), predicts in silico the number of HLA mismatch-derived epitopes that can be presented to helper T cells [ 24 ]. Nonetheless, the up-to-date existing data are still poor and more research is needed before wider application is feasible [ 25 ].…”
