Photodermatoses: classification, evaluation and management

Abstract: Photodermatoses are skin disorders induced or exacerbated by light. They can be broadly classified into four groups: (i) immunologically mediated photodermatoses (idioapathic); (ii) drug- and chemical-induced photosensitivity; (iii) defective DNA repair disorders; and (iv) photoaggravated dermatoses. The exact pathomechanism of those diverse skin reactions to light radiation remains unclear. Immunologically mediated photodermatoses are the most common dermatoses among all photosesnsitive disorders. The managem… Show more

“…Most phototests are applied on the upper back or on the buttocks, partly because these areas are quite flat, and partly because they are in general less exposed to sunlight (19,20). Although less common, however, application on the inner forearms is also stated to be appropriate (21). The choice of test location for this study was related to the selfreading approach utilized; this meant that an extended number of participants could be included because follow-up visits for test reading at the clinic was not necessary.…”

“…Notably, our case showed targetoid lesions of vesicles and blisters on normal‐appearing skin, whereas phototoxic reaction usually shows erythemas and vesicles with pain and stinging, and photoallergic reaction tends to result in pruritic eczematous eruptions with lichenification . Moreover, in phototoxic or photoallergic reaction, the MED for UVA decreases, and that for UVB is within the normal range . In contrast, our case showed hypersensitivity to UVB.…”

“…18 The SCENIHR reports 10,11 acknowledge that a small minority of individuals with PLE may get photoprovoked skin responses following artificial light exposure. The forearm is another site that is commonly used for phototesting, 22 and for convenience, this site was chosen for the patient's exposure to lamps. Based on an EU total population of 500 million of which PLE occurs in 20% this would equate to 100 000 affected individuals.…”
