Resumo O objetivo é analisar a inserção do trabalho do farmacêutico na atenção primária no Brasil. Foram realizadas buscas nas bases de dados BVS, SciELO, Lilacs e Medline, no período de 1998 a 2016. Dos 157 artigos encontrados, excluindo-se os duplicados, teses, dissertações e revisões e após leitura integral, foram incluídos 9 que tratavam do trabalho do farmacêutico e que relatavam experiências, atribuições, potencialidades, dificuldades e desafios para a prática profissional. Os resultados apontam produção incipiente e predomínio de estudos qualitativos a partir de 2007. A inserção na equipe, como espaço de qualificação das ações, é tema central dos estudos, que apontam desafios e dificuldades quanto ao reconhecimento e aceitação das intervenções do farmacêutico. As potencialidades estão no âmbito das ações voltadas para o usuário, as famílias e a equipe, assim como na formação profissional e na difusão dos resultados das ações farmacêuticas. Há predomínio do isolamento do farmacêutico na atenção primária, com perspectivas de fortalecimento da sua integração à equipe, que tem sido impulsionada pelas recentes mudanças institucionais e normativas no cenário nacional. Os pesquisadores focam nas potencialidades para a prática profissional, com o olhar para o futuro em construção.

This study was carried out to understand the perceptions and expectations of the other health care professionals about pharmacists' role in primary health care centers and hospitals in Santiago de Cuba (Cuba). A cross-sectional descriptive study was conducted, applying a self-administered questionnaire to health care professionals. The sample included 763 professionals (40.9% physicians and 59.1 % nurses) from hospitals and primary health care clinics, chosen by random stratified sampling,. The most common activity performed by pharmacists was the distribution of medications or the drug supply management (51.5%) and adverse drug reactions reporting (29.4%), activities performed daily by pharmacists. Pharmaceutical care practice was considered as poor by the professionals, and only 15.6% of them have collaborated in these programs in the past. For nurses and physicians, interprofessional communication occurs sometimes (56.9%) and never (25.8%). Pharmacists' integration into the health team was considered as fair (50.7%). A total of 70.1% perceived the pharmacist as a drug expert and 60.1% as a professional who provides medication counseling. The main expectations these health care professionals expressed were that pharmacists provide more education about medication (97.4%) and improve their interprofessional communication techniques in order to communicate more effectively with the other members of the health care team (96.6%).Uniterms: Pharmacists/professional functions. Interprofessional communication.O presente trabalho analisa o papel do farmacêutico na atenção primária de saúde e em hospitais, na percepção de profissionais da saúde na província de Santiago de Cuba. Um estudo descritivo transversal foi conduzido, através da aplicação de um questionário autoadministrado a uma amostra de 763 profissionais de atenção primária e hospitalar, 40,9% de médicos e 59,1% de enfermeiros. As funções farmacêuticas mais reconhecidas pelos profissionais desta amostra foram o fornecimento e distribuição de medicamentos (51,5%) e o relato de reações adversas medicamentosas (29,4%), funções desenvolvidas diariamente. A prática da atenção farmacêutica é limitada na percepção dos profissionais e apenas 15,6% têm participado em programas desse tipo. Comunicação interprofissional ocorre às vezes (56,9%) e nunca (25,8%), com maiores dificuldades na atenção primária de saúde. A integração do farmacêutico na equipe de saúde foi considerada regular (50,7%). Um total de 70,1% dos profissionais percebeu o farmacêutico como especialista em medicamentos e 64,1% como o profissional que aconselha sobre o uso de medicamentos. As principais expectativas colocam-se na necessidade de que o farmacêutico ofereça educação sobre o uso de medicamentos (97,4%) e reforce as suas técnicas da comunicação na equipe multidisciplinar (96,6%).Unitermos: Farmacêutico/funções profissionais. The aim of Pharmaceutical Care programs is to improve patients' quality of life, and such programs are particularly effective in the case of chronic diseases such as hypertension. The objective of this longitudinal study was to analyze a Pharmaceutical Care model for hypertensive patients receiving care within the Family Health Strategy (FHS). All patients were being seen by an FHS team affiliated to a primary healthcare unit in Goiânia, Goiás, Brazil. Fourteen patients participated in the study, with each patient receiving six home visits during the Pharmaceutical Care. Overall, 142 drug-related problems were reported, the most common concerning the ineffectiveness of treatment (33.8%). A total of 135 pharmaceutical interventions were performed, 92.6% of which involved pharmacist-patient communication, with 48.8% of these interventions being implemented. Cardiovascular risk decreased in three patients and remained unchanged in nine. In hypertensive patients with diabetes, fasting glucose levels were reduced in six out of nine cases. The Pharmaceutical Care model proposed here was effective in detecting drug-related problems and in proposing interventions to resolve or prevent these problems. Consequently, this may have contributed towards improving clinical parameters, such as fasting glucose levels and cardiovascular risk in hypertensive patients receiving care within the FHS.Uniterms: Pharmaceutical care. Primary health care.A Atenção Farmacêutica tem como objetivo conseguir resultados concretos que melhorem a qualidade de vida do paciente, contribuindo especialmente em doenças crônicas, como a Hipertensão Arterial. O objetivo deste estudo foi analisar um modelo de Atenção Farmacêutica a pacientes com Hipertensão Arterial assistidos pela Estratégia Saúde da Família (ESF). Trata-se de um estudo longitudinal realizado com pacientes assistidos pela ESF de uma Unidade Básica de Saúde de Goiânia-GO. Participaram do estudo, 14 pacientes que receberam seis visitas domiciliares para Atenção Farmacêutica. Registraramse 142 Problemas Relacionados a Medicamentos (PRM) sendo mais frequente a falta de efetividade do tratamento (33,8%). Realizaram-se 135 intervenções farmacêuticas, das quais 92,6% foram com a comunicação farmacêutico-paciente, em que 48,8% das intervenções foram implementadas. Observou-se redução do risco cardiovascular em três pacientes e nove pacientes não apresentaram alteração no nível de risco cardiovascular. Entre os pacientes hipertensos e diabéticos observou-se redução da glicemia de jejum em seis. O modelo de Atenção Farmacêutica proposto foi capaz de detectar PRM propondo intervenções para resolvê-los e preveni-los e, consequentemente, pode ter contribuído na melhora de parâmetros clínicos como glicemia de jejum e risco cardiovascular em pacientes hipertensos assistidos pela ESF.Unitermos: Atenção farmacêutica.
