“…6,13,14 CRMP-5 is expressed in the optic nerve, neurons of the central and peripheral nervous systems, oligodendrocytes, and the cytoplasm of SCLC cells. 5,6,10,11,[13][14][15] It has been suggested that autoantibodies generated against the neoplastic CRMP-5 react as a host immune response to normal tissues, resulting in various neuronal disorders.- 6,10,16 In our case, bilateral meningeal enhancement of the optic nerve was evident on MRI, indicating that the main pathogenesis of the optic neuropathy was bilateral perioptic neuritis due to the involvement of oligodendrocytes around the optic nerve. A 68-year-old male presented with blurred vision in both eyes. Ophthalmoscopy revealed bilateral prominent disc swelling and vitritis. No systematic neurological symptoms were observed. Magnetic resonance imaging revealed bilateral meningeal enhancement of the optic nerve. Small cell carcinoma was found, and antibodies against collapsing response-mediating protein-5 (CRMP-5) were detected in the serum. Ophthalmological manifestations disappeared during a decrease in tumour size with treatment for the malignancy. This case report describes this rare case of anti-CRMP-5 antibody-positive paraneoplastic perioptic neuritis without neurological symptoms, showing that prompt diagnosis and timely treatment of the underlying tumour are crucial to prevent increased levels of autoantibodies and irreversible damage to the nervous system. ResumenLos síndromes paraneoplásicos representan manifestaciones clínicas que producen los tumores en sitios distantes a ellos y que no están relacionadas físicamente con ellos ni con sus metástasis. Diferentes tumores gastrointestinales pueden presentar síndromes o manifestaciones sistémicas, dermatológicas, hematológi-cas, renales y neurológicas, entre otras. Esas alteraciones son independientes del efecto local del tumor, la invasión a otros órga-nos, los déficits nutricionales y las consecuencias derivadas del tratamiento antineoplásico (1, 2). La gran variedad de manifestaciones clínicas es secundaria a la liberación de sustancias por parte de las células neoplásicas malignas (2) que liberan hormonas, péptidos similares a hormonas, factores de crecimiento y citoquinas (1, 2, 3). Adicionalmente, también participan respuestas inmunitarias que, inicialmente, estaban dirigidas contra las nuevas sustancias o antígenos tumorales (oncoantígenos) y que, por reacción cruzada, terminan lesionando tejidos normales (3, 4) y, finalmente, acumulando complejos inmunes (5). Los diferentes SPN se dividen según el órgano o sistema al que afecten y, clásicamente, se clasifican en endocrinometabólicos, dermatológicos, hematológicos, reumatológicos y neurológicos (3). En la presente revisión, se describen los SPN producidos por tumores gastrointestinales (GI).

a b s t r a c tPurpose: To report novel optical coherence tomography findings in a case of anti-a-enolase cancer associated retinopathy. Observations: An elderly female presented with bilateral decreased vision and a recent diagnosis of ovarian carcinoma. Optical coherence tomography demonstrated bilateral loss of outer retinal structures and macular edema. Serum testing found antibodies against a-enolase and 82e84 kDa proteins. Outer retinal structures showed recovery, macular edema resolved and repeat anti-retinal antibody testing became negative following cancer therapy and topical difluprednate treatment. Conclusions and importance: Cancer associated retinopathy is a paraneoplastic disease that results in damage to retinal structures through an autoimmune response. The damage is generally considered to be irreversible; however, in rare cases, such as observed here, retinal structures may demonstrate recovery after treatment.
