Opacification of piggyback IOLs associated with an amorphous material attached to interlenticular surfaces

Abstract: Although the exact composition of the material between the lenses could not be established, hypotheses were advanced to understand the pathological mechanism associated with this condition. This case is different from those in previous reports of opacification composed of cortex and cells between piggyback IOLs.

“…72,73 However, we have recently received in our Center four new pairs of acrylic piggyback lenses explanted because of ILO. Three of these new cases shared the common aspect that exhaustive attempts to clean the interface between the lenses were not performed by the surgeons before explantation, probably because ILO is now a well-known entity.…”

“…The cases described in the two above-mentioned reports were considered as different forms of ILO at that time. 73 However, further analysis of the lenses explanted without previous attempts to clean their opposing surfaces and thus with the ILO components in situ helped us to understand its pathogenesis. 74 It shows that the classification of ILO in different forms may be artificial.…”

“…A segunda LIO implantada no nosso paciente foi de silicone exatamente para facilitar o seu explante. A utilização de duas lentes de acrílico está relacionada a uma maior incidência de fibrose e adesão entre as mesmas dificultando ou mesmo impedindo a retirada de apenas uma das lentes (13)(14)17) . O implante duplo "piggyback" em crianças com catarata congênita foi descrito pela primeira e única vez em 2001 (18) .…”

“…3,4 The presence of 2 IOL optics in the capsular bag raises concerns about the possibility of ingrowth of regenerative/ proliferative crystalline lens material between them, with formation of interlenticular opacification (ILO). [5][6][7][8][9][10] The aim of this study was to evaluate the outcome of opacification within the capsular bag, including anterior capsule opacification (ACO) and posterior capsule opacification (PCO), but focusing on ILO associated with 2 types of piggyback IOLs compared with the Synchrony.…”
